Employee was attempting to fuel the D8 Dozer while standing on the crawlers. EE slipped and fell off the crawlers which caused pain in both legs. Employee was diagnosed with contusions and taken off work for 4 days.

Employee stated that EE was operating the dozer on the Raw Coal Stockpile when EE felt a pain in neck. Employee stated that EE felt pain when EE turned to look behind EE while backing the dozer up.

As employee was exiting the D10 Dozer at the platform, EE slipped and struck right knee against the frame of the platform. This caused a laceration to right knee requiring sutures.

Employee stated that while pushing refuse with a D9N Dozer, the dozer trammed over an offset in the material causing employee's neck to jar. this resulted in pain in employee's neck and right shoulder. The employee missed one scheduled work day and returned to work with a full release.

Employee was carrying a hydraulic jack and dropped it and attempted to catch the jack. When EE did, felt a muscle pull resulting in a torn bicep.

Employee strained his lower back while reaching for a water hose.

Employee was changing a hydraulic hose on a Caterpillar D8 Bull Dozer, in the process of tightening the "C" flange clamp on the SAE Code 62, Caterpillar, 4-bolt flange, his wrench slipped causing him to hyper-extend his right arm, he felt a pop in his right shoulder tearing his rotator-cuff/the rotator cuff tendon/glenohumeral joint

Employee's finger was pinched between two compressed gas cylinders.

Employee stated that his loaded haul truck slid into the ditch along the haul road during icy road conditions and he strained his lower back while holding on to the steering wheel. He finished his shift on the day of the incident, returned to work the next day and completed that work shift. Medical records were obtained on 3/22/10. Incident is under investigation.

This person has filed an occupational Pneumoconiosis claim with Brickstreet Mutual Ins. His last project assignment was at Apogee Coal's Fanco Plant.

Employee was using a slate bar to free up a lump of coal that was stuck in the feeder area. The coal released suddenly and the slate bar struck the employee's left thumb, resulting in a laceration and fracture.

Employee was wakling down a grated walkway. The grating was wet and he slipped and landed on his knee. He did not report the injury until 5/28/09 and didn't seek treatment until 6/12/09.

Employee states that he stepped on a piece of metal and slipped scraping his left knee and causing fluid build-up and bruising when he fell. Employee continued working his shift and did not seek medical treatment until 2/7/09 when he was put on restricted duty.

Employee was walking down a slope to his work area. He was carrying some rebar. The ground was wet and he lost his footing. He strained his Right Ankle, Right Hip, and back.

Employee suffered a fracture to his left foot when he was struck by a crusher motor. The motor shifted and dropped as it was being hoisted.

Employee was using a slate bar to pry a lodged piece of metal loose in the raw coal chute to the #2 raw coal vibrator. The bar slipped, allowing his hand to strike against the flop-gate lever on the plant feed belt discharge chute.

Bending metal plate on truck bed, while swinging the sledge hammer, something in lower stomach pulled.

Employee was coming down from loadout while it was snowing and expresses he slipped and fell upon his arm striking his right elbow. Employee was unattentive to his working conditions and failed to use proper points of contact.
