Employee was walking along a sediment control ditch over uneven ground to check a point source discharge at the outlet for this structure. Employee's foot slipped and employee's body twisted as employee fell to the ground. Employee broke right lower leg.

Security found a trailer that had just begun to burn. EE immediately called the fire dept. While they were responding EE began to use portable fire extinguishers. Upon arrival the fire departments were able to extinguish the fire. The fire burnt approx. 45 minutes. No injuries were reported MSHA Hotline was notified.

EE was assisting a driver in locating a popping noise they heard. EE told driver to raise & lower the bed, as EE bent over to look & listen EE inadvertently placed fingers on the lip at the bottom of the hoist cylinder as it was being lowered. EE's fingers were caught between cylinder and lip causing avulsions to each fingers from tip to nail bed. Soft tissue damage only to tips.

Removing a hardbar pin from a D-10 when a piece of metal from the pin hammer entered employee's left thigh when struck with a sledge hammer.

This office was notified on 07/09/2019 that the employee was diagnosed with occupational pneumoconiosis on 02/05/2019.

Excavator owned by WV Dept. of Highways caught on fire at the end of their shift that was located on our property. It was finally extinguished without injury. MSHA hot line was contacted. The excavator was a total loss. An investigation by MSHA was conducted on 2-21-19.

Employee's air drill was laying on top of a tool box of the welding truck and was not in use. As they walked by the drill, their leg brushed against the supply hose for the drill. It knocked the drill off and by reflex they grabbed the drill. The bit struck their hand causing a laceration to the palm of the left hand.

While walking across a muddy bench the employee twisted right ankle. On 2/16/2019 ee sought medical attention and was put on modified duty with a sprained right ankle

EE driving on site, fell asleep, hit guard rail, drove straight to Gate. Another EE was working gate. Area mgr & Ops. mgr. contacted. Area mgr. went on site. Ambulance called to check EE. EE did not want to go to hospital, changed mind, and decided to go; complaining of stomach pain. EE went hospital for evaluation. EE released with no restrictions on 10/28/2018.

As a lightning storm passed over the mine a bolt of lightning struck our incoming power. Power was lost. It was found that a bank of transformers had been struck and one was burning. The fire was extinguished but rekindled later. At that time both Hot Lines (MSHA and State) were notified .While talking with MSHA I was notified that the fire had been extinguished.

Removing a bad bushing from a D10 back bar. Employee hit the bushing to shift it over so that a retaining ring could be removed. Grease and metal slivers came back and hit the employee in the eye.

Employee was dismounting a 16G grader after working on the fuel linkage. Employee was coming down the steps when they stumbled and fell to the ground landing on the left hand (approximately 2'). This resulted in a hairline fracture to the forearm just below the elbow.

Employee was operating a riding lawn mower when ee came down the bank and began to turn to go back up. As ee turned one of the front wheels came off the ground and the mower began to turn over. EE then released seat belt and jumped off. When doing this ee struck left wrist on the mower and this resulted in two fractures to wrist.

Employee was walking back to lube truck after servicing an excavator when ee stepped and twisted left knee.

Employee was striking a bucket tooth to remove it from the shank with a sledge hammer when a piece of metal broke loose from the tooth lodging in the employee's lower right leg. The injury resulted in 6 stitches.

Employee was removing a hydraulic hose that had been disconnected on both ends and the clamps were removed from a 785 Caterpillar rock truck. As employee pulled they felt a pain in the upper back. Hose measured 36" long X 1.5" diameter.

Walked by another mechanic that was using a cutting torch, a piece of slag flew and hit employee in the middle of the back. When employee tried to wipe the slag off it caused a thermal burn to the middle finger of the right hand.

Employee was in the process of hooking up a catheads on a Highwall Miner car. As the car started, ee bent over to pick up a control cable and place it on the car. As ee stood up and began to walk ee stumbled and fell. This resulted in receiving a laceration to the right forehead. It is unknown as to what ee struck head on.

Employee was lifting the door on loader to check the transmission fluid and as ee pulled the door, it got stuck causing to strain to open it. This resulted in a hernia.

Employee called and informed management that ee had saw a doctor and had been taken off work having been diagnosed with carpal tunnel of both hands.

Employee was in the process of tightening a bolt on a pull down cable on a drill when EE felt a pull in EE's lower abdomen where EE had a previous hernia two years ago. After going to a doctor EE was diagnosed with another hernia in the same area as EE's last one. Employee continues to work as of this date. Management was not notified as to EE's diagnoses until 10-25-2017

Driver exiting and only caught bottom step with their toes. Their foot slipped off and sprained ankle when they hit the ground.

Carpal tunnel diagnosis

Employee was replacing a bearing on a highwall miner add car, when the bearing that employee was turning suddenly turned pinching employee's left index finger. Employee was wearing protective gloves. This resulted in an avulsion to the tip of employee's left index finger.

Employee stepped off the stairway onto the ground. When ee stepped with left foot, it slipped in the mud and ee felt a "pop" in knee. EE continued to work until 5/25/2017 when ee was diagnosed with a torn meniscus.

Employee slipped dismounting a dozer and twisted left elbow. Resulting in a strain/sprain.

Employee was greasing the bucket on the Caterpillar 5230. Employee had left hand on the grease hose when a small hole ruptured in the hose. Grease struck left thumb and broke the skin. Not penetrating the tissue.

Employee was walking from a drill to service truck and stepped on some un-even ground twisting right knee.

Employee was dismounting water truck when they stepped on a rock and the rock gave way causing EE to twist ankle. EE continued to work performing regular job. During a follow up visit the doctor requested physical therapy which began on 8-5-2016 causing this incident to be reportable. EE continues to work while attending therapy in the evenings.

EE was cutting apart some CB actuators with a knife. EE was cutting away from body but was holding material with left hand. The knife slipped and struck left wrist. Resulted in a stab wound in left wrist, resulting in 3 stitches.

EE was operating a 993K loader loading binder. A piece of binder broke loose. The operator felt a tweek in lower back. EE did not receive medical treatment until 12/07/2016, the first treatment. EE was off work at that time with Meniers disease. Management thought that EE was still of work for the Meniers disease and not for the back injury up until 02/17/2017.

Employee was operating a Hitachi excavator (EX-0102) repairing access road berms when the excavator caught fire. EE was informed by a passerby, rock truck operator. The employee shut the excavator off and exited. The fire was extinguished approximately 25 minutes later. No injuries occurred due to the incident. MSHA was called in a timely manner.

EE was unhooking an Add car cathead. EE placed EE's foot on the top of the cars push arm. EE's foot slipped off and was caught between the push arm and the flipper jack. EE quickly removed EE's foot and received a stretched tendon attached to a bone in the top of EE's foot. Medical evidence did not revealed injury until EE received an MRI and it was read (4/22/16).

Employee had exited the mantrip at the end of EE's shift and was in walking across the parking lot when another employee, in their personal vehicle struck EE, knocking EE to the ground. Employee was able to get up with assistance.

EE stated that while he was grinding on a weld Saturday he felt like dust in his eye but thought he wiped it out. He finished his shift and Sunday morning he woke and felt something in his eye. He went to the eye doctor on Monday morning where the dust was removed and he was prescribed antibiotic drops for his eyes. He returned to work the next shift with a full release.

Employee was lifting a front differential up, cradling it close to her body and felt a burn in her mid-section and a pull in her ankle. She was examined by physician on 8/12/15 and diagnosed with a hernia.

Employee was operating a stand-up battery powered forklift in the warehouse. While backing up, employee steered the forklift in the wrong direction causing it to strike a steel pole. Employees left foot was caught between the forklift and the pole smashing the distal end of his foot from the side resulting in fractures.

Employee was using a come-a-long and the truck's boom to install a new steering cylinder, the come-a-long slipped causing the cylinder to abruptly move forward, smashing employee's left thumb against the frame and steering cylinder of the CAT 992D (fracture and 3 sutures).

Employee was preparing to cut a tree when a limb fell and hit him knocking him to the ground.

Employee tried to open the driver's door of the lube truck but it would not open, so he entered through the passenger door and tried to open the driver's door. As he pushed down on the door handle, the handle broke in half causing a laceration to his left hand. (14 Sutures)

EE was using a 16 pound sledge hammer to straighten a metal guard. As he swung the hammer, he felt a pop and pain in his left shoulder. EE continued to work his scheduled shifts but due to continued discomfort he saw a physician on 01/09/15 and received an MRI. Diagnosed with 2 tendon tears. EE returned to work until 02/04/15, which was his first day away from work.

While working on the Tractor's air conditioning system, the employee stepped down to dismount the truck and slipped causing him to strike his head against the garage door frame resulting in a lacertion. Employee received 4 sutures.

While helping mechanic replace the tracks on dozer, employee had is hand on top of the tracks. When boom started to raise the tracks to connect them, employee's hand was pulled up, pinching his left little finger between the tracks, resulting in a laceration (10 Sutures).

Employee exited the tractor cab on the passengers side facing outward away from the truck and was dismounting, when he missed the last step causing him to fall onto the concrete floor, straining his groin. He was seen at the ER and excused from work.

Stepped out of bulk truck slipped and fell on ice twisting left knee. Sought medical attention on 2/28/14 full release. Seen specialist on 3/13/14 full release. Still having pain had MRI 5/23/14 was put off for surgery on 6/2/14 with a torn meniscus.

On 02/04/14 employee was dismounting a D-11-R Dozer and as he stepped off the push arm while still holding the handrail, he twisted to turn his body when he felt and heard a pop in his right shoulder. He continued to work but due to continued discomfort, employee decided to seek medical attention on 02/19/14 and has been excused from work.

We were prying on a drive line yoke on a 993K loader, the bar slipped off of the spider and struck me in the face hitting my right cheek..

Employee was operating and descending a loaded tractor and trailer on the Laxare haul road when the driver lost control at some point causing the tractor and trailer to overturn onto its left side in the ditch. Driver was extricated by mine personnel and transported to the ER via EMS.

Employee heard noise, as he was walking in surface area near highwall miner, he tripped on pile of coal & fell, hitting head on miner.

Employee has been diagnosed with Bilateral Carpal Tunnel Syndrome due to repetitive motion while operating a coal truck.

Employee has been diagnosed with Occupational Pneumoconiosis without pulmonary function impairment.

EE STATES WHILE TIGHTENING LUG NUTS ON THE TOP OF A WHEEL WHILE STANDING ON THE GROUND AND HOLDING THE AIR GUN ABOVE HIS HEAD HE FELT HIS BACK POP BETWEEN HIS SHOULDER BLADES AND THE BASE OF HIS NECK. NO PAIN INITIALLY PAIN STARTED AFTER HE STOPPED MOVING AND WAS SITTING IN TRUCK

Employee was trying to break loose a bolt on a D10 hoist jack when he began to experience neck pain. He seen a physician later that day and was diagnosed with a cervical strain and excused from work.

Employee was removing the belly pan on a D11R dozer when the belly pan fell hitting his right leg. He required 2 Sutures.

Employee was moving an empty compressed gas bottle into the bottle rack when the bottom of the bottle slid unexpectedly. In trying to control the bottle he suffered a strained/contusion of the rib.

EE WAS CUTTING LINER FROM TRUCK BED PLATE. THE EMPLOYEE CUT THE TOP, BOTTOM AND END OF THE LEFT FACING OF LINER. THE EMPLOYEE WAS IN THE PROCESS OF CUTTING THE PLUG WELD WHEN THE PLATE ROLLED OUT IN A SNAPPING MOTION STRIKING THE EMPLOYEE ON THE LEFT LEG BREAKING THE SMALL BONE IN LOWER PART OF LEG.

Employee was installing a hydraulic hose on a D-11 Dozer; in the process of bending the hose, it slipped hitting him in the mouth causing a laceration to his upper lip. He received 1 suture.

Employee was using a grinder when he got metal shavings in his left eye. He continued to work his shift but the following day, he went to see a physician and was prescribed an antibiotic eye drop. He returned to work.

Employee slipped on icy steps outside of office building causing him to twist his right foot/ankle resulting in a fracture of his foot/ankle. He was treated and transported by EMS to the ER. He has been excused from work.

Injured employee was in the process of installing cutting edges on a D11R dozer, while lifting the cutting edge with a 6ft. nylon strap rated for 1,550 lbs, the strap broke causing the cutting edge to fall on the employees right foot. He was transported to the ER and diagnosed with multiple foot fractures and required surgery the following day. He has been excused from work.

Injured employee tripped over a loader bucket tooth and fell to the ground on 08/06/12 injuring his right shoulder. He was seen at the ER on the day of the incident and released to return the work the following day. Due to continued discomfort of the injury, the employee was re-evaluated and excused from work on 11/01/12.

Was sliding a front axle under a 992D using two booms on mechanics trucks. One strap broke and the clevis on the end struck him in the right cheek. Axle was maybe 1 inch off ground.

Employee was conducting the Pre-Op on a truck when he stepped on a piece of material causing him to fall and injure his left leg. He was seen by the ER physician and excused from work.

Employee was conducting the Pre-Op check on the truck when the dirt/ground gave away causing him to fall and injure his right knee. He was seen by the ER physician and excused from work.

Employee was cranking the handle on the water hose reel when he felt his right shoulder pop, causing pain. He was seen by the ER physician and excused from work.

While climbing down from equipment, employee's ring, on left ring finger, got hung on handrail, causing ring to cut into finger requiring 7 sutures. Employee returned to work the following day.

While loading muffler with a forklift into truck bed, the muffler got stuck on warehouse door, and as employee pushed against muffler, his right hand was smashed, resulting in simple fracture to his right ring finger.

Injured employee was assisting with the removal of a transmission from a Kenworth T800 Tandem truck when his right ring finger was smashed between the drive lines causing a laceration and fracture. He was treated at the ER and released to return to work. No sutures required for the laceration.

Injured employee was dismounting a D-11 Dozer when he slipped and fell causing a 2nd degree burn to his hand and bilateral knee pain. He was seen at the ER and released to return to work by the ER physician. Injured employee return to his physician on 11/28/2011 for continued knee discomfort and was excused from work on 11/28/2011.

Injured employee was attending a safety meeting when he became hypoglycemia and passed out. He struck his head on the floor causing a laceration on his forehead above his left eye. He was treated and transported to the ER where he received 6 sutures.

Injured employee was dismounting surface drill on 11/04/2011, when he stepped on a rock and twisted his left ankle. He went to the doctor and was released to return to work. He continued to work his regularly scheduled shifts until 11/16/2011, that's when he went back to the doctor for continued discomfort. He was diagnosed with a sprain and excused from work on 11/16/2011.

A section of the highwall approximately 50 feet wide, 25 feet long and 10 feet thick fell onto the front of the AddCar Highwall Miner, causing a stop in normal production activity for longer than one (1) hour. There was no injury to persons as the pit had already been evacuated due to the wall moving.

Recoil on seat on ET-2102 bad causing a strain on lower back. Did not go to hospital until 9/28/11.

Coming down ladder of Ex5500, muscles in back of both arms and center of back began hurting and become sore. Did not go to the doctor until 8-26-11.

On stepladder leaning against rear of excavator looking for oil leak when machine shook the ladder and caused him to fall and striking the ground.

Employee fell from the second step of a Caterpillar 777 Water Truck resulting in a strained knee. On 8/25/2011, employee was diagnosed with a torn ACL in his left knee. He has been placed off work until surgery on 9/12/2011.

Operating Komatsu 830E electric rock truck when an unknown object flew into his right eye. His eyes were flushed with eye wash and transported to hospital. He was diagnosed with a slight scratch on the eye and given ointment to treat the scratch. He was released back to duty with no limitations.

While another employee was using a hammer to install bucket tooth on loader, employee was watching. A piece of metal came off hammer or pin & struck employee in right eye.

Employee was mounting a Volvo A40D Articulating Truck when he felt and heard a loud pop come from his right leg. He experienced pain and could not bear weight on his right leg. He was transported to hospital and given a preliminary diagnosis of a torn muscle and referred to a specialist. He was administered morphine for pain.

A fire did occur on mine property involving an over turned Kenworth Tandem coal truck. The fire lasted for approximately one (1) hour before being extinguished. There was no personal injury involved with this accident.

EE was traveling to the pit when he ran through a rough spot in the road causing the loader to bounce. He then felt a sharp pain in his lower back. He continued to work until 1:30pm and was transported to the hospital and was treated for a strain and released. On 5/23/2011 he was sent home because he could not perform his duties as a loader operator.

Member ate lunch at 12:00pm, by 2:30pm he had become very ill and was transported to hospital. He was diagnosed as having food poisoning. He was given intravenous fluids and released.

Member was steam cleaning on a bulldozer. As he pulled the trigger on the steam wand, the force of the released water caused him to fall off the bulldozer to the ground. He stuck his right hand out to break his off, causing injury. He was transported to hospital and diagnosed with a sprained wrist. A brace was applied and he was released back to work.

Member placed his right foot on the addcar to unhook the cathead. When the miner operator saw the cable get tight he trammed forward causing the member to get his right foot wedged between the jack and the car. This caused three (3) hairline fractures in his right foot and was hospitalized overnight for observation.

Member was zip tying a light to his hard hat when the knife cut through the zip tie, causing a laceration to his right thumb.

Employee was dismounting truck to scan card at scale and slipped. He fell onto the catwalk, he stuck his right hand out to catch his fall and received a laceration to his right hand that required 5 sutures to close the wound.

Employee slipped on a set of steps but did not fall. He began to complain with lower back pain. On 12/9/2010 he was seen by a specialist and released back to work with no injury listed. The em ployee did not return to work and sought a second and third opinion and was placed off work by a clinic doctor. Employee did not have contact with employer until late April 2011.

Employee was operating drill and looking out to find flag marker on the hole. As he closed the door, he got his right hand caught in the door causing an avulsion to his right middle and ring fingers that required a total of 10 sutures.

Employee raised his boom on a service truck with his left hand caught between the hook and the bumper of the truck. He completed his shift and then went to x-rays which showed a fractured bone in his left ring finger.

Employee was putting right angle chain back together after removing links to tighten chain. He was using a 4 pound hammer when he felt pain in his left hand. He removed his glove to find his left thumb bleeding and could not move his left thumb. Surgery was required to remove a piece of metal which required 5 sutures.

Employee was attempting to hook up a Highwall miner dolly to a truck. The tongue broke loose, fell to the hitch, causing his right hand to be pinched between the tongue and hitch of the truck. He was admitted to the hospital and on 9/23/2010, surgery was required to amputate his right index finger.

Employee was assisting in replacing a bucket lip. As he was putting the edge against the underside of the bucket so it could be welded, one end fell and pinched the employee's right hand between edge and cement pad.

Member was removing a tire from the bed of a pick-up truck when his foot slipped off the rear bumper of the truck. As he fell to the ground, he attempted to catch his fall with his right hand. He then felt pain in his right hand and was transported to the hospital, where he was diagnosed with a fractured hand. A cast was applied to his right hand and he was released back to duty.

Stockton coal stockpile was smoldering with flames.

EE was helping another member perform maintenance on a coal truck. EE struck his right thumb with a hammer causing a laceration that required suturing.

Employee was jacking a truck up when the jack kicked out striking him in the right hand. he felt slight numbness in his fingers but continued to work the remainder of his shift and thereafter. On 9/30/2010 he had a minor surgery to repair a nerve in his right hand. The surgery required 5 sutures to close the incision that was made.

EE was assisting mechanic in pulling a 773 coal truck from the ditchline. He started walking across the road to help drag the cable, when he fell to the ground. He was tranported to hospital and diagnosed with a right fractured wrist.

He was changing ladder on excavator, when he fell off the ladder to the ground. He was transported to hospital and diagnosed with a sprained left ankle. On 03/01/2010 he returned to doctor and re-diagnosed with a fractured left ankle.

Employee was cutting a tree, as the tree was falling, it brushed against another tree causing a limb to fall back, striking the employee in the left shoulder.

Member was fueling tandem coal truck, when he finished, he climbed up on top step to inform driver he was done. Member lost his balance and fell backward to the ground, striking head on ground, causing laceration that required six staples.

Employee was stemming holes, pushing spoil in with his feet. Left knee "popped". Employee walked it off, but on the following Monday (8/24) while greasing truck, knee went out again, causing "unbearable" pain. Employee went to Charleston hospital.

EE was installing a fuel tank on a tractor inside the truck shop, when lightening struck a nearby transformer on a power pole. The electrical charge traveled down the power pole and disbursed out through the ground, making contact with EE. He was transported to CAMC General Hospital, evaluated, given fluids as a precaution and released back to duty.

Injury was changing a valve insert in Komatsu 930 E Rock Truck when a bolt of lightening struck the ground approximately 60 feet from him. The electricity traveled through the ground and entered his body at the left shoulder and exited through both of his feet causing burns. He was transported to CAMC General Hospital, evaluated, monitored and released.

EE was holding a cylinder while co-worker was attempting to insert a retaining pin. Co-worker lost his grip on the pin and dropped it. The pin struck EE in the face causing a laceration to his upper lip and striking against his left wrist. Six(6) Sutures were required to his upper lip and a small chipped bone in his left wrist.

As driver exited cab of truck, foot slipped from step, struck lower back on step. Late report, unable to contact EE, injury may be contrived based upon the mechanism of injury. It appears that he exited the cab forward.

He had walked down three steps, upon reaching the ground level, he stepped on a rock and lost his balance and fell. He put out his left hand to break his fall and cut the palm of his hand on the rock. Seven stitches were required.

Employee was walking towards truck garage and slipped. As he was falling, he grabbed a hold of an unfinished tin building and cut the palm of his right hand.

Employee was trying to free a tire from a block of ice. The ice broke free and struck the employee's hand(R) and fractured his middle and ring fingers.

Employee was walking back to his equipment and slipped on a patch of ice. He complained of pain in his Left Foot. He was transported to the hospital, by ambulance, where the x-rays showed a fractured bone in his left foot.

Employee was grinding welds to reinstall blocks. He thought that he suffered from flashburn and later found that a piece of metal was in his right eye. The metal had to be surgically scraped to be removed.

Fleet mechanic working on roof top a/c unit checking for power to fan that would not run. Touching wire connector w/probe and fan started, striking finger.

Employee stated he was getting out of his truck and twisted his right ankle. He stated this has happened before.

Operator was mounting equipment when he heard and felt a pop in his left shoulder.

Employee fell from equipment during repair. Employee injured back.

EE was filling a hydraulic oil tank on a 992c loader. He climbed down the ladder far enought to see the sight glass, while on the ladder a step broke loose and caused him to fall to the ground.

Employee was cleaning his windshield and lost traction on steering tire. His left leg hit something making a gash in his lower left leg.

Two members were loading a tire in the back of a service truck when one of the member's lost his grip and let go of the tire. The other member maintained a hold on the tire and felt pain in his lower abdomen.

Employee is a Long-Haul Driver making a delivery and was on top of tanker using a squeegee to push the remainder of product (emulsion) out of truck. After finishing the job he attempted to stand upright and back began hurting. Strained back.

EE was mounting a doxer when his footing slipped and he fell, while holding onto the handrail. EE complained of lower back pain at the end of his shift, EE was diagnosed with a sprain to his lower back.

EE was operating a Caol truck and exiting the pit, he came ti a stop with the truck off balance and leaning to the right, the bed turned over on the right side, raising the cab in the air. EE suffered a lower back and neck strain.

Member was attempting to adjust seat on 988F Loader when his fingers were cut on the adjuster lever. Member received laceration to the index, middle and ring fingers on the left hand.

Driver stated that unit jumped out of 2nd gear descending on haul road. Laden truck gathered speed as he attempted to re-engage. Jake brake was ineffective as were foot brakes because of frozen sand/dirt on roadway. While negotiating a curve the truck rolled onto its right side, rolling over both a sump barrier and berm, falling into a ditch 20' - 30' below road surface.

Driver stated that unit jumped out of 2nd gear descending on haul road. Laden truck gathered speed as he attempted to re-engage. Jake brake was ineffective as were foot brakes because of frozen sand/dirt on roadway. While negotiating a curve the truck rolled onto its right side, rolling over both a sump barrier and berm, falling into a ditch 20' - 30' below road surface.

Member was driving water truck down Laxare haulroad. Member said his brakes were not holding and he drove the truck into a sump. Member complained of pain in his right hip.

Employee was kicking dirt over down lines when he felt a pain in his right knee. He reported the pain but did not seek medical attention until 1/18/08 as the pain increased.

Member was riding off property when another vehicle slid in a curve and struck his vehicle. Member complained of pain left arm and abdomen

Member was driving off property when another vehicle slid into his lane hitting his car. Member complained of pain in his right arm and neck

Employee was walking across parking lot and tripped on a crib block. Employee fell to ground landing on his right shoulder. Employee continued to work until 11/6/2007 when his Dr. took him off work.

At about 8:50 am a 930E rock truck caught on fire. MSHA was notified of the fire at 9:00am. Local fire departments responded to the fire. The fire was extingushed at 2:30pm. The 930E rock truck received extensive fire damage.The driver of the truck was taken to a local hospital to be checked out. He was released with no injuries. The driver returned to work on 8-9-2007

Employee felt pain in elbow after day of stemming holes. Worked the next day, pain grew worse. Took 8/10/07 off. Initial Dr. visit was on 8/14/07. Diagnosed as "tennis elbow".

Driver braked on flat ground at #17 Marker. Started out in 1st gear, shift to 2nd, as he was breaking over crown to drop off never heard EE brake hit or engine rev up. Passenger asked if he was in gear, & trainee grabbed shifter & wiggled it. It was in neutral, another EE told Inj EE to put it in ditch.

Employee was checking engine oil on rock truck. He was stepping back, tripped and fell backwards into the trucks handrails striking his shoulder and neck. The employee was not taken off work until 3-14-2007.

Driver stated he got into berm and went into ditch while driving the coal truck.

Employee was walking down steps on an 785 rock truck. He slipped coming down the steps hurting his left shoulder. Employee continued to work until April 26th when he was taken off work.

Employee was trying to raise hood on a portal bus. The hood latch broke and the hood fell sideways causing the employee to feel a sharp pain in his lower back. The accident was discovered in an internal audit as not being reported.

Driver lost control of vehicle on steep grade and drove into sump, overturning vehicle to avoid other traffic. Investigation on-going as to reason for loss of vehicle control.

Employee alleged while getting into coal truck he slipped and fell injuring his neck and back.

Employee was driving the explosive truck. He was backing the unit into position to lead blast. Truck backed into soft shoulder and overturned.

EE was a passenger in the explosive truck. Driver was backing unit into position to load blast. Truck backed onto soft shoulder and rolled over.

Driver alleges while closing tailgate, his hand slipped off tailgate and tailgate fell on middle finger of right hand.

Employee was standing on the car loading side of the launch. The miner was starting into hole #738. Injured stated that he took a step backwards to regain his balance when he fell about five feet to the ground, landing on his left shoulder.

Employee was removing a jack stand from underneath a coal truck, when the pin on the jack stand worked out, allowing top of jack to slide down pinching the right ring finger.

EE was using the boom on his service truck to pull the Tram Motor Guards off of the 3500. The guard came forward and slid down the hoses. EE threw up his left hand to stop guard. The guard had a sharp corner and lacerated EE's hand.

Employee was ascending a 6' step ladder. While on the third step, the ladder shifted causing him to fall to the ground on his right side. The fall resulted in a strain/bruise to the upper right hip.

while removing a pin from an addcar with his right hand, he dropped the pin on his left thumb crushing his thumb.

Employee slipped and fell on the High Wall Miner Launch. The ring and middle fingers on his right hand were caught in the grating, resulting in the fingers being fractured.

Employee was troubleshooting the contactors, when the miner was energized the contactors blew up, causing flash burns to ee's eyes.

Employee was attempting to remove the springs from the coal truck when fingers were caught between the axle and springs.

Employee was mounting rock truck when hand rail gave way. Employee fell approx. 3 ft and landed on his lower back. Employee did not begin to miss work until 2-15-06.

EMPLOYEE WAS HELPING TO MAKE A SPLICE IN A RUBBER HOSE WHEN THE KNIFE HE WAS USING BROKE. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER.

PULLING FUEL HOSE THROUGH MUD STRAINED GROIN AREA - APPROACHING DOZER. DELAYED TREATMENT UNTIL 1-2-06.

Injured was tramming toward stacker when the loader he was operating hit a hole and jarred the operator.

Unloading equipment and flag pole came out. Employee did not notice and jabbed flagpole in eye.

Operating excavator, building temp. access road over to bench on refuse impoundment. Trammed onto rocks thought to be stable. When he picked up another bucket of material & started to swing around to place the material, the rocks under the tracks gave way & allowed the sides of the tracks to dig in. Excavator tipped over & rolled down the slope. Laceration to forehead.

INJ BENT DOWN TO PICK UP A CRESCENT WRENCH, AS HE STOOD BACK UP HE TURNED TO THE LEFT. THIS CAUSED A P0PPING NOISE IN HIS LEFT KNEE. CONT'D TO WORK UNTIL 8/22/05. TAKEN OFF WORK BY DR FOR TEAR OF MINISCUS AND CARTILAGE IN LEFT KNEE.

MEMBER WAS INSTALLING PIN TO COUPLE HWM CARS ON AN ADDINGTON HWM SYSTEM. AS HE INSTALLED THE PIN, HIF FINGER BECAME CAUGHT BETWEEN THE CAR FRAME THE LIP OF THE PIN. THIS RESULTED IN 2 SUTURES AND A FRACTURE TO HIS LEFT INDEX FINGER.

Employee was leaving work in his personal vehicle. He was involved in a two vehicle incident involving a coal truck. Employee was diagnosed with whiplash.

Employee had come off of the loader he was operating to help remove rock from on top of the miner. He was pulling rock off of the side of the miner when his left hand was cut on a piece of slate rock. This resulted in 6 stitches.

WHILE INSTALLING BOLTS EMPLOYEE LOST BALANCE AND STRUCK HEAD ON PUSH ARM OF D9 DOZER.

Employee was helping to change the drive line in the Tandem truck he was operating. As he was pushing the drive line into the yoke he sprained his right thumb.

EMPLOYEE WAS LIFTING BAGS WHILE LOADING A BLAST. HIS FOOT SLIPPED CAUSING HIM TO TWIST. HE EXPERIENCED PAIN IN LOWER BACK

STRAINED RIGHT SIDE OF BACK WHILE INSTALLING A FILTER BASE ON A D9R DIFFERENTIAL.

EMPLOYEE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL IN BOTH WRISTS

Employee had finished hooking up cat and tossed cathead into tray of addcar. Cathead bounced and contacted his left index finger, partially tearing off fingernail.

Due to occupational illness, as substantiated by medical records, employee became unable to work because of the ailments described in the medical records. This was not due to a single event at the mine, but repeated trauma over an extended period of time due to normal job duties.

EE AND CO-WORKERS WERE INSTALLING A GATHERING HEAD IN THE HWM. AS THE POT DROPPED INTO POSITION, THE SLATE BAR USED TO GUIDE THE POT INTO PLACE BECAME CAUGHT. THE WEIGHT TRANSFER CAUSED THE BAR TO "KICK BACK" STRIKING EE ON THE RIGHT SIDE OF FACE

WELDING ON AXLE SEAT & WHILE CLEANING METAL PIECE OF RUST GOT INTO HIS EYE.

Injured was walking across pit and stepped in a snow-covered frozen rut, resulting in a sprained ankle.

Injured was traveling from the E2 to the E3 manbar, as he entered the E3 curve he took curve at too low of an angle, therefore trailer began to push the tractor across the curve. Thr driver tried to correct the situation, but failed. the truck overturned onto the driver's side.

Injured was in the back of mechanics truck referiving a cloth sling. as he exited the bed he was holding on to the hand rail with his right hand. his feet slipped out from under him due to spilled oil. this resulted in a sprain to this right wrist.

EE WAS MOVING CABLE OFF OF PONTOON END OF STACKER BELT, FELT A "POP" IN LEFT ELBOW. EE CONTINUED TO WORK SHIFT AND WENT TO REALISH BOONE MEDICAL CENTER WHERE EE WAS DIAGNOSED WITH TENDONITIS AND A MUSCLE SPASM. EE WAS NOT GIVEN ANY RESTRICTIONS

Injured was removing axle from tandem truck using 16 lb hammer to loosen axle. While swinging hammer felt sharp pain in lower back.

WHILE DUMPING A LOADED TRACTOR AND TRAILER, THE BED JACK BURST DUE TO THE PACKING GLAND BLOWING OUT. INJURED WAS TRANSPORTED TO HOSPITAL WHERE HE WAS DIAGNOSED WITH A SPRAIN.

EMPLOYEE WAS CLEANING THE TAILGATE OF THE TRUCK HE WAS TRAINING IN WHEN HE SLIPPED CAUSING HIS FINGER TO BE CAUGHT BETWEEN TAILGATE AND FRAME OF BED.

Injured was connecting heads when he felt pull in left arm. Continued to work and reported accident on 9-28-04. He was seen at hospital and was released to come back to work for follow up on 10-6. He was placed on physical therapy. He did not miss any work.

HOLDING ENGINE SIDE GUARD TO START BOLT, DROPPED PANEL AND CAUGHT IT PULLING MUSCLES IN LOWER BACK. *EMPLOYEE WAS OFF WORK 7/7 & 7/8 BUT DID NOT SEEK TREATMENT UNTIL 7/9 RETURNED ON 7/12.

EMPLOYEE WAS REMOVING BALL STUD 8RCM #7 785 WHEN THE RACE BROKE AND A SLIVER OF METAL STUCK IN HIS LEFT ARM.

INJURED WAS MOVING WATER LINE & FELT PAIN IN SHOULDER. INJURED WENT TO DR ON JUNE 9TH & CONTACTED BLACK CASTLE SAFETY DEPT. INJURED'S FIRST DAY AWAY FROM WORK WILL BE JUNE 18TH. DR DID NOT TAKE INJURED AWAY FROM WORK UNTIL 6-17-04.

Came off hill to leave property. Stopped at 39 marker on laxaire hill to let loaded truck pass. Proceeded down hill releasing clutch and turning jake brake on. Applied light brake pressure and didn't have any. Tried to gear down and couldn't. Put truck in sump ditch between 38-39 to stop truck.

EE STATES THAT HE STEPPED ON A ROCK THAT GAVE WAY, CAUSING HIM TO FALL RESULTING IN BACK SPRAIN. EE CONTINUED TO WORK & DID NOT REPORT ACCIDENT UNTIL NEXT MORNING.

THIS IS A NON-INJURY WALL FAILURE. SECTION OF HIGHWALL MEASURING 120' LONG, 0' TO 12 FT THICK & APPROX 80' HIGH FELL AND INTERRUPTED THE MINING CYCLE.

TIGHTENING TANDEM ON PASSENGER SIDE. USING LUG WRENCH TO TIGHTEN LUGS, EMPLOYEE WAS PRESSING DOWN WITH FOOT. SAID THAT HIS FOOT WAS SLIPPING OFF SO HE SWITCHED TO THE OTHER SIDE AND BEGAN TO PULL ON LUG WRENCH STRAINING HIS BACK.

EE WAS SCALING A LOAD OF COAL AT BRANCH SCALES. HE WAS STANDING ON DRIVERS SIDE FUEL TANK STEP LOST HIS FOOTING AND FELL FROM STEP INTO MUD & WATER BESIDE THE SCALE.

Equipment fire of D11R dozer. Operator parked equipment at 2:50 am. Mechanic and fueler then proceeded to fix handrail and fuel equipment. No welding or torch was used to fix handrail. Greaser then noticed dozer on fire. all fire extinguishers were spent along with the fire cart.

EMPLOYEE WAS REMOVING WATER TUBE FROM TOP OF RADIATOR TO THE THERMOSTAT HOUSING. HE HAD LOOSENED THE CLAMPS AROUND THE RUBBER GASKETS. EMPLOYEE BEGAN TO PULL THE TUBE FROM THE TOP OF THE RADIA TOR. THE TUBE BROKE FREE AND STRUCK EMPLOYEE IN THE NOSE. THIS RESULTED IN A FRACTURE.

FINES BELT WAS FROZEN, WHILE TRYING TO FREE BELT EE GLOVE BECAME ENTANGLED IN BELT DRIVE PULLEY'S WHILE TRYINT TO JAGG THE BELT. THE RESULTED IN A LACERATION TO LEFT INDEX FINGER WITH 10 SUTUR ES.

WHILE TRYING TO LOAD OXYGEN BOTTLE, HIS ELBOW SPUN AROUNG HITTING THE DOOR.

EE WAS HELPING LOAD A TIRE INTO THE SERVICE TRUCK. LATER IN THE SHIFT EE STATED HIS BACK WAS HURTING HIM. EE COMPLETED HIS SHIFT AND TRANSPORTEDHIMSELF TO THE HOSPITAL. HE WAS DIAGNOSED WITH PULLED MUSCLES IN HIS BACK.

EE WAS CHECKING THE OIL ON THE 992C LOADER. HE OPENED THE ENGINE COMPARTMENT AND LOST HIS BALANCE & SLIPPED. AS HE TRIED TO CATCH HIMSELF HE FELL TO THE GROUND. RESULTED IN STRAIN TO LEFT SHOU LDER.

EMPLOYEE STATED HE STOPPED AT TOP OF HILL, PUT TRUCK IN 2ND GEAR PRIOR TO STARTING DOWN. GOING DOWN THE HILL THE BRAKES STOPPED WORKING. AT THE #5 MARKER HE DROVE INTO THE STRADDLE BERM AND IN TO A SUMP. HE RECEIVED ON BROKEN RIB. WAS TOLD BY DOCTOR TO RETURN TO WORK IN ONE WEEK.

EE STATES THAT AS HE EXITED THE TRUCK SCOLAR HE HIT A HOLE BLA THE SCALES AND THE ROAD. THIS CAUSED HIS ARM TO BE CAUGHT IN THE STEERING WHEEL INJURY HIS LEFT ELBOW.

EE WAS CLEANING MIRROR ON OFF SIDE OF RICK TRUCK. HE STATES THAT HE REMEMBERS CLEANING THE MIRROR AND THEN LYING ON THE GROUND. IT APPEARS THAT HE BECAME DIZZY AFTER RAISING UP FROM CLEANING M IRRORS.

EE WAS A PASSENGER IN THE EXPLOSIVES TRUCK. THE TRUCK WENT INTO THE DITCH AND TURNED ONTO ITS SIDE. EE RECEIVED A BRUISED SHOULDER (RT). HE WAS NOT WEARING HIS SEAT BELT.

EE WAS DRIVING EXPLOSIVES TRUCK ON MAIN HAUL ROAD. HE PULLED TO RIGHT SHOULDER TO ALLOW EMPTY COAL TRUCK TO PASS. SOFT SHOULDER GAVE WAY CAUSING TRUCK TO ROLL ONTO SIDE. EE WAS TREATED AT E.R. FOR CUTS TO HEAD. GIVEN PRESCRIPTION MEDICATION & TIME OFF WORK. RELEASED FROM E.R. SAME DAY.

FOOT CAUGHT IN BETWEEN THE CONVEYOR CAR & GLIDES. *I RECEIVED THIS ACCIDENT REPORT FROM IND. 5/7/03. I DID INVESTIGATE & THEN REPORTED TO MSHA.

SLIPPED ON D10N AND HIT STEP ON PUSH ARM; CONTUSION AND KNOT TO RIGHT LOWER LEG.

SLIPPED ON STEPS GOING UP LOADER, FELL OFF. DIESEL FUEL SPILLED ON STEPS, SLIPPING.
