He was breaking rock & coal off the roof bolter cable. He was using a short roof bolt & a hammer. While doing so he missed the bolt striking his hand. He finished the shift and went to the Dr. the next day. He got a return to work slip for 6/18/13.

EE was performing checks on the dust sampling device being worn by the miner operator. When he walked by the turn table of the machine a rock meas. 74" x 39" x 8" thick fell and struck EE on the left side of his body, resulting in a chipped distal femur, fractured knee, tibia, fibula and multiple lacerations to the face and head.

EE was taking cable down as roof bolter was backing out. The operator turned the cable reel on and the cable jerked off the hangers, striking him and driving him to the ground. He continued working his regular schedule through 1/3/11, then Doctor ordered a 20 pound maximum lift.

EE was riding in a mantrip that was pulling a broken down mantrip. He was sitting on his side when the chain pulled tight. He ALLEDGEDLY hit his hip on a piece of metal on the back of the seat. This injury was not approved by Brick Street that occurred in the course of and as a result of covered employment until August 13, 2010.

EE was taking down the roof bolter cable that was hung. The offside roof bolt operator started the bolter and the cable reel was left on. The cable hit under his right arm pit, lifting him into the air, jamming his right hand against the roof.

On 3/9/06 the OP Medical Board findings of 1/12/06 did not make a diagnosis of OP. No award was granted and is not entitled to any medical benefits. Therefore ee should be removed from the OP list.

EE was setting a steel roof jack under a steel I-beam at the fan housing. Picked up jack & twisted his left knee & strained his shoulder. He returned to work on 8/22/05 but was unable to return to work on 8/23/05. EE HAD ROTATOR CUFF & KNEE SURGERY.
