Employee was performing maintenance activities inside the Barge Jaw Crusher. EE was standing on a piece of plywood that shifted, causing EE to lose footing and fall into the jaw area. EE was wearing fall protection and SRL (Self-retracting lanyard) that stopped EE from falling farther into the jaw crusher.

Employee and coworker were positioning a chute guard, weighing approx. 50 pounds. As they lifted the guard, one of the two employes handling the guard felt a pop in his left shoulder.

Employee was replacing bearing and stub shaft assembly. Employee put thumb between upper stub shaft and screw to pull out gasket. The movement caused the screw and stub shaft to come together pinching the employee's finger.
