Security guard fell or slipped from the cab of his personal vehicle into a shallow diversion ditch at the base of a berm along a roadway. He was found with his face submerged in approx 12" of water. The death was drowning & that a contributory condition was marked alcohol intoxication. The case was not determined to be chargeable until December 22, 2014.

Employee accesing a loaded barge, he stepped on a piece of angle iron welded on the cell with his right foot and held onto a D shaped ring that was also welded onto to the side cell, with his right hand. Foot slipped off of the piece of angle and he fell backwards, trying to catch himself with his right hand, chipped a bone his right wrist.

EMPLOYEE STATES SHE WAS LOCKING THE DOOR TO SCALEHOUSE AND WHEN SHE TURNED AROUND HER RIGHT FOOT SLIPPED AND GOT HUNG UNDER THE STEP. SHE FELL AND LANDED ON HER RIGHT ANKLE. SCALEHOUSE HAS TWO STEPS.

EE WAS IN THE SHOP ASSISTING TWO OTHER EE'S WITH END LOADER MAINTENANCE. THEY STARTED TO HOIST A 55 GALLON DRUM OF ANTIFREEZE UP ONTO THE END LOADER. AS THE WEIGHT WAS PICKED UP ON THE DRUM, O NE OF THE DRUM ANCHOR HOOKS DISLODGED ALLOWING THE HOIST BLOCK TO SWING STRIKING HIM ABOVE THE LEFT EYE. THE INCIDENT CAUSED A LACERATION REQUIRING A COUPLE OF SUTURES TO CLOSE.

EE WAS ASSISTING INSALLING ADDITIONAL SUPPORT STRUCTURES FOR THE JAWS BIN BELT CONVEYOR WALKWAY. THE SUPPORTS WERE FABRICATED BEFORE INSTALLATION OUR OF 4" ANGLE STEEL & 4" FLAT STEEL. FOUR SUPPORTS HAD BEEN INSTALLED & THEY WERE IN THE PROCESS OF INSTALLING THE 5TH SUPPORT AN INTERNATIONAL 510 LOADER WAS BEING USED TO POSITION & HOLD THE PROCESS IN PLACE BY WEDGING THE SUPPORT

THE SCREEN WAS BEING CARRIED UP THE STEPS TO THE 1ST FLO OR TO ACCESS THE CHAIN HOIST. AS THEY NEARED THE TOP OF THE STEPS & WERE NEGOTITING THE SCRE3N OVER THE HANDRAIL EE FELT A SHARP PAIN IN THE ABODMEN AREA. HE HAD A PREVIOUS NON WORK RELATED

EE WAS USING A SLATE BAR TO BREAK UP A LARGE LUMP OF COAL AT THE HEAD ROLLER AREA OF THE CRUSHER BELT. THE ELECTROMAGNET FOR PICKING UP METAL TRASH WAS ENERGIZED. EE GOT THE SLATE BAR TOO CLOS E TO THE MAGNET AND THE MAGNET PULLED THE BAR SMASHING THE LEFT THUMB OF EE BETWEEN THE BAR AND THE MAGNET. EE SUFFERED A LACERATION OF THE LEFT THUMB RECEIVING 7 SUTURES TO CLOSE.
