Employee was operating a roof bolter when his finger was mashed between rotation hose fitting and boom frame of roof bolter, causing fracture and laceration to his left ring finger.
EMPLOYEE WAS OPENING A BUCKET OF MINER BITS WITH A UTILITY KNIFE. KNIFE SLIPPED AND CUT HIS LEFT ARM JUST ABOVE THE WRIST CAUSING A LACERATION THAT REQUIRED STITCHES.
EE had placed his hands on top of roof bolter canopy to pull himself up to when a piece of draw rock fell from overhead onto his hands causing lacerations to his fingers on his right and left hands that required stitches.

Employee was installing roof bolts, he was positioning roof plate with his left hand as he pushed up bolt his finger became lodged between roof and roof plate causing a laceration that required stitches.
EMPLOYEE WAS WALKING ACROSS SECTION WHEN HE STEPPED IN A HOLE CAUSING A SPRAIN/STRAIN TO HIS LEFT KNEE. THIS ACCIDENT ORIGINALLY OCCURRED IN AUGUST BUT DID NOT BECOME REPORTABLE UNTIL 11/24.

Ee was checking center line when a rock fell from overhead striking him on the head and shoulder. He went to the local emergency room where he was examined, x-rayed and given a full return to work release. Ee continued to experience pain and became a lost time as of July 13, 2004.
