Employee slipped on miner cable and sustained a strain to the right knee.
An employee received a fracture to the right wrist while driving a 2 man personnel carrier over the mine track system.
TRUCK RAN INTO DITCH AND FLIPPED OVER.
Injured was backing scoop around the corner to reposition scoop, when the corner rolled & a piece of rock fell in the deck of the scoop striking his right arm at the elbow resulting in a bruise.
A roof fall was discovered at approximately 8:40 AM between spad 75637 and 75663 in the number 1 entry (left return). Dimensions of the fall were 120' long x 18' wide x 6' high. The area was bolted with 6' fully grouted bolts. Location of the fall was 1,600' outby the section dumping point.
