Employee initially stated that they began welding and the auto darkening feature on their helmet was delayed, which they believed caused a burn to the eye. They sought evaluation today, 1/7, and was diagnosed with a foreign body of the right eye.

Employee was traveling on a haul road in a water truck when they hit an oversized speed bump and it catapulted them up and they hit the ceiling of the cab. Employee was not wearing a hard hat (no room).

Using a pipe wrench to break loose a plugged piece of equipment. EE stated EE was putting pressure on it, it broke loose and EE's left hand got caught between the pipe wrench and shiv. EE stated it crushed EE's hand in the same place of EE's previous fracture from last year.

Employee was servicing equipment, EE started to descend the ladder. When EE got to the last rung, EE's left foot going to the ground and as EE was letting go of the rails with EE's hands and stepping down with EE's right foot...EE caught EE's boot solidly on the last step which caused EE to fall back. EE's body twisted as EE was falling, landing EE on EE's right hip and back.

EE was replacing a roller on a conveyor in the Auburn pit. As EE was pulling it out to turn it the roller pinched EE's right index finger between the roller and an angle iron on the conveyor.

While using a drill press the sheet of metal was clamped down but started to move. EE tried to secure it with EE's left gloved hand. As EE was securing it EE's glove caught the drill and entangled EE's left hand and thumb into the drill.

Miner was at an off site location, picking up debris that was dumped illegally. Was lifting a piece of concrete debris, and strained a muscle in his back, causing some pain and stiffness. It was decided to have the miner's back looked at by a physician, out of caution.

Miner was putting a bearing housing on a press table, to press out bearing. Pryed up one side of housing with a bar and a small block to get a better bite on housing for positioning over press. Housing slipped off the pry bar and slid and pinched miner's finger between housing and press table, resulting in a cut finger, requiring stitches.

Off-hwy end-dump truck veered off roadway onto the 8'+ shoulder, struck a buried drainage pipe, traveled up the steep slope adjacent to the shoulder, struck a boulder (approx 5' in dia.) & power pole, turned over onto its driver's side & came to rest on the side of the vehicle with the driver's side of the bed of the truck resting on the boulder.

Employee was clearing rocks that were stuck inside the crusher jaw. He was using a wedge and a cable to clear the rocks and once the jaw was clear he began pulling the cable up. At this time a rock fell causing the cable to tighten and pinch his left thumb between the cable and a steel frame. The employee received 7 stitches and was released to full duty.

Employee was walking to refill a grease gun. The building where the grease is located requires walking down a slope. While walking to the building employee slipped on a rock, lost his balance and twisted right knee. The employee felt a pop and instant pain in right knee.

Employee was changing the screens at the crusher when he lost his balance and the screen fell back on his left kneecap. He called our occupational nurse who advised him to ice and take ibuprofen. Two days later the nurse advised him to see a doctor. He went to the clinic on 1/24/08 and was diagnosed with a knee contusion.

Employee was cleaning a powerscreen for transport when he noticed a piece of steel cable stuck between the screen and head pulley. As the emplyee attempted to free the cable it whipped around and struck him on the hand.

Employee was working on the Classifier Cylinders at the Crusher and he knelt down on a steel sheet which had rocks on it. His knee landed on one of the rocks. He reported incident to his supervisor. Supervisor had employee call company provided 24 hour nurse for advice on injury. He followed instructions. On 7/13/07 the nurse advised employee he should go see a docotor.

7/5/07 employee reported he had a non-work related shoulder injury and couldn't work. Employee states he noticed pain at home the night of 7/3/07. 7/16/07 employee saw his Dr. 7/18/07 employee reported his injury was work related but does not know how or when it occured. Employee put on light duty 7/19/07. Dr referred employee to surgeon for possible torn rotator cuff.

While cutting a piece of angle iron with a chop saw, the blade jammed which pulled the angle iron. The iron cut the employees gloved finger. He recieved 4 stitches and returned to work with modifications.

In the afternoon on 10/20/06, employee noticed a small cut between 2 fingers on his right hand. Over the weekend it got red and swollen. On Sunday, 10/22/06, employee called his supervisor and made arrangements to see a doctor. Employee was given antibiotics and a tetanus shot then released to regular duty.

While using a utility knife to cut a zip tie off an extension cord, the knife slipped and cut the employee's left index finger. He recieved 5 stitches and was released to regular duty.

Employee was installing tail pulley on crusher, employee tried to adjust his grip and the pulley dropped on his left hand injuring the ring and pinky finger. The incident occured on 5/18/06 employee saw Dr. 5/19/06. Contusion on hand and fracture to finger.

Employee was in a manlift vertically fitting a 2" pipe that was 18" long. Employee welded a tack and as he adjusted the pipe the tack broke and the pipe struck his upper lip. Employee recieved stitches and was released back to regular duty. All appropriate PPE was worn.

4/4/06 employee reported that he went to his Dr. on 4/3/06 because his left arm was bothering him. No X-rays were taken - employee was diagnosed with tendonitis of left arm and was put on light duty. Employee does not recall a specific incident that would have caused injury. Employee states that his arm had been bothering him since around 3-28-06 but did not report to employer.

Employee was operating a CAT End Dump, hauling up a slope when the load shifted to the back causing the front wheels to come off the ground. The load then dumped out and as the truck came back down the employee struck his head causing lacerations on both his lip and right eyelid.

During a cone liner change the employee was cleaning the crusher component with a wire wheel grinder and air nozzel. Debris (assumed metal) went in his eye. He then rinsed out his eye and later went to the doctor. He was wearing safety glasses at the time of the incident.

Ee reports that after lunch his hands started to itch. then on the weekend, ee reported his hands swelled and became more irritated. EE WAS OPERATING A CAT773 OFF ROAD END DUMP & REVERSING TO DUMP A LOAD OF SAND WHEN THE INNER FACING SIDE WALL OF THE LEFT FRT TIRE FAILED. THE RESULTING RELEASE OF AIR ROCKED THE OPERATOR IN HIS DRIVERS SEAT, AND DESPITE EAR PLUGS CAUSED DAMAGE TO HIS INNER EAR, LEFT ARM & LEFT SIDE OF BODY.

EMPLOYEE WAS GREASING A FLOATING PUMP STATION WHEN THE RAFT TIPPED OVER CAUSING THE EMPLOYEE TO STRIKE HIS LIP ON SOME PART OF THE RAFT AND THROWING HIM INTO THE WATER. COUNTER WEIGHTS PREVIOU SLY PLACED ON THE RAFT TO HELP INCREASE PUMPING VOLUME. THESE WEIGHTS CAUSED THE INSTABILITY OF THE RAFT ALLOWING IT TO FLIP EASILY. EMPLOYEE INJURED LIP. NO PROPERTY DAMAGE.

EE WAS IN MACK TRUCK BEING LOADED BY A LOADER. WHEN THE LOADER WENT TO DUMP A LOAD OF G ROCK INTO THE DUMP TRUCK, THE WETNESS OF THE MATERIAL CAUSED IT TO COME OUT ALL AT ONCE. THE IMPACT OF THE WET MATERIAL CAUSED THE EE TO GET BOUNCED AROUND INSIDE THE CAB OF THE TRUCK HITTING HER HEAD AGAINST THE DOOR. CAUSATION: POSSIBLY LOADING WET MATERIAL TOO FAST.
