Shoveling in mud around belt drive. Got foot stuck in mud - lost balance & fell backward against rib, breaking right shoulder.
During pre-shift, found roof fall at break 68 & 69 outby spad 809. Fall 6x20x40.
When putting breaker in, breaker and/or recepticle flashed creating burns to hands and face.
#5 belt-intersection fell-no.5 entry 1 crosscut outby survey station no. 611. non injury.
SLIPPED AND FELL WHILE SETTING SPAD NEAR FACE. TWISTED HIP AND BRUISED HIP BONE.
