EE parked the wheel loader he was operating at the shop to repair damaged hose. EE descended down the loader ladder too fast and slipped on the last step about 20 inches off the ground. EE jumped back and landed on his back. Weather conditions were rainy.

EE DUMPED LOAD OF STONE WITH HAUL TRUCK BESIDE STOCKPILE. WITHOUT FULLY LOWERING TRUCK BED, EE TRAVELED APPROX 400 FEET TOWARDS AN UNDERPASS. TRUCK BED CAME IN CONTACT WITH OVERPASS, CAUSING EE TO BE JOLTED IN CAB. EE HAD CUTS TO L. WRIST, L. HAND & UNDER LIP ON CHIN AND LOWER GUM; NICKS ABOVE R. EYE & R. EAR; TWO CHIPPED TEETH.

3 STITCHES REQUIRED EE WAS WORKING WITH ANOTHER EE AT STONE PROCESSING PLANT CHANGING WIRE MESH SCREENS ON THE VIBRATING SCREEN. EE TRIED TO HANDLE ONE SCREEN CLOTH BY HIMSELF RATHER THEN GET THE OTHER EE'S HELP. SCREEN CLOTH CUT EE ABOVE LEFT EYE.
