Employee was walking beside a scoop, he tripped and fell into the blade of a bow saw lying on the scoop. The fall resulted in a laceration of the top part of his left hand requiring stitches.

A roof fall occurred in the last open x-cut in #5 entry, measuring 120'x18'x10' thick. Area was bolted with 3/4"x72" tension rebar with T3 roof channels on 42" ctr's. 2-10' long cable bolts are between each row of roof bolts. X-cut had been centered jacked with 100 ton jacks. Poor immediate roof with over mining at 60' and 1000' of overburden were contributing factors.

Employee was changing crawler take-up on Joy 14CM15 miner. The gathering head pan was chained up. He placed himself under the gathering head pan to push take-up in place with his feet. The chain broke causing him to be caught between the gathering pan and mine floor. He had fracture of nasal bone & 3 ribs, sprain of sacroiliac & pelvis, pneumothorax trauma & liver laceration.

A roof fall occurred in a 4-way intersection at spad no. 11278 measuring 60' x 20' x 12' in thickness. The area was bolted with 3/4" x 6' tension roof bolts. At least 8 - 10' long cable bolts were installed in the area also. No injuries occurred.

A roof fall occurred 120' inby spad #11035 in crosscut between #5 & #6 intake entries. The fall measured 50' x 20' x 6' thick and was bolted with 6' long by 3/4" dia. tension rebar roof bolts. The area had previously been breakered off with 100 ton Heitzman jacks due to deteriorating roof conditions.

The fall measured 30'x 20'x 10' thick and was roof bolted with 3/4" dia., 6' long tension rebar roof bolts with 8"x8" plates with rock shields and supplemented with 10' long cable bolts. The fall may have occurred due to over mining approximately 70' above.

A roof fall occurred in an intake aircourse near spad No. 10819. The fall measured 120 ft. in length, 20 ft. in width and 9 ft. in thickness. The area was roof bolted with 3/4" diameter 6 ft. and 8 ft. tension rebar roof bolts. The fall was probably caused by overmining abutment stress. No injuries were involved.

A rock fell from the mine roof, striking the ATRS of the Fletcher roof bolter. When the rock hit the ATRS, employee was positioned near the rear of the bolter. The rock falling onto the machine, bounced him off the machine onto the mine floor with possible rib fractures.

EE picked up a bag of rock dust, twisted to put on Boss Hoss. Alleged pain in his lower back. E-Doc #14116925 that was submitted 5/10/11 for alleged back injury on 4/28/11 was determined by Workers Compensation (BrickStreet) to have sufficient inconsistencies and contradictions to concluded that he was not injured in the course of and result of employment.

The area was bolted with 6' tension rebar bolts and supplemented with 12' cable bolts on 16' centers. This area is under stress from over mining. Some water was present in the roof.
