Victim,entered the property passing the No Trepasssing Signs with four other individuals. Our understanding is they began to swim in the pond and he drowned before reaching the shoreline. The incident occurred after the end of our shift which ended at 4:30 pm and no miners where on site at the time. Victim was NOT a miner or an EMPLOYEE of the mine.
3 employees were installing a roller on a conveyor. One employee lost his grip and the injured employee tried to maintain his grip, resulting in pinching of finger between the roller and conveyor frame.
Employee was replacing damaged conveyor. Employee made contact with metal conveyor that was energized.
Installing an I-beam in the shaker deck. Had to do very heavy lifting and standing in an awkward position. Strained abdomen. This incident did not become reportable until his first doctors visit on 3/24/08.
Operating forklift, wheel went in a rut and steering knob on steering wheel spun around real fast and hit left thumb, breaking left thumb.
Stepping down from loader. Missed a step. Fell to ground hitting head on step. Laceration to head with stitches.
Lost balance while crouched down - Kneeled on wrench. The incident was a first-aid only when it happened. It did not become reportable until 5-31-05 when employee went to the doctor to have knee drained.
Employee was clearing sand off the side boards of his dump truck with back of shovel. The wind blew and sand flew into employee's right eye.
Incident occurred on 6/5/04 and treated as First Aid only until doctor visit on 6/21/04. Left brake caliper was partially disassembled and on mounting pedestal. He bent down to pick up a fallen Allen wrench and caliper fall off and hit top of knee.
WHILE HOLDING A 3 FOOT LENGTH OF STEEL PLATING IN PLACE TO WELD, IT SLIPPED, STRIKING LEFT EAR.
HE WAS HOLDING THE BECKET BY IT'S EYES AND WAS TAPPING THE SHORT LENGTH OF PROTRUDING CABLE AGAINST THE DECK USING THE WEIGHT OF THE BECKET TO FORCE THE CABLE THROUGH. CABLE RELEASED AND TRAP PED 2 FINGERS.
CUTTING COVER OFF S.O. CORD. KNIFE SLIPPED, CUTTING FINGER.
SLIPPED ON LOADER STEP WHEN GETTING ON LOADER.
WHILE WASHING DOWN A CONCRETE SLAB, EE STEPPED BACKWARDS OFF SLAB AND LANDED AWKWARDLY ON SURROUNDING GRADE.
EE WAS ON LIGHT DUTY AND WAS HELPING WITH GENERAL MAINTENANCE ON THE DREDGE.
WAS PULLING WASH DOWN HOSE ACROSS TOP OF EMPTY TUNNEL. STEPPED ON SOME ROCKS WHILE PULLING & TURNED KNEE, TWISTED & POPPED OUT.
CLEANING OFF ROCKS ON TOP OF A GRIZZLY ON TOP OF A FEEDER ABOARD A DREDGE. HITTING A ROCK WITH A SLEDGE HAMMER. ROCK BROKE & HIT LEG.
CLIMBING DOWN FROM CAB, LOST FOOTING, CAUGHT SELF WITH RIGHT ARM.
SLIPPED OFF CRUSHED ROCK PILE WITH LOADER HITTING LEVEL GROUND HARD.
CLIMBING LADDER UP TO WINCH HOUSE, STRUCK LEFT KEE ON NEXT STEP UP.
STEPPED ON ROUND ROCK, ANKLE TURNED, KNEE BUCKLED, LEG FOLDED UNDER HIM.
EE WAS CUTTING A PLATE. PIECE FELL OFF OF PLATE, AT THE CUT, AND SWUNG WRONG WAY. PIECE FELL ON SHIN AND REQUIRED STITCHES.
CHANGING BRAKE ROTOR ON DREDGE, ROTOR SLIPPED OFF HUB AND PINCHED FINGER AGAINST OTHER HALF OF HUB ON MOTOR SHAFT.
