On Nov 19, 2008, employee was in the process of changing a broken miner bit. While he was striking the insert with a sledge hammer, his left hand slipped down the bit lug resulting in him striking his left hand against the insert with the sledge hammer.

EE was tramming the bolter up to install another row of bolts. A rock fell from between the last two bolts that were installed and hit his left hand that was on the inch control levers. He received 5 stitches to his left ring finger.

Unintentional roof fall occured at White Buck No. 2 Mine. The fall occured on 1st Right off of the Southwest Mains at Spad No 10141. The fall measured 10' length, 20' Wide, 6' in Height. No injuries occured, no passage of men affected.

On 10-26-07, an unplanned roof fall occurred on one belt, break 32 between track entry & return entry, spad no. 68314. The return stopping was crushed out. All miners were escorted outby the fall, until stopping is replaced, and the area is deemed safe by the operator.

EE was unloading a bonder off of the mantrip and smashed his finger between the boncer and horn on the mantrip. He went to the hospital when he received 4 stitches in his right ring finger.

Employee was hanging roof bolter cable and walking when his head contacted a belt "L" bracket that was installed in the mine roof. The bracket had been installed for a belt move and the roof was approximately 5 feet high. *Note: Referred to specialist for diagnosis on Sept. 14, 2006.*

On 4/17/2006 at approx 9 am an unplanned roof fall was found at the old 1st N-W mains Spad No. 9162 in #9 entry. The fall does not impede travel or ventilation and has been hazard danger off.

On 1-10-06 at 9:05 am in the old section, spad #10159 off southwest mains a roof fall was discovered. This fall is in the old pannel and did not impead travel or ventilation. The fall measured 40' in length. 20' in width, and was ranging from 6" to 6' in height.

He was installing tram motor on shuttle car. He had his hand on top of tram motor while trying to line up pin holes. Forklift was shut off when it drifted allowing motor to spring up against bottom of buggy smashing hand.

EMPLOYEE WAS PUTTING 4" WATERLINE TOGETHER USING A CRIB BLOCK. WHEN HE PULLED BACK A CRIB BLOCK TO BUMP THE WATERLINE TOGETHER IT SLIPPED OUT OF HIS HAND & THE CORNER OF THE CRIB LANDED ON TOP OF HIS LEFT FOOT AT THE EDGE OF HIS METATARSAL BOOT. HE FINISHED HIS SHIFT THEN WENT TO THE DOCTOR.

ON MAY 2, 2003, EE WAS UNLOADING A TIMBER MEASURING 82" LONG, 8 1/2" ACROSS WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. INJURED CONTINUED TO WORK AND WENT OVER TO THE NEXT BLOCK AND CUT 8 MOR E BREAKER TIMBERS. HE WAS PICKING UP THE BUTS THAT HAD BEEN SAWED OFF WHEN THE PAIN GOT WORSE.

WHEN A ROCK 3" THICK X 3 1/2'X3 1/2' AROUND FELL FR OM THE TOP BETWEEN THE RIB AND THE BOLTS ON THE RIGHT SIDE OF HT ENETRY. STRIKING HIM ON THE HEAD, NECK AND SHOULDERS. HE WAS PLACED ON A BACKBOARD AND TRANSPORTED OUTSIDE.
