The individual was using a pry bar to align a head light on the Fletcher roof bolting machine. The bar slipped and caused him to strike his hand against the frame of the machine. This resulted in a fracture of his right ring finger.

EE had installed a bottom roller on offside of the belt. While crossing back over the belt he placed his left hand on the structure to step off the belt. EE felt a sharp pain in his elbow when stepping off of the belt.

The individual had hung curtain beside the roof bolting machine and when walking behind the machine he turned his ankle and felt his knee pop. He did not go to the doctor until 9/910 and was placed on restricted duty beginning 9/10/10.

The employee was building a brattice and as he was driving a cap wedge over a block, his hammer hit the roof and a small piece of rock hit him in the right eye. He was wearing safety glasses. He went to the doctor and had the foreign object removed. This is a medical only report. There were no restricted or lost work days.

Fall was discovered at 9:56 pm on 2/21/10. The fall is approx 750' outby active section in the #2 entry 1 crosscut outby SS 867. The fall is approx 20' long X 20' wide X15' high. The fall did not affect ventilation travel didnt not cause any injuries and did not trap any equipment. The area has been timbered and dangered off and the fall will not be cleaned up.

EE was operating the continuous miner in the # 1 entry. EE was sitting on his feet and a small piece of rock fell off the rib knocking him off balance and into the miner striking his face on the frame of the miner.

The individual was on his knees putting cross overs on the miner cable. When he raised up, he hit his right ear on a rib angle cutting his right ear. The cut required stitches.

The injured and another miner were setting roof jacks along the haulway at SS #888. When the other worker took a jack from the jacks piled along the rib, another jack fell on the left foot of the injured.

A fall was discovered at 8:40 pm on 7-26-09 by foreman who started his shift at 6pm to preshift. The fall was in the #3 belt entry at SS No 881 of the No. 1 main west set of entries. The fall measured 20' wide by 25' long by 15' high. The fall will be cleaned up and the area supported.

The injured was hanging the electrial cable to the roof bolting machine. The machine operator engaged the cable reel and caused the cable to knock EE down to the mine floor.

The continuous mining machine finished a left run out of the entry while pillaring. The injured was preparing to reposition the mobile roof supports when the mining machine accidently struck a piece of metal on the MRS. The metal shot off unexpectedly striking him on th right forearm. This resulted in a laceration which required sutures.

The employee was using a 3-wheeled rubber tired personnel carrier to pull up extra cable for the roof bolting machine. When he cut the rope loose from the 3-wheeled machine, he cut his left pointer finger. The laceration required sutures.

The fall was approximately 1,400' ouby the working section and will not be cleaned up. There were no injuries or equipment involved.
