Employee was assisting in the removal of a steel beam. While preparing to cut a beam another employee in the man basket struck a piece of steel causing it to fall on the employees leg. EE has a small fracture on EE's patella as well as a laceration above EE's knee. EE is on modified duty.
Worker was walking on 2nd floor of transfer. EE stepped on unsecured decking. The decking slipped and EE fell to the floor below. EE hurt EE's knee in the initial fall and then EE's chest when EE fell over after landing. EE was wearing fall protection but was not tied off.
While working near the 505 Truck Feeder, employee slipped and fell against handrail fracturing two ribs.
Employee has been diagnosed with pneumoconiosis.
While welding arms for a return roller training bracket, a piece of welding slag went into miner's ear. Employee has continued to work with no lost days. Employee had outpatient surgery on 01/08/2021. Accident is medical reportable.
Employee was assisting with the removal of a screen side board from the basement of the plant when it slid down a post it was leaning against and caught the employee's right foot. The employee was wearing metatarsal boots at the time.
Employee slipped in mud and struck the corner of their thumb on the sharp corner of a piece of sheeting.
Employee was climbing on side of truck dump getting ready to weld and piece of existing 8" channel broke loose and pinched 3 fingers. Employee was wearing metacarpal gloves at the time.
Employee was cutting a tree when the tree split and fell on them, breaking their leg
Employee was driving on Rt. 60 toward Montgomery area when the employee slowed to turn right into prep plant, employee got hit by the back / side area.
Employee was operating an air impact wrench. The Snap lock ring broke on the wrench causing the socket to come off. The socket struck the employee in the mouth causing a laceration. The laceration required 6 sutures.
Employee was cleaning out a belt chute with a slate bar. A rock broke loose pulling slate bar away from employee's hand and pinning it against the chute. This resulted in a laceration on the left thumb that required 5 sutures.
Employee was mounting an 850 John Deere dozer, lost footing due to muddy conditions and fell against dozer tracks striking rib cage.
Employee was cleaning out a belt chute with a slate bar. A rock broke loose striking the employees hand and pinning it against the metal wall and the slate bar. This resulted in a laceration that required 7 sutures.
Claim for Occupational Pneumoconiosis due to alleged coal dust exposure.
When support column was loosened it fell and hit employee's right foot/heel causing a cut to employee's right heel.
A fire at the Mammoth Magnetite Plant storage belt stacker. The plant was being deconstructed by Powell Construction. At approx 6:20pm a pedestrian traveling Rt 60 noticed smoke and stopped by Mammoth Guard Shack and notified the guard on duty. The time of the fire and duration are unknown, however minimal damage occurred.
Employee was installing grizzly bar on the down river crusher. EE picked the bar up with an electrical hoist. When putting the bar into place the EE pinched right thumb between the grizzly bar and the frame of the crusher. This caused the thumb to burst open and caused a laceration that required 8 sutures.
Employee was attached a 4' section of 4" threaded pipe to a 90 degree pipe el and it slipped when the threads did not catch and part of the threaded portion struck employee on the left wrist.
Employee was helping 3 other employees drag metal grating across the plant floor. Employee felt a sharp pain in lower stomach. Employee continued to work, and was diagnosed with a hernia on 10/27/2016. The employee has been released to work regular duties until 1-13-2017 when employee will go before a surgeon to have the hernia repaired.
The operator was notified on 5/24/2016 that the former employee (Last exposure date worked 10/2/2015) has been diagnosed with Occupational Pneumoconiosis.
Employee bent down to pick up a piece of 8" channel, he felt something pull in his lower abdomen. He continued to work his shift. On 10/9/2014 the employee decided to go to the doctor nothing was found. The employee continued to work until he returned to the doctor a second time on 11/7/2014 where a hernia was found. His first day missed work was 11/10/2014.
Employee was changing out the dryer basket in #2 dryer when his hand slipped, and rammed the basket. The basket hit his hand cutting through his glove, and causing a laceration, resulting in the employee receiving 5 sutures.
Employee was working at tail of #119 belt and fell while he was stepping backward. He fell striking his lower back on material.
EE was removing bolt out of taper lock bushing with an air impact wrench. As he held the taper lock the bushing shattered, cutting the palm and thumb of left hand. Was wearing leather gloves.
Employee was removing snap ring off track take up on D8 dozer. The snap ring slipped and cut his index finger on left hand.
Employee was helping remove falk fluid drive coupler when it fell off shaft unexpectedly and pinched the thumb of his right hand between the coupling and frame. His thumb was smashed partially dislodging his thumb nail. He received five sutures to attach his thumb nail. He returned to his regular job at full duty the same day.
Employee was mounting D10R dozer and slipped and fell to ground on his back.
Employee was shoveling out the down river feeder. When he pushed the shovel forward his arm caught a piece of metal on the feeder box causing a small laceration.
Member was cutting a piece of belt rubber with a razor knife. The knife slipped and caused a laceration to his left arm just above his wrist.
Employee was using a 4" portable grinder when a piece of metal shaving went up under his safety glasses into his right eye causing irritation requiring prescription eye drops.
Member was working to unstop a drain pipe that ran from second floor of plant to the basement. He entered a restricted area and was walking across a metal sub floor to reach the pipe. A section of floor broke and he fell 15' 9", striking a Catflock tank and sliding down it to the basement floor.
Member was removing roller on the 105 belt. Other ee was using a hammer to drive a bolt out. A piece of metal from the bolt broke off and struck him in the right forearm.
Member was pushing tin off beltline. As he was pushing the tin off it caught on a bolt head and caused a laceration to the palm of his right hand
EE was prying on a sampler lid with a slate bar. The bar slipped off lid, smashing thumb between bar and metal lid. This caused a laceration to his right thumb.
Employee was going from 92 decline screen to sampling column tails. While walking by 24 refuse screen walkway, he tripped over toe plate and scraped his shin on toe plate causing a laceration.
Employee was assisting with the installation of pipe in the plant when he claims he felt a stinging sensation on his elbow and noticed a spider crawling on his arm. Later a knot formed on his elbow containining infection.
Employee turned to reach for a chainfall when he stepped in a hole with his left foot causing him to sprain an ankle.
Employee was coming down the stairs in the plant and his foot slipped causing him to come down on the side of his right foot.
Employee was exiting truck and his foot slipped from the step; he was holding onto grab bar. He strained his right arm and shoulder.
EE was attempting to open a keg of bolts. In attempting to remove the lid, hisw hand slipped catching the edge of the metal lid.
While cutting off a 100 ft piece of 48" 4 ply conveyor belt from a belt stand, straight utility knife slipped off belt, cutting upper thigh on right leg.
EMPLOYEE WAS CUTTING BRUSH UNDERNEATH A POWERLINE ON SIDE OF A MOUNTAIN USING A CHAIN SAW CUTTING 1" - 2" TREES/BRUSH. CHAIN SAW KICKED BACK AND CAUGHT HIS KNEE. EMPLOYEE HAS BEEN WORKING AS A TREE CUTTER SINCE HE WAS 16 YEARS OLD.
An employee was lifting a water pump discharge line out of a magnetite the cleaner with a 30-40 lb. flange on the end when he felt a sharp pain in his low back.
EMPLOYEE WAS INSTALLING A FREQUENCY DRIVE IN AN EMPTY SPOT IN AN ELECTRICAL CABINET. AS HE WAS TIGHTENING THE BOLTS, THE WRENCH GOT INTO A BUSS BAR AND CREATED AN ELECTRICAL ARC CAUSING BURNS ON BOTH HANDS.
EE ALLEGES THAT WHILE HELPING LIFT PUMP, HE FELT PAIN IN HIS CHEST.

EMPLOYEE ALLEGES THAT APPROX 3 HOURS AFTER CUTTING SIDE CLAMPING BARS (POLYETHYLENE) HE STARTED EXPERIENCING TIGHTENING IN HIS CHEST AND SHORTNESS OF BREATH HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS TREATED AND RELEASED.
WHILE SETTING A GRID COUPLING HUB ON TOP OF MOTOR HE SET IT DOWN ON A GREASE FITTING, CATCHING HIS RIGHT RING FINGER BETWEEN GRID COUPLING HUB & GREASE FITTING.

EMP HAS FILED A STATE WORKER'S COMP CLAIM FOR ALLEGED CARPAL TUNNEL SYNDROME.
EE STATED THAT WHILE LOADING A 16" CULVERT PIPE INTO A 988 LOADER BUCKET WHEN PIPE WENT INTO BUCKET THE PIPE ROLLED AND HE A HOLD OF THE END OF THE PIPE, IT CUT HIS SMALL FINGER ON RIGHT HAND.
EE WAS MOVING A PIECE OF ROCK FROM THE BELT LINE. WHEN HE ROLLED THE ROCK ONTO THE WALKWAY, IT BOUNCED AND PINCED HIS HAND BETWEEN THE ROCK AND THE METAL WALKWAY FRAME.
WHILE GETTING OFF OF THE SERVICE TRUCK, HE JUMPED OFF REAR BUMPER ONTO THE GROUND TWISTING HIS LEFT KNEE.
EE STATED WHILE COMING DOWN STEPS AND TAIL OF #1 REFUSE BELT HE STEPPED ON A PIECE OF KEY STEEL THAT WAS LAYING ON STEP, CAUSING HIM TO SLIP AND TWIST HIS LEFT KNEE.
EE STATED WHILE HELPING REMOVE THE MOTOR FROM THE STOCKTON FEEDER IN THE PROCESS OF PUSHING AGAINST THE MOTOR, HE HIT HIS BACK ON A BOLT.
