On 7/29/16, employee and co-worker were re-welding guards over the drive shaft of the turbine pump located on the surface above the #3 Shaft. Shortly after welding was completed, an ignition occurred in the #3 Shaft and employee received significant burns and lacerations. The investigation of this event is on-going. Employee died from the injuries on 8/4/16.

Employee stated while recovering a torque arm from a belt drive, the arm contacted a steel beam causing it to jerk sideways striking employee on the right leg. EE refused 1st Aid and was transported to the surface by EMT. EE refused ambulance and left unassisted and ambulatory to seek medical treatment. EE was given time off work to seek follow up with orthopedic doctor.

Employee stated while turning lower left lift, the right corner rib rolled out striking employee in the ribs. Employee declined first aid and finished shift. EE upon exiting mine, traveled via POV to medical clinic. X-rays were inconclusive and EE was given time away from work. EE remains away from work at this time.

Insulation on a compressor caught fire, IE attempted to put out fire with gloved left hand, resulting in 2nd degree burns. Miner was fully trained, wearing PPE and acting in compliance with the law.

A malfunction of a slope car part occurred. NO persons were injured as a result of this incident. Production had not yet started and all employees were recalled to the surface. The primary and alternate escapeways were still available. The car was repaired at 09:45am.

Employee stated EE knelt to measure coal. Upon rising, EE's R knee popped, and felt pain when EE put weight on it. EE finished shift. EE refused ambulance and medical attention at this time. EE sought medical attention on 11-23-19 and referred to orthopedic specialist. EE visited specialist on 12-2-19 and given time off for further evaluation.

Employee stated EE was crossing slope cable. EE stated the cable moved and struck EE in the leg. EE finished shift and did not seek medical attention at this time. EE first sought medical attention on 11-25-19. Upon exam, an x-ray was ordered which indicated a fracture. EE has been referred to Orthopedic specialist.

A non-injury accident occurred when the slope car malfunctioned. Production was halted and people withdrawn from the mine. Both escapeways were available at all times. NO persons were injured as a result of this mechanical malfunction. The malfunction was corrected at approximately 3:00pm.

A non injury incident occurred when the axle to the weight car broke. All personnel were withdrawn from the mine. Both the primary and alternate escapeways were available for use. NO PERSONS WERE INJURED. The weight car was removed and the slope car was returned to service approximately 50 minutes later.

A malfunction of the slope car motor occurred. All workers were withdrawn from the mine. No injuries occurred during this event. The primary and secondary escapeways were both still available. The motor was changed and the car restored to service at approximately 8:00 pm on 8/9/19.

A non-injury event occurred when the slope car mechanically malfunctioned. Production was halted and men withdrawn from the face to effect repairs. Attempts to correct the malfunction were unsuccessful all personnel were removed from the mine until repairs are made. Both primary and alternate escapeways were available for use. NO PERSONS WERE INJURED DURING THIS EVENT.

Employee stated ee was walking next to belt line when foot became stuck in the mud resulting in a twisting motion to back causing severe pain. Employee was transported outside on backboard to waiting ambulance. Employee was taken to hospital and was given time off until 9/10/18.

During inspection, a cable connection was found to be installed not according to manufacturer's recommendations on the Emergency Hoist. All EE's were withdrawn from the face area to outby the feeder dumping point while adjustments were made. No persons were injured or killed during this event and the alternate escapeway was still available. The repairs were completed in two hours.

Employee stated was walking around scoop when EE tripped on a box and fell across the bucket of a scoop striking EE's ribs. EE finished the shift and sought medical treatment and was given a full release but was diagnosed with a rib fracture. EE returned to work on EE's next scheduled shift and has not missed any work.

Employee stated was shoveling belt in sloppy conditions. As EE was dumping a shovel onto the belt, the mud adhered to the shovel causing it to contact the belt. When EE jerked the shovel back, EE felt a pain in lower left abdomen. EE exited mine and refused transportation. EE traveled to ER and was referred to specialist. EE is off work for unspecified amount of time.

Employee stated the shuttle car canopy contacted a curtain board breaking it in half. The inby end of the board sprung under the canopy and as the SC moved forward, it forced the board toward the employee. The end of the board struck the employee under the jaw causing a laceration. EE controlled bleeding and traveled outside. EE waived amb and drove to hosp where EE recv'd sutures.

A mechanical failure of the slope car occurred resulting in the emergency braking system to lock up. A grounded cable to the brake coil was discovered and a new wire was installed. Production was halted, men withdrawn to outby the dumping point until repairs were completed at 11:17 am.

Employee stated was downstacking structure on rail flatcar when another structure rolled down & struck right hand causing a large knot on the back of hand. EE & co-worker were working on each side of the car. 48' structure fell about 12'18' hitting EE in right hand which was holding another piece of structure.

Employees stated was picking up a cinder block when another block fell off pallet striking the ring finger on left hand. EE refused first aid and exited mine. EE refused ambulance and traveled via POV to LRMC ER where EE had a X-ray and found that EE received a broken fourth distal digit. EE was given 3 days off work and referred to a specialist.

Employee stated was assisting setting the feeder. When the feeder was properly positioned the coworker lowered the feeder onto the tailpiece. At this time, the tailpiece chute suddenly moved catching the right thumb between the tailpiece and the chute. The EE applied a dressing to the wound and finished the shift. After work EE received 9 sutures and was released to return to work.

EE stated went to tear down fly pads outby in #5 Entry, when EE turned to take a step after tearing the fly pad down, EE said EE stepped in a hole and turned ankle. EE stated that it was wet and muddy in this Entry. Employee was transported to RGH by ambulance.

Employee stated ee had already installed 2 bolts in row and decided to install a spot bolt next to the rib. After installing the spot bolt, ee swung in and a rock 48" x 32" x 4-5" fell out between bolts hitting ee in the back. EE went to Princeton Community Hosp. ER. X-ray indicated possible fracture of C-7 and ee was given time away from work and referred to specialist

Employee was assisting 2 others in rotating the drums of the CM when they felt pain in the lower back/hip. EE refused 1st aid, waited for a short time but then exited the mine to recuperate. EE traveled home but on 2/10/18 at 4:45 pm stated they were going to RGH ER to be checked. After examination, EE was give time away until 02/16/18 or sees a PCP. EE remains off work.

A roof fall was discovered at apx. 7:00am on the 3 south mains panel, 7c/c, #6 entry between spad #4128 & #4179. The fall measured apx. 74 in length, 20 in width and 2 to 8 in thickness. There were no injuries or interruption in ventilation. The area was dangered off, the HV cable was re-routed and timbers were set preventing access to all approaches of the area.

Employee stated was cleaning sprays on CM. As EE stepped to the back of the CM to help a coworker, EE turned back to the head and struck the back of left hand on a sharp piece of metal causing a laceration. EE was transported to ER and received 6 stitches. EE returned to work on next scheduled shift.

A contractor Employee stated was sliding track ties under the rail when the rail dropped pinching left ring finger. EE applied a rag to the wound and refused further 1st aid. EE was transported to the surface to waiting ambulance and then to Raleigh Gen. Hosp ER where EE received stitches. EE remains off work. EE was not wearing gloves at the time.

Employee stated was drilling hole in 4L cc. While installing finishing steel in the hole, a piece of rock fell pinching right thumb between the rock and pusher steel causing a crushing injury. EE was transported to Welch hospital. EE was transferred to RGH where EE was admitted overnight. EE underwent surgery and was released the next day. EE remains off work at this time.

Employee was pulling CM slack into heading for left crosscut when the slack fouled causing them to be pulled backward onto their left knee. After throwing the cable down EE tried to stand and felt a pain in the left knee. EE was transported to surface to ambulance and to hospital. EE was referred to ortho dr and remains off work at this time.

A CM cut the fiber optic cable used for tracking. The dispatcher notified the RP and all personnel inby to begin manual tracking procedures. The repairman was summoned to make repairs. Approaching the 12hr limit mentioned in the ERP, the hotline was notified at 12:46pm Ref#00793039. No persons were injured or trapped. The cable was repaired and tracking restored at 3:00pm.

Employee was squaring end of belt for splice when a piece of rock fell from the top striking them on the left arm. EE was administered 1st aid and transported to surface and then to hospital ER. Diagnosis was broken left ulna and laceration. EE fracture & wound was treated in surgery and was admitted to hospital.

An electrical malfunction caused fuses to blow to the emergency hoist. Production was halted, personnel were withdrawn to the feeder line until repairs could be made. The back up power supply was initiated at 10:04am and the hoist returned to service. No persons were injured or ill during this incident

After arriving at home, employee called wishing to seek treatment. Employee visited ER and received X-rays which were negative and discharged. EE visited PCP on 6/7/17 and was given time off to assess possible pulled muscle.

Employee stated while riding a Mac8 to the section, the driver backed into and dislodged a timber which fell striking ee on the hardhat. EE did not loose consciousness, refused backboard and was transported to the surface by EMT with a cervical collar in place. EE was transported to hospital via ambulance and diagnosed with a concussion and given one week off per protocol.

A wheel on the weight car attached to the slope car broke off. Production was stopped and all miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate escapeway is still travelable. The weight car was removed from the slope car and the slope car was available at 4:12pm. No persons were injured or killed or trapped during this incident.

The slope car used for hoisting men in/out of the mine had an electrical malfunction. A production was stopped and miners withdrawn to outby the feeder. The primary escapeway is still available and the alternate is still travelable. No persons were injured or killed. The slope car was repaired at 10:55am

The slope hoisting car that hauls men in and out of the mine went down. A bad battery was found to be the problem. The slope car was fixed at approximately 12:30am. Men were still able to get out of the mine and no portals were blocked.

Employee stated was assisting persons build a stopping when air pressure pushed out the stopping. A block fell striking EE on the hand. EE exited the mine and sought medical treatment. EE was referred to a specialist and was released back to work. EE worked several shifts and returned back to the doctor and began missing work 02/02/2017.

Employee stated was cutting bands from around a pallet of cinder blocks when the cut end of the band flew up and struck employee on the face under safety glasses. Employee exited the mine, sought medical treatment and was referred to a specialist. The specialist prescribed the employee with eye drops and fully released the employee to return to work.

This is a non-injury incident. The slope car had an electrical malfunction at 3:59am and was temporarily out of order. All underground personnel were notified with section crews withdrawn to the fresh air congregating point. An electrician was sent and a transformer was replaced. The car was running again at 4:21am

An electrical malfunction in the slope car control system caused a fuse to blow. Electricians were dispatched immediately to resolve the issue. The mine was not producing at the time. Only 10 employees were underground at the time and they were evacuated until the fuse was replaced at 3:00pm.

Employee stated, was roof bolting in 5R c/c when struck by loose rock which fell during the bolting cycle. Rock impacted personal canopy and then pushed employee to ground. EE was transported to surface by EMTs and taken to RGH ER. EE was treated and released but returned to doctor 2 days later and is away from work until 6-3-16

Complete automated repairs were completed at 12:53am.
