Employee states he was adding hydraulic fluid to #11301 D8 R dozer at the Bragg Fork Impoundment. The employee slipped and fell 6 feet 3 inches to the ground. Employee was transported to CAMC General by Air Evac. Employee was admitted and diagnosed with back and neck pain and released on 7/31/2015 and referred to a Specialist.

Employee states he was turning the ammonia off on the B-side thickener and the valve leaked and sprayed ammonia on him. Employee went to Boone Memorial Hospital and referred to a Specialist. Employee was released to return to work. On 7/30/2015 the employee missed work due to an adverse reaction with the antibiotic that was prescribed.

Employee was dismounting #11301 D8 dozer on the refuse area and his foot slipped and he fell on his back and right hip. Conditions at the time of the incident were wet and muddy. Employee was diagnosed with Lumbago and taken off work on 06/17/2015.

Employee was moving side plate off the crusher on the welding table. The flanges dropped off the table mashing his right hand middle finger causing a laceration. Employee went to Raleigh General Hospital and received seven sutures.

Employee stepped in a hole and twisted his left knee and hip. Employee did not miss any work until 9/25/2013 when surgery was performed to repair a torn meniscus of his left knee.

Employee was lifting a five gallon bucket of hydraulic oil up onto the tracks of #18605 a D10N dozer. While in the motion of lifting employee felt pain in his lower right back. Employee went to CAMC General on 3/19/2013 and referred to a specialist. Employee was treated by Dr on 3/26/2013 and diagnosed with an acute lower lumbar strain and taken off work thru 4/11/2013.

Employee was sitting down under a belt and grabbed hold of the structure to stand up and his left knee popped and he fell to the ground. An MRI revealed on 4/15/2013 that the employee had a tear of the medial cartilage or meniscus in his left knee and is off work and is now a lost time injury.

Employee stepped in a hole and twisted his left knee. Employee was diagnosed with a torn meniscus and continued to work until 5/10/2013 at which time surgery was performed to repair the tear. This is now a lost time injury.

According to the employee an incident occurred that allegedly injured his shoulder. This form is submitted to abate citation #8113668 and in no way substantiates the employee's claim. The employee requested exploratory surgery that revealed no damage. Management reserves the right to further investigate this alleged occurrence and appeal the issuance written by the inspector.

A chain hoist was attached to a "lifting lug" to drag in chutework/placework. The lifting lug broke, allowing the chain hoist hook to fly free, striking the employee in the left cheek.

He was inspecting belt cleaners on the belt head. He couldn't get a good look, so he removed the safety quard to get a better look at the cleaners, while the belt was running. While trying to inspect the cleaner, he slipped and somehow caught his wrist between the roller and a I beam. He jerked away, finding his wrist badly cut.

Employee was attempting to install an idler on the #23 belt. Employee got his fourth finger caught between the idler and toe board pinching his finger causing a laceration. Employee was transported to Boone Memorial Hospital were he received five stitches and was released to return to work.

Employee stepped into a 14" wide by 3' long hole with a 6" drop to the new beam which had to be open due to construction. Employee caught himself on a 3' column that had a sharp corner causing a puncture wound to his left forearm. The wound required 3 stitches inside and 9 stitches outside.

While lifting a bucket of oil into the back of a pick-up truck, employee suffered a herina to the umbilical area. * NOTE: Employee's condition was not diagnosed until 5/12/08. Employee has not missed any work days as of 5/14/08 but surgery to repair the injury site has been scheduled.

THE EMPLOYEE WALKED THROUGH A DOORWAY AND THE PROTRUDING PIPE STRUCK THE DOOR FRAME CAUSING THE EMP TO TWIST SUDDENLY. EMPLOYEE REPORTED A DULL PAIN IN LOWER BACK.

EMPLOYEE WAS CUTTING A SECTION OF BEAM OUT OF THE FLOOR STEEL FOR REPLACEMENT. WHEN THE CUT WAS COMPLETED, THE BEAM SHIFTED. HE JUMPED BACK, LOSING HIS BALANCE, FALLING APPROXIMATELY 4' TO THE FLOOR BELOW. HE WAS WEARING A HARNESS THAT WAS CONNECTED TO THE STEEL BUT AT FLOOR LEVEL. THE LANYARD WAS NOT CONNECTED HIGH ENOUGH TO PREVENT THE FALL.
