EMPLOYEE WAS ASSISTING IN REMOVING A PIECE OF FOULED DRILL STEEL OUT OF TOP. WHEN EMPLOYEE WALKED AROUND FRONT OF BOLTER HE SLIPPED AND FELL, KNOCKING HIS HARD HAT OFF. THE STEEL FELL ND HIT HIM ON THE TOP OF THE HEAD.
