EE was attempting to climb onto the 850 John Deere dozer. He had placed one foot on the step. When he tried to place the 2nd foot on the step, that foot slipped in the snow. He was holding onto the handrail. When his foot slipped, it caused him to twist and fall against the tracks of the dozer, pulling a muscle and pinching a nerve in his back.

Employee went to trouble shoot a leveling sensor located at the top of a silo. He claimed up the ladder and open the hatch at the top of the silo. When he looked inside a gust of wind caught a hold of the hatch door (30" x 30" x 1/4" steel plate on hinges) striking him in the back of the head causing him to hit the bridge of his nose on the steel frame opening of the hatch.

Employee was cleaning the belt take up roller on the plant feed belt with a hammer. His right hand and lower arm got caught and pulled into the roller. This event is under further investigation.

Raw coal stockpile showed signs of heat. A front end loader was brought in to open up and investigate the situation. It was determined that there were several areas that were burning. After further evaluation, it was decided to contact the local fire department for assistance. Hot areas were seperated from the stockpile with mobile equipment and cooled down with water.

Employee exited loader to dislodge material stuck in grizzly. As he dislodged the material, his middle finger of left hand was injured. Medical treatment included pinning of tip of finger.

Employee was attempting to hang a trolley on an I beam. The trolley slipped and the employee attempted to catch the trolley as it fell. The employee went to the doctor and has diagnosed as having a rupture that will require surgery.

Employee was performing maintenance on the belt press. He attempted to lift the end of a roll with a come-a-long when he felt a sharp pain in the right shoulder. The pain persisted the rest of the shift with some tingling in his right arm and hand. He went to the chiropractic doctor the next day.

EE AND CO-WORKER HAD BEEN DISASSEMBLING ROTARY GEAR BOX TO FIND ORIGIN OF PROBLEM. THE COVER PLATE WAS STANDING ON END ON FLOOR OF SHOP WHEN IT WAS EITHER BUMPED OR BRUSHED BY A GEAR ATTACHED TO OVERHEAD CRANE. THE COVER PLATE FELL OVER ON EE'S FOOT, CO-WORKER WAS WASHING A GEAR AT PARTS WASHER WITH HIS BACK TO INJURED EE.

EE WAS WALKING DOWN A BANK TO CHECK THE WATER QUALITY OF THE DISCHARGE FROM A POND. HE SLIPPED ON A WET ROCK AND FELL, FRACTURING A BONE IN HIS LEG.
