An unplanned roof fall occurred outby the section loading point above anchorage. The fall is located at #16 crosscut from the #7-#8 entries measuring approximately 90 feet long by 10 foot thick. Supplemental roof support was added in the adjacent entries, and the fall has been breakered off.

Employee was carrying a 40lb. bag of B-Bond to place on a scoop, their feet got tangled in a banding strap and they fell twisting a knee. Employee worked next scheduled shift then returned to a specialist on 6/2/2025 where they were diagnosed with a torn meniscus.

Employee reported that their eye was irritated after making a cable splice. Employee was unable to identify the mechanism of injury, but was diagnosed with having a foreign object in their eye that was removed in an outpatient surgery.

EE was shoveling at a v-plow on the #10 Belt when EE slipped resulting in straining EE's hamstring. EE was placed off work on 5/28/2025 after seeing a specialist that diagnosed EE with two torn tendons in the hamstring.

EE was struck by a rib roll in the right foot while setting over in cut with EE's continuous miner. EE did not initially seek medical treatment for EE's injury. EE reported a sharp pain on 5/17/2025 and sought medical treatment at that time. EE was diagnosed with an Achilles Tendon rupture on 5/28/2025 and placed off work at that time.

Panther Creek Mining LLC · Caught in, under or between a moving and a stationary object

A motorman at Maple Eagle #1 injured right pinky finger while attempting to put the 15-ton Brookville locomotive back on the rail. EE grabbed the handle of the lifting jack to adjust its position on the mine floor and had not removed their hand. When the weight of the locomotive was applied, this pinched the victim's pinky finger between the jack handle and lifting mechanism.

Employee was in the process of knocking the miner boom pin back in on unit #2 on #112 C/M, when a piece of metal came off the pin and struck the employee in the left pinky finger. On 04/25/2024 the employee had the metal taken out of EE's finger at CAMC and was placed off work.

A roof fall occurred at 9 belt intake #7 entry between 9 and 10 break. Ventilation was not affected; the area was timbered off and the lifeline was rerouted. The roof fall measured 20 feet in length and approximately 6 - 7 feet high.

Employee was in the process of loading the roof bolter in #5 entry break 17 on Unit 2. Employee was sliding a bundle of roof bolt plates over to make room for roof bolts. As employee pushed on a bundle of plates their left forearm was cut from another bundle of plates. Employee was seen at hospital and received 5 stitches and was given a full work release.

Employee had got off the mantrip to open a set of airlock doors on 12 track. As EE started to walk towards the doors EE felt a pop in the right knee. EE was seen at a medical clinic and was given a full work release. On 05/17/2024 EE was placed off work to have the graph repaired on the meniscus and ACL.

On 01/10/2023 at 3:00pm we had a roof fall at 13brk on 6 belt. The mine was being fire bossed at this time and had been idle due to power loss at the mine. Once the roof fall was discovered the area was dangered off and a plan was submitted to clean up the area and re-bolt the area. No injuries occurred from this incident.

An unintentional roof fall above the anchor point occurred on 10/30/2023 on Unit 2 inby the feeder line at 12 break between 5 right extending to 7 right. The fall was approximately 240 feet in length and above the anchorage point. The operator has decided to pull off the section and move to a new location.

An unintentional roof fall above the anchor point occurred on 10/25/2023 on 10 belt break 41 in the #2 entry. The fall measured 20' long by 20' wide, and approximately 10 foot in height. No injuries occurred. The Travel way for the air way was blocked, ventilation was not affected.

Employee was mining in 2-left at 19 brk on #1 section. EE heard the rib start popping so EE moved to the face of #2 that was drove up 4'. After loading the shuttle car, a rock rolled out of the face striking EE on the lower left leg / foot. The rock measured 18' L x 2' w x 1' thick.

Employee was bolting in #5 entry on #1 section. After drilling with the 3' starter, EE inserted a 6' stack steel and used the fast feed to run the steel up the hole. EE let off the fast feed about 1 foot from the termination point but the fast feed stuck causing the steel to break striking EE on the right index finger resulting in 7 stitches.

EE was spot bolting in #6 entry on 2 section. EE was operating the 406 R/B while swinging the drill station out the drill steel caught the top and broke near the chuck. The steel sprung toward EE and struck EE on the left ring finger. EE transported self to plateau medical and was given 4 stitches in the finger, EE reported back to work on EE's next shift.

Employee was helping get a mantrip through a track switch on the surface when the mantrip struck employee's right foot causing employee to fall and hurt left knee. On 6/28/2023 the operator received the report of an MRI showing a non-displaced fracture of the patella. This is now being reported as an NFDL incident.

Employee stated was setting the roof bolter up in #9 entry on #1 section when EE noticed a rock falling. Employee stated tried to move out of the way, but the rock struck EE's left forearm resulting in a laceration. The rock measured 6"Lx2"Wx1/4"thick. The employee received 7 stitches and returned to work on EE's next scheduled shift.

A contract motorman working through a contractor company was in the process of jacking a flatcar back on the track, when the motorman slipped and fell on right knee resulting in pain in lower back. The motorman sought treatment at a clinic where the motorman was placed on restricted duty.

Employee was dragging an airlock door away from the track when employee felt pain in left arm. Employee sought medical treatment and returned to work. On 12/5/22 employee was placed off work and surgery was scheduled for 12/8/22 to repair bicep tear.

Injured employee was in the process of cutting expanded metal with a grinder to fabricate a guard for a belt head, when the grinder caught the expanded metal causing the disc to climb up the metal and contact employee's left index finger. Employee finished shift and sought medical treatment where employee received 7 sutures. Medical Only.

Employee was riding a 2-passenger mantrip. They were traveling up a hill when the yoke on the rear drive shaft broke. When the yoke broke from the rear drive shaft, the mantrip lost braking capabilities, causing the mantrip to coast backwards down the hill. The front passenger tire struck the employee's left lower leg.

The employee was helping spot a rib bolt when a rock rolled out of the rib and struck EE behind the metatarsal guard of EE's boot. The rock punched a hole in the top of EE's foot that was too deep the clean, so the medical facility did not clean it right away. This prevented stitching of the wound.

EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.

Employee was in the process of cutting belt rubber with a knife and while cutting the belt the knife slipped, causing them to cut their left thumb. This resulted in a laceration that required 5 stitches to close the wound.

When the inby motorman applied the brakes, the trip started sliding downgrade toward the mainline switch. The inby motorman stated EE was ejected when the motor derailed at the mainline switch.

EE did not receive any stitches, but was referred to a had specialist for further examination/ treatment. EE was allowed to return to work on restricted duty.

Employee was loading roof bolts onto the Roof Bolter. While doing so, a piece of mine roof approximately 8" x 8" x 2" fell and struck EE on the right middle finger causing a laceration that required part of the fingernail to have to be removed and 2 stitches to close the wound. The mining height is 8 1/2 feet in this area.

The employee was constructing a stopping at 59cc #1 Section between #7 and #8 entries. The area was 9' 3" high and 18' 8" wide. EE was standing on a ladder to place another block on the stopping. As EE placed the block, a section of rock measuring 24" x 13" x 1" fell out and struck EE on top of the right hand. The injury took 7 stitches to close.

This incident became reportable on 3/27/2018.

Injured employee and two other employees were changing the cable reel on a shuttle car. They were using a 3/4 ton chain ratchet to pull the cable reel out. Injured employee was using left hand to operate the chain ratchet and had right hand resting on the chain the chain ratchet was hanging on. The cable reel shifted causing the chain ratchet to slide down the chain.

Employee was stepping off the back end of the U307 roof bolter on the 002 section to go assist other workers with building a stopping, when he slipped and fell off the roof bolter and his SCSR located on his belt struck the step on the back of the machine causing pain in his lower back. X-rays were negative at Montgomery General Hospital and his is on modified duty until 11-11-11.

He had locked the main breaker, but did not lock out at the power center. While connecting leads, the EE removed trouble shooting gloves, his hand came in contact with a lead on the main breaker.
