Employee was using an air hammer to remove a bushing. The bushing splintered striking the employee in the face. A small sliver of metal was embedded into the employees forehead.

While lining up the bolts in the bottom piece to of the crusher the holes in the top side one of the bolts would not line up correctly. Employee used a pry bar to pull on the bolt to. When the bolt lined up to the hole the top side of the cone set into place which pulled the pry bar towards the steel lining of the cone. Employees finger was pinched causing laceration

Employee was putting a tail gate on dump truck. One pin was out, one pin was still hanging on top of bed. Employee moved ladder striking old pin, knocking it out of bracket. Pin fell striking employee above the eye.
