The employee was tramming company ID 537 CIA scoop when EE trammed over a piece of four-inch driscoplex 6300 water pipe approximately 17ft long causing the loose end to enter the scoop operator compartment striking EE. The employee received first aid and was transported to the surface before being transported via ambulance to CAMC General where EE was pronounced deceased.

EE was attempting to identify the problems with the scrubber motor on the JM 6911 continuous miner on the number one section in the number one entry when EE came in contact with electricity. The employee was brought to the surface and transported by ambulance to a Hospital where EE later passed away from the injury sustained from the incident.

Employee was injured when EE's left foot was struck by a rock that dislodged from the mine rib. EE was diagnosed with a fractured metatarsal and EE's foot was temporarily immobilized.

An unplanned roof fall occurred on 10/18/2025 at approximately 1:16 AM on the 4 right panel of Unit 1. The fall occurred in the intersection of No. 8 entry, crosscut No. 17 and was approximately 20ft X 20ft X 10ft thick.

Injured employee was in the process of closing air lock doors at the mouth of the section when left hand was caught between the doors. This caused a laceration on the left index finger, resulting in 8 sutures.

Employee was operating continuous miner when a rock measuring 23" long X 18" wide X 3.5" thick dislodged from the top striking EE in the left shoulder. Employee initially sought medical treatment and was released to return to work. Employee went to a follow-up appointment on 10/31/2024 where EE was placed off work for physical therapy treatment.

Employee states was in the process of bending a roof bolt when it became lodged under the drill pot. While attempting to free the bolt it suddenly sprang upward striking EE in the face causing a laceration to EE's upper lip that resulted in 3 sutures.

Employee was preparing to make a belt splice on the slope belt. In the process of moving the belt cutter to cut the belt, a finger encountered the knife blade of the cutter, and cut their left middle finger. The employee finished the shift then went to the CAMC General to be examined. Employee was given 10 stitches in left middle finger by a physician.

Continuous miner operator was taking the last lift out of the block when the corner of the rib fell out striking EE resulting in a dislocated right knee and fractured left ankle. Lost workdays were a result of this injury.

At approx. 9:20pm it was found that the Coal Fork elevator was not operating. Once electricians arrived they determined there was a ground that caused the contactor to go bad. At approx. 1:15am the elevator was back in full operation.

On 8/28/2023 9" of rain fell in about 3 hours. This allowed water to run down the box cut into the slope area of the mine and pool up. This area of the mine is the old mine in the seam above where we currently mine at the top of the internal slope located at Dawes.

Employee was moving a rock to set a timber when EE felt a pain in EE's right shoulder. Employee initially sought medical treatment and was released to return to work. Employee went to a follow up appointment on 09/26/23 where EE was advised that the MRI that was given on 08/16/23 showed a labral tear of the right shoulder that will require surgical repair in the future.

EE stated they were helping remove a rock from on top of the miner. They grasped the rock placing one hand on each side and pulled the rock to slide it off to the ground. In the process the rock slid down and smashed their fingers between the rock and the metal shield of the miner. EE received 3 stitches in the left index finger.

Teammate stated EE was in the process of replacing bits on the 088 miner. EE had removed the old bit and was pushing the new bit into place when it became stuck so EE took EE's pick hammer and struck the bit to force it in place. After several hits a piece of metal came back and struck EE. On 3/30/2023 EE had the metal removed and received 3 stitches.

Employee stated had just finished using the mine phone at the section tail and started to walk to the face. As employee stepped toward the feeder employee tripped over employee's feet and fell allowing left forearm to come in contact with the flag pin that holds the section tail in place. Employee received 10 stitches and was given a full release back to work.

EE was in the process of changing a boom swing pin on the 401 roof bolter. EE had removed the old pin and set it in the tray of the bolter. As EE was driving the new pin in place the old pin which weighs approx. 33 pounds fell out of the tray smashing EE's left pinky finger. EE received 3 stitches and was diagnosed with a fracture of the pinky finger.

Employee stated employee was helping another employee unload oxygen bottes on Unit 4. After they had finished unloading the injured employee stated shoulder started to hurt. Employee stated that EE was in the process of removing rock off of the left C/M Machine on the Unit 2 Section. EE stated that EE was going to attempt to push a rock off the machine so they could move it. In the process of EE pushing the rock off the C/M machine it broke. This caused EE's hand to slip allowing EE's arm to strike the sharp edge of the rock. EE received 6 stitches.

The employee received 5 stitches

Employee stated that EE was traveling with an inspector and they were going through a mandoor. EE was holding the door for them to go through and after the last went through EE turned to go through. EE's hand slipped off of the handle and the door slipped out of EE's hand causing the door to smash EE's right thumb. Employee received 4 stitches.

(On 8/25/2022 employee was diagnosed with a broken 5th digit "pinky" finger.)

The Coal Fork Main mine hoist was down from 9:30am until 11am on August 2nd due to a contactor in the hoist house going bad. The contactor was replaced and the hoist is working as of 11am on August 2nd.

Employee stated employee was not paying attention and drove into the cross under guard at the #1A belt line. This is where the track and belt cross. Employee received 6 stitches in face and returned to work.

Employee stated was in the process of adding 4" aqua waterline in prep for a belt move. Employee picked up a crib block and used it to strike the end of the waterline and after several hits employee caught a finger between the crib and the waterline. On December 7th employee had the end of the finger removed due to nothing left to protect the bone.

Employee was changing a rotation hose on the 419 roof bolter and the channel locks employee was using slipped off the fitting causing employee's right palm to come in contact with a metal tag on the bolt tray. This caused a 5 inch cut that required 5 stitches.

Employee stated IE and another worker were in the process of replacing the rub rail on the 6927 cm. They set the rub rail on the bracket and was in the process of pushing it on in. IE had finger caught between the rub rail and the frame on the miner. IE received stitches on 8/3/2021 and a skin graft.

The American Eagle Mine experienced an unplanned roof fall above the anchorage zone in between 76 and 77 breaks on the old 2-South mains in the #7 entry. This is not an escape way and does not alter ventilation. The dimensions are approximately 50' long 18' wide and approx. 8' thick. No injuries occurred during fall.

While doing so EE felt a pop and instant pain in right shoulder. This employee returned to work the next day and recently went back to doctor and was placed off work for surgery as of 5/18/2018.

Employee was watching the C/M machine cable as the C/M machine was advancing to another entry on the 17 HG section. EE had EE's back turned away from the roof bolter cable which was laying on the ground. When the roof bolter operator engaged the cable reel on the roof bolting machine the cable struck the employee on the back of EE's feet and caused EE to fall on EE's back.

The Dawes elevator failed to operate at 2:45 PM on 09/09/2016 due to a buffer switch being grounded. They got the issue fixed at 4:55 PM and it started working again. There were no employees on the elevator when this incident occurred.

An unplanned roof fall that impedes travel of the track / secondary escape way occurred at 25 crosscut on the 4 South track. The width of the fall was 45 feet wide. The length of the fall was 20 feet long. The thickest point of the fall was 2 1/2 feet thick.

Employee was assisting in putting a metal guard over a hydraulic jack on the take up on the 1 North B Belt Drive. While sliding the guard into place it moved unexpectedly and slid off the frame and caught the injured employees hand between the guard and the frame. This caused lacerations to both middle and ring fingers on EE's left hand.

Employee was operating the #419 roof bolting machine on the 1 North Section. EE was bolting in the #5 left entry. In the process of EE starting to drill a hole in the top to install a roof bolt, a piece of rock that measured 26"X10"X8" fell from the mine roof and struck the injured employee in the right hand.
