While cleaning a back-up camera, employee fell approximately 3 feet from the side ladder, striking EE's left shoulder. The employee received only diagnostic care until January 21, 2026, when EE began physical therapy.

Working on #2 separator, the miner was installing a lower distributor table while putting in bolts, laying on back, and working overhead, a foreign object fell in EE's eye while wearing safety glasses. EE was prescribed eye drops.

Miner was welding utilizing an approved auto darkening lens welding helmet and ANSI approved safety glasses, when a piece of metal entered their left eye. It was not embedded, but prescription medications were provided.

While loading the Rod Mill with 11'X4" steel rods, one of the rods fell out of the cradle and struck the miner on top of miners right foot.

The injured miner was placing an opened bag of Portland Cement next to the project, when a plume of the power like contents of the bag struck EE in EE's left eye. The miner washed EE's eye out at eye-wash station and received prescription pain medication at emergency room. The employee returned to work the next day.

Supervisor was checking operation of automatic sampler. While holding the retractable rod connecter to the sample container His hand was pulled towards the opening. Given the manner he was grasping the rod, thumb pointing towards opening the sampler pulled his hand into the side injuring his thumb of Rt. Hand. Initial first aid indicates possible stitches only.

Black smoke observed coming from fluidizing fan of dryer. Dryer was not running. Due to unsuccessful attempts to start the dryer, fuel oil inside of combustion chamber was smoldering. Issue was contained inside of combustion chamber. Fire department called as a precautionary measure. MSHA was notified within 30 minutes of the unplanned event.

Employee was assisting in the placement of a bundle of rods. The rods slowly swung toward the employee and he put his right hand up to steady them. The rods inside of the bundle shifted catching the tips of his middle and ring fingers of his right hand. Per employee, butterfly closures were used.

Employee was carrying two pry bars and a chain pull up a set of stairs. When he got onto the 2nd floor, his right knee gave out. Employee had strained his knee approx. 4-6 weeks ago on a job. Diagnosed with a torn minicus. Minicus was repaired on 10/16/09. Employee did not miss work, but is on restrictive duty for two weeks.

Employee changed out propane tank (approx 65 lbs) from forklift. He lifted the full bottle, turned to his left and set the bottle onto the forklift. Later that same day, he began to experience pain in the right shoulder area and right arm. He stated that the only heavy object he had lifted on this day was the propane bottle. Employee waited for 2 days before reporting.

Employee was attempting to retrieve a tool from the back of a 4x4 utility cart(Pug). As he reached into the bed of the Pug, the operator started to back up, striking the employees right foot. This resulted in a broken leg. The backup alarm was working on the utility cart. The employee stated that a lack of communication and inattention were to blame. Restrictive duty.

2 mechanics were hoisting an 8 in. flanged fitting of discharge hose to re-attach it to a pump that had been repaired. One bolt was put into the flanged fitting to and secured in place with a nut to hold it in place. Due to the tension on the hose, the bolt popped loose causing the hose to break free hitting the mechanic in the lower lip causing a laceration.

EE was walking down the steps at the crusher bldg.EE made a right turn at the landing,his right knee "popped" and ee felt pain.He was not carrying anything,there were no unsafe conditions or unsafe acts.Incident happened on 4/10/08 and was classified as a first aid until 5/2/08,when EE recieved knee surgery.

Employee using a saw to cut trees and brush. Did not feel anything hit his face or eyes, but later felt an irritation in his left eye. He washed his eye out at 9:30 pm on 6/18 and at 5:00 am on 6/19. He the reported to plant management and was taken for med attn. It is possible that when he wiped his brow with his glove that he may have wiped the piece of rust into his eye.

On the first step of the ladder for the L330 Loader. Started the climb in a way that threw off center of gravity, using his left foot and left hand to "swing" his right foot and hand into a position. Employee's right hand was oily and slipped, he swung out, which put all of his weight on his left arm. He felt a pop in his arm.

Employee popped his right knee out of place doing maint. work on a loader. He indicated that the damage was from a pre-existing condition caused by a skiing accident and that the "popping" of his knee happened often. Injury was ruled a first aid at the time-no lost work days. Employee went for surgery on April 26, 2007 causing the first aid to become a LWC on April 27, 2007.

EE was cutting an airline on a Microsizer project. He laid his utility knife down without closing it. When he flipped the hose around to straighten it, he ran his left hand into the open utility knife, cutting his hand. This required 4 stitches. He returned to work the same afternoon.

Employee had climbed a ladder to access a platform in order to unplug a chute. As the employee was looking up, he stepped through the access hole, slipped and caught himself on a piece of grating. By grabbing the grating, the employee cut his right index and middle fingers which required stitches. Employee returned to work, next day, no restrictions.

EE CONTINUED TO WROK HIS SCHEDULED SHIFT.
