EE was sacking 100 pound bags and stacking them on pallets. He felt overloaded with the amount he was sacking and he started feeling pain in his lower left lumbar area. The same illness was reported back in August of 2009, therefore it is a recurrence. He will be placed on modified duty until further notice.

EE was in the process of stacking 100lb. bags on to pallets. He was completing stacking bags on the last pallet of his shift when he felt a pain in his left shoulder. He reported that he felt fatigued and the product was coming faster than he could handle.

EE was cleaning the overchute above the cone crusher and was pulling the rock away and then felt a sharp pain starting at his shoulder and ran down his arm. This incident is considered an illness, due to the fact that a previous injury occurred to the same shoulder back in 1992.

Injury strained his back while climbing a ladder. He has had previous problems with lower back strains and inflamation prior to this incident. He did not have what would be considered an accident. It was considered a strain due to his physical condition and age. The original accident occured on 7/28/09, but was not diagnosed until 8/3/09.

During inspection of dust collector baghouse, employee got dust in left eye. Employee was wearing proper PPE provised for performing this procedure. Employee was wearing half mack respirator nstead of full face respirator provided. *Accident not reported until 8/19/08.*

While installing a haul road warning sign, employee stepped onto berm to pack dirt around the sign when he felt a sharp shooting pain in right ankle. Diagnosed with soft tissue strain or sprain to right ankle.

Employee was examining bad skirting and cut his hand on the conveyor belt. During inspection of guarding and skirting on conveyor #9 (BC-09) the injured employee was directing the maintenance man to a section that needed repair by pointing his finger. The employee's finger became caught between the skirting and the conveyor belt causing a cut to his finger that required sutures.
