Driver was at parked truck (#5 lot) to start pre-shift inspection. Truck started to move. Driver fell under the truck and truck ran over driver's legs from about the knee area down.

Employee deliberately disobeyed warnings to cease welding on inflated floatation tire rim ten minutes before tire blew up. Heat from welding plus compression of air in tire equals massive explosion.

Employee was pulling on the bottom of tailgate. Trying to install a crib block between the gate and bed because the gate was sprung and would not latch. Employee pulled back on gate dislodging crib block, causing it to fall on top of EE's right foot. Causing fracture to top of right foot.

Truck driver climbed up on the right front fender of EE's coal truck to add motor oil to the motor, slipped off of fender landing on EE's right side of EE's head and right shoulder. Had pain in EE's shoulder and back.

Employee had a slate bar in dilute impeller pulling down when impeller broke loose and employee fell to the left with both feet on the floor. Two other employees were on the other side with slate bar pushing up.

Employee was sitting down with sledge hammer busting rocks out of the scalping screen, as employee went to stand up employee slipped and twisted left ankle. Employee was taken to Elkins hospital where they conducted an X-ray which showed that employee had a fractured bone in left ankle.

EMPLOYEE WAS OPENING A DOOR TO GAIN ACCESS TO THE BOTTOM DECK OF THE RAW COAL SCREEN. THE HINGES ON THE DOOR WERE RUSTED AND BROKE OFF, CAUSING THE DOOR TO DROP DOWN AND SMASH THE EMPLOYEES FINGERS.

Employee was putting a bottom deck screen in a scalping screen. Once the screen started to drop to the bottom deck, it flipped up on one corner and hit the employee across the bridge of the nose causing a laceration and a broken nose.

While removing section of heavy media pipe (approx. 10" dia. X 24" length) EE went to push on side pipe as it was being removed it came out swung around striking him on the right wrist/forearm, breaking wrist. (closed fracture)

While moving pipe from old scrappa at top floor loadout we had cut off. He was turning pipe and it had a sharp edge on pipe that cut through his glove and into his left index finger at app. the first bend of finger. Cut appeared to be about 1" across finger and could not tell how deep cut was.

The injured was walking around the bucket of the parked endloader in order to get in the machine when his right foot slipped on a patch of ice in the roadway and his left foot remained stationary. This caused his left foot/leg to twist and resulted in his full body weight to shift to his left foot/leg causing a fracture just above the ankle.

The employee was walking across a floor that was being prepared for concrete, he tripped on the reinforcing wire mesh. The trip caused him to fall striking his knee on a metal curb - requiring 6 sutures.
