Deep Mine #26 Coal

Employee was bolting the 2nd cut out of 4R. While installing a rib bolt in 3rd row of bolts a piece of draw rock measuring 70.5" long, 0" to 6.5" thick and 46.5" wide fell out from the top and the edge of the rock caught the back of EE's right heel causing an avulsion. **Also fracture to heal bone.**

Employee was pulling a miner cable pull rope to attach it to the tail boom of the miner and fell after becoming entangled in the miner water line. Employee fell backwards on to left buttocks. Employee is complaining of pain near tail bone radiating down left leg. Employee continued to work until 6/21/17.

Employee was loading high voltage cable. The employee tripped over the cable causing them to fall landing on the left arm resulting in a fracture of the left radius.

Superior Coal Services Contractor was attempting to go through a mandoor. EE states, pushed the door open to step through. Stuck hand out to catch the spring loaded door and when it came back the locking mechanism of the door had a sharp corner that hit right pinky finger causing a laceration requiring stitches.

Employee suffered an injury to EE's right shoulder on 10/15/15 with a previous employer which required surgery. EE was released from the Dr. on 9/12/16 and was hired at this mine on 9/20/16. On 9/23/16 EE started having pain in the same shoulder and sought a medical evaluation and was diagnosed with Tendonitis and/or Tenosynovitis on 9/26/16 from EE's previous injury.

Bates Contractor was helping hang miner cable. When lifting the cable up to the roof he felt pain in his lower abdomen. This resulted in a hernia.

Employee was moving roof caps (pizza pans) to drill operator station when he twisted around to put the pans on the drill station he felt pain in his lower back and right thigh.

Employee states while traveling downhill a hand drill became loose trying to fall off of man trip. When he turned to secure it he veered off of travel way striking his ear on a roof pan hanging from the roof and cutting his ear. This resulted in 5 stitches.

Bates Contractor was operating Fletcher (DDR) walk thru roof bolter installing wire mesh in the #4 entry on Northeast Mains. His left arm became entangled in the rotating drill steel/chuck causing amputation of the arm at the elbow.

Bates Contractor was installing wire mesh. He drilled his hole, put the glue and bolt in the hole and dropped it down to the drill chuck. He stated it didn't line up and was fouled to turn by hand so he bumped the rotation and it twisted his hand around the roof bolt. This resulted in dislocation to the left ring and pinky fingers and a laceration requiring stitches to his pinky.

Employee states he was trying to get finisher piece of steel out of hole. When he pulled, the steel didn't come out but he came off the steel with hands and felt pain in his left shoulder and fingers.

Employee was removing a canopy from a drill that was stuck. The employee was rocking the canopy to loosen it. When the canopy came loose, it slid to the side. He caught the canopy and lowered it to the ground causing pain in his right shoulder. He continued to work up until he had surgery on 7/21/14.

Employee states he was closing lock blade utility knife and he closed it on his middle finger on his right hand resulting in a laceration requiring stitches.

Investigation of the primary escape way air shaft revealed that large amounts of ice removed from the shaft liner has fallen and broke the left guide rope eye bolt anchor provided for the emergency escape hoist at the bottom of the air shaft.

Employee stated that while he was installing the middle bolt in the second row of bolts in the #1 heading, the pan on the 5' resin bolt caught on a piece of metal jamming his right wrist and causing pain to run up his right arm to his shoulder.

Employee had raised the cover over reel to check the cable and left it in a raised position. He dropped the remote box for the MRS and when he bent over to pick up the box he placed his right hand on top of the MRS frame. The lid fell catching his right little finger between the frame and lid causing a laceration resulting in stitches.

Employee was walking around the end of a fly pad when he struck his head on the rib. He continued to work up until 1/17/14.

Employee states he was coming on to the feeder when he hit a hole causing his head to strike the canopy. His body came down hitting his tail bone on edge of the seat. This resulted in pain in his lower and center back, hip and left leg.

Employee was loading duster with a bulk bag of rock dust. As he was cutting the bag open his feet slipped causing his hand to slide down the utility knife he was using cutting his middle and ring finger on his right hand. This resulted in 5 stitches to each finger.

Employee was operating tractor coming up slope against the air current. Employee had his hand on bracket that holds the windshield in place when in up position. The windshield was down and the tractor hit a hole causing the windshield to bounce up and the air current caught it which forced the windshield to up position pinching the employee's right hand resulting in 6 stitches.

Employee states that he was hanging MRS cable when he felt pain in his lower back, hip, groin and left leg.

Employee was helping another employee clear miner cable for roof drill when he felt pain in his right pelvic region.

Employee was walking in walk way toward the rear of drill. As he approached the rear, he leaned forward (due to height) with his hand on the drill. The drill ran over some gob causing his elbow to lodge in the top causing injury to his right wrist.

Employee was starting to drill first hole. A piece of draw rock fell striking him on the right shoulder then striking his right knee resulting in pain and swelling to his knee.

Employee was installing a rib bolt. He was starting the hole and had the head tilted over. Safety arm guard wrapped around drill steel which resulted in constricting the material around his arm.

Employee stated that he walked into a curtain board striking his hard hat. He felt pain in his neck and numbness in his left arm. He continued to work until the doctor took him off on 12/18/12.

Employee was drilling a hole for installing a rib bolt when the steel broke and struck the employee in the face.

Bates Contractor was drilling a test hole when the steel broke. The broken steel whipped around cutting him under his right eye on the cheek bone. The cut required stitches.

Employee was in the process of cutting conveyor belt while making a splice. He was using a utility knife when it slipped hitting his right leg approx. 2" above the knee cap. This resulted in 3 stitches.

A roof fall approx. 60' x 50' x 20' high was discovered at the top end bleeder, 9C-Rt Panel off #2 North Mains.

Employee was installing the 3rd bolt on the 3rd row of bolts when a 2' long x 2' wide x 0 to 4" thick rock fell striking him on the hard hat and sliding down to his left shoulder.

On 3/15/2012 we were notified by worker's compensation that the employee was awarded a hearing loss settlement.

Employee was trying to remove a shear pin from a rock duster using a 6' bolt. He struck his small, ring, and middle fingers on augar mashing and causing a laceration resulting in 3 stitches to his middle finger on his right hand.

Employee was attempting to align steel to start drilling a hole. A piece of draw rock kicked out knocking hand from steel and mashing it against drill pot.

Received a CWP diagnosis for EE on 1/3/2011.

Employee was in the process of drilling anchor holes for tailpiece with a jack hammer when he pulled it up he felt pain in his lower back running down through his legs. He continued to work until 1/5/2012, when the doctor removed from work.

On 11/21/11 employee was hanging a block curtain in #2 entry when the drill opr. turned the cable reel on. His feet were in a loop of cable and swept him off his feet. He stated that he felt pain in his left shoulder. He had surgery on his shoulder on 4/3/12. We obtained confirmation on 4/3/12 that this was related to the injury that occurred on 11/21/11.

Employee was trouble shooting for a hydraulic leak when he reached and pulled on hoses and had several in hand. One of the hoses had a small hole. Under pressure while he was searching the hoses the hose ruptured and came in contact with his right hand causing a laceration due to the pressure from the hose.

Employee states while walking in the #3 entry he stepped in a hole and felt pain in his right knee.

Employee was running miner in No. 1 heading to right hand lift when a piece of rib rolled off striking him on lower back and across both ankles. Accident occurred at MMU 001, #1 entry 20' in by spad #19641. Rock was 12' 2" long, 25" wide and 12" thick.

Employee was swinging the drill boom in the canopy and struck his hard hat causing laceration to the ear.

As employee was loading shuttle car the rib rolled and struck him on his right hip resulting in a fracture.

Received notification of a CWP claim on 6/22/2011.

Employee was in the process of unlinking a chain when the cutter head moved causing the chain to tighten pinching the employees index finger; resulting in avulsion to the soft tissue of the finger requiring skin graft.

Employee was walking up the #3 entry and slipped in the mud causing him to twist his left ankle causing a fracture.

Employee was trimming insulation on a cable with a knife when the blade came out and cut his ring finger on his left hand. The laceration required stitches to close.

Employee was installing pogo sticks behind the line curtain when a piece of draw rock fell striking him on the base of the neck and shoulders.

Employee was checking the suction on the drill head. He placed his left hand on the drill head and the drill steel fell out of the hole striking him on his left hand.

Employee was lifting a piece of structure off of a structure car when he experienced pain on left side of his groin. Resulted in a hernia

Employee driving a mantrip in the outby area of the mine. While traveling through a tight area a pogo stick that was on the back of the mantrip came in contact with a ventilation control, caused the pogo stick to spring around hitting employee in the face. Employee received multiple fractures to the face area including nose, orbital bones, and upper part of mouth.

Employee was assisting in moving Shuttle Car cable slack up during a power move when his right foot got entangled in the slack as it was being pulled causing him to be pulled down twisting his right knee.

Employee was stacking crib block, while tossing crib block off the pallet his right middle finger was caught between two (2) crib blocks causing a laceration across the end of his finger resulting in stitches.

Employee was installing roof bolt when the bolt kicked out of drill chuck while rotating. As employee tried to push bolt back over the chuck, the bolt hit his right hand, resulting of bruising on the right hand.

Employee was pushing drill steel into hole when the steel broke, striking his middle finger on the left hand, resulting in a fracture and cut to tip of his finger requiring five (5) stitches.

Employee was starting to drill with steel in the mine roof when an undetectable piece of draw rock fell measuring 12X8 thick fell vertically with the end striking his right foot behind steel toe. The area employee was bolting in was 6 high. Injury resulted in fracture to the 4th metatarsal on his right foot.

Employee was loading supplies on roof bolter, when he felt a pain above his navel area. Employee sought medical attention and was diagnosed with a hernia.

Employee was taking a rock off of the miner using slate bar, when the slate bar slipped and caught his right ring finger between rub rail and slate bar. (Resulting in simple fracture and requiring 7 stitches.)

Employee was riding mantrip on the way to section when the mantrip hit a bump in the travelway. Employee felt pain in lower back.(Resulted in strained Lower Back)

Employee was standing on drill platform when coal slid down the T-Bar hitting him on the Right leg. Employee continued to work until 12/08/2010.

Employee was tramming a shuttle car to load at the miner when he struck a rock in the haul road with the off side of his shuttle car which throwed him into the canopy and back into his seat c ausing pain in his lower back.

A possible methane ignition occurred on 002-#1 section in # 5 entry 60' inby SS# 15043 during the first left lift off of #4 pillar block. MSHA and DMME were notified 103j order was issued by phone and modified to a 103k upon arrival. No visible evidence of ignition on rib or Miner.

A unintentional roof fall occurred at this location. Approx. 8000 ft off#1 Section, SS# 312, Left return. No ventilation was blocked, no escape way was blocked. Fall was approx. 20 wide 20 long and 8 to 10 high and will be cribbed off and not cleaned up. Fall was found during weekly examination. This area was mined 2/2003 and supported with 5 torque tension bolts.

Employee was preparing to do maintenance on miner. While placing cribs under the miner cat the miner fell resulting in amputation of multiple fingers on right hand.

Employee was walking by the front of the drill parked between #5 and#6 when the rib fell off the corner in #5 dislodging a pogo stick which struck him in the mouth cracking his tooth.

While installing a 12' cable bolt the employee pinched his ring finger on his right hand causing a laceration to the palm side of his finger. He stated he was unaware of how or when, but later stated that he may have pinched his fingers between two pieces of drill steel.

Injured employee was helping another employee hoist a pick breaker drive chain with a winch boom on a diesel man trip. The cable on the winch broke causing the drive chain to drop and strike injured employee on the right hand. (stitches)

An unintentional, non-injury roof fall was discovered on the North Mains section in the return. The fall measured 100' X 19' X from 10' to 12'. The fall will be cleaned up.

Employee was cutting wire with bolt wrench when he struck left hand between thumb and fore finger.

While attempting to lift a safety jack off of a roof drill, employee felt a sharp pain in his back.

Employee bent over to pick up a roof bolt to install when a piece of draw rock, measuring 4' X 19" X 4.5", fell from the top and struck him in the back.

Employee was leveling the bottom to start the first row of block, when a piece of coal measuring 4' X 2' X 3" struck him on the left forearm, causing a fracture.

Employee drilled a hole for the roof bolt. When he picked up the roof bolt to be installed, he turned to put the bolt in the hole and felt a pain in his lower back.

While helping slide the rub rail in place emloyee got his right middle finger caught between the rub rail and frame of the miner. This caused a fracture on the tip of his finger and also required stitches.

Employee experienced a "pop" in his right knee while helping to move crib block. Continued his regular job until surgery on 1/18/2010.

A non-injury roof fall occurred outby the #4 section.

Employee was climbing out of operator's compartment. He laid his hand on top of canopy, and his gloved hand was struck by a piece of rock.

An unintentional non-injury roof fall was discovered on panel 6 E. Ventilation was not affected and the fall will not be cleaned up. The fall measured 180' X 18' X 12' up to 15'.

Employee was building brattice using block and as he was lifting a block, he felt pain in his left side.

Employee was loading a piece of belt structure into a scoop, when the bucket was lowered on his right foot.

Employee stated that while attempting to plug a shuttle car cathead into receptacle, that it slipped, causing him to strike his right ring finger against the frame of the power center. He received a laceration that required stitches.

Employee stated that he finished cleaning the area and reached in scoop to get a bag of rock dust when a rock, measuring 12" X 18" X up to 2", fell from top striking his left hand. He was initially released to regular duties but after a follow up was taken off duty. He began losing time 8-12-09.

Employee stated that while he was lifting miner cable to hang he felt a pain in his groin area. He was released to regular duty but had surgery and began to lose time on 9-01-09.

Employee was pushing cable up in the hole when he felt pain in his left shoulder. Continued to work regular job but had surgery on 01/07/2010.

Employee stated that he was removing drill steel from a hole that had been drilled. A piece of finishing steel got stuck and he was trying to remove it, when it came loose and struck him in the mouth.

Employee was pushing up the 2nd bolt, when a piece of rock 6" x 4" fell and struck him on the left arm, causing a laceration that required sutures.

While attempting to place a roof bolt in drill head, employee inadvertently lowered the drill head onto his left foot.

Notified by Worker's Comp Board that they have ruled that the employee has been diagnosed with carpal tunnel syndome and is scheduled for surgery on 4/1/09.

While setting a screw type jack, employee made a mis-hit with his hammer and hit his finger against the jack and the hammer. Resulted in a fracture on the tip of his finger on left hand, resulting in no lost work days.

Employee was helping load reducer with a hoist when reducer fouled on rear of mantrip. When he tried to free the reducer, the hoisting strap slipped causing it to move suddenly and catch his right ring finger against the reducer and mantrip. Resulted in a fracture and required stitches.

While helping miner operator move miner cable, rib rolled off striking him on his right side.

Unit was on pillars, had a fall that caught 2 mobile roof support units.

While retreat mining on 002 section, mining last left lift, the roof fell and trapped mobile roof support at approx. 1:45 a.m. The rock was removed by a scoop and the mobile roof support was moved from the rock.

A piece of draw rock fell striking his hand while it was on top of the drill canopy casuing a cut requiring sutures.

While loading a trickle duster, employee bent over and picked up a bag of rock dust and felt pain in the right side of his lower back.

At approximatley 4:00 p.m. while mining the #6 entry a cavity in the coal seam was encountered that contained a pocket of methane gas. As the continuous miner was backing out of the face a glow was seen at the ripper head. The gas ignited; there was no explosive force associated. The report was not filed on time due to an oversight. A citation (6641282) has been issued.

Employee sprained his right knee in the #4 heading while marking centers, when he stepped on a rock. He continued to work until the doctor took him off on 10/23/08 for arthroscopic knee surgery. He started to lose time as of that date.

Employee was installing a piece of top belt structure and he experienced pain in his upper right arm. Initially first aid notice only accident, but condition did not improve. On 11/20/2008 surgery was performed to repair torn muscle. He began losing time as of the date.

Backing out into the haulway with his hand resting next to the motor lid, when he hit a hole in the roadway causing the lid to raise up and down catching his right thumb causing a laceration requiring stitches.

Cutting conduit with knife when knife slipped cutting finger requiring stitches.

Putting battery powered mantrip on charge, when it rolled into him, catching him between the mantrip and the power center.

Removing drill steel from hole. Placed wrench on steel to spin it to assist in removing from hole. Glove was caught between steel and wrench, when rotation started, left hand was twisted. Stress fracture to small bone in hand.

Employee was assisting another employee with a car cable that was not taking up; when he discovered the problem, the operator started his shuttle car causing the cable to start taking up. The cable caught the injured's legs causing him to experience a same level fall resulting in a fracture to his left wrist.

The employee was attempting to cut open a bag of rock dust that had fallen into the rock duster when his hand slipped resulting in a laceration to his thumb on his left hand. Required stitches to close the laceration.

A piece of rock fell from the coal rib striking the employee on his lower right leg and ankle rrsulting in an injury

EE allegges that he was lifting the miner cable to be hung from the roof, EE experienced a popping in his right shoulder and experienced a sharp pain. He continued to work until 2/26/08 at which time the Dr took him off work and diagonised EE with a rotator cuff tear. Surgery is scheduled for 3/5/08.

As the injured employee was swinging out the drill head to install a roof bolt, a piece of drawrock slid down the drill canopy. The rock struck the employee on his lower right leg resulting in a non-displaced fracture to his right distal fibula.

The injured employee was checking the dump valve for the dust box on the left side roof drill. When the stab jack was inadvertently raised, the flapper on the dust box closed on his left thumb resulting in a laceration and fracture to his thumb.

A roof fall was discovered by the fireboss at approx 1:15 pm. The fall is located in the left return off Lick Fork Mains #3 approx 3,700 feet outby the #002 MMU near SS #8571 at the mouth of the Panel 12 Left. The fall measured approx 20' long x 26' wide x 7-9' thick. No equipment, no injury occurred and no ventilation interruption occurred as a result of the fall.

The injured EE failed to maintain control of Mack 800 tandem truck he was driving, causing him to collide with another truck. This accident did not result in a serious accident, as he was released from a local ER with minor contusions and laceration.

The employee alleges that he was repairing a belt crossover as he was taking off a board that he felt a pain in his left groin area. Surgery was completed 10-12-07 to correct the hernia.

The employee alleges that as he was building a ventilation brattice he picked up a cap wedge and raised it over his head to wedge the brattice in place. He alleges that he felt a pop in his right shoulder as he lifted the wedge over his head.

A roof fall occured inby the modible roof supports with a partial fall on #3 MRS. Fall measured 18 feet x 1-6 feet thick with a unknown length. The area was developed 7/20/07.

The employee laid over 5-6 feet of miner cable when he fell to the ground complaining of severe lower back pain. Employee started missing work 9-25-2007.

The injured employee stepped up to the miner to check the pressures on the mobile roof supports when an undetectable rib and brow fell striking the employee on his back and his legs riding him to the mine floor. The rock brow measured 7.5' long x 14" wide x 0-16" thick.

The employee alleges that he was pulling a piece of curtain from under some coal, when he experienced pain in his right shoulder. Employee started missing work 9/10/07.

The employee states that he was hauling supplies to the roof drills. He was carrying the rope bolts on top of the scoop and was passing through a set of fly pads when one of the rope bolts hooked on the fly pads and sprung back striking him on his right forearm causing a laceration that required stitches.

The roof fall was discovered by EE on 8-13-07 at 9:40 AM as he was firebossing area. Fall was in the intersection & measured 40' long x 20' wide x 0-12' thick. Area was developed 12-2002. No injury resulted, no ventilation interruption occurred, no equipment involved. Fall will not be cleaned up. Fall occurred on Southwest Mains bleeder panel in #3 heading @ survey station #10.

The employee alleges that as he was cleaning a piece of rock off the drill canopy another piece of draw rock fell from the roof striking him on top of his left hand. The piece of rock caused a small laceration that required three stitches to close the wound.

Employee was helping hang a roof bolter trailing cable when he felt pain in his back. He continued to work until June 20, 2008 when he began missing time.

The inj EE was loading metal plates onto forklift when the metal plates shifted catching right thumb between 2 metal plates resulting in a laceration & fracture to right thumb.

Inj EE was struck on hard hat by draw rock that slid down his back onto his left lower leg resulting in fractures to 4th & 5th metatarsal bone on left foot.

The miner had cut approximately 17 feet out of the right cut of the block to the right of #4 entry. The operator noticed the warning light going red on the MRS due to the pressure and the miner was removed. The left MRS malfunctioned. After it was removed the right MRS became caught. The roof broke in front of the MRS entrapping it. A hydraulic retriever pulled it from the rock.

The EE alleges that he had an accident 6/8/07, but did not report incident until 7/2/07. Alleges small piece of drawrock fell, striking him on right wrist as he was starting to drill hole for roof bolt. Small piece of rock impaled wrist & was surgically removed 7/19/07 at which time injury became reportable.

The EE was behind the line curtain getting ready to advance the curtain, when a piece of drawrock 30" X 28" X 2.5" thick fell, striking the EE on his left lateral lower leg, resulting in an abrasion and contusions to the leg. The EE continued to work until 5/22/07.

The injured employee felt pain in his lower groin area as he was stacking up a solid block stopping. The employee continued to work until 5/12/07, at which time, he was diagonised with a hernia and will require surgery to repair.

Fall of roof discovered by belt examiner on 3-23-2007 at 2:50 pm. Fall was located at the mouth of bleeder panel Southwest Mains #2 heading in the returns entries. The fall measured approximately 0-10 feet thick x 19 feet wide x 40 feet long. No equipment was involved or the ventilation was not interupted. The area was bolted with 5 foot torque tension bolts.

As the employee was operating the roof drill, a small sharp piece of draw slate (3"x3") fell striking him on the upper lip resulting in a small laceration requiring sutures to close.

The employee alleges that as he was removing a cover from the continuous miner when the cover fell suddenly causing pain to his upper left arm resulting in a sprain/strain. He continued to work until he was taken off work by the phyiscian on 3-9-07.

As the injured employee was exiting the scoop deck, he experienced pain in his right knee. He began missing work on 3-7-2007.

The employee was carrying a crib block to the miner to perform the work needed when he experienced a sharp pain to his right groin area resulting in a strain to his groin.

Employee was in the process of removing the plug from the gearcase when the plug suddenly loosened causing him to strike a miner bit with his mouth resulting in a fracture to his tooth resulting in dental care.

While testing the functions of the MRS, the injured EE had his left hand on the machine when a sudden movement caused a lid that had been raised to fall mashing the #3, #4, #5 fingers on his left hand, resulting in fractures and lacerations to these fingers.

As the employee was carrying a piece of metal around the miner he turned & hit his hand on the cutter head of the miner causing a fracture to his fifth finger on his left hand.

A fall of roof occurred measuring 0-10' high X 12' wide trapping the mobile roof support. The ventilation was not disturbed and the fall will not be cleaned up. The fall occurred on the pillar section 001 MMU #5 pillar block at SS #8425.

The EE was walking to 4 right crosscut when he alleges that he struck a brow that caused pain in his neck. The average seam height is approx 70" and the height to the brow was approx 62".

EE was lifting a piece of metal, when he felt a sting above his right groin area, that resulted in a hernia that required surgery to repair. He was diagnosed on 10/06/06 and continued to work thru 10/07/06.

A rock fall occurred in the first hand cut in the #4 pillar block in #5 heading 100' inby SS #7437. The continuous miner was stuck by a rock that fell on the miner head in the first cut; no roof bolts pulled or fall above anchorage. The cut was approximately 20-25', miner was stuck by 3-5' rock. No ventilation was disturbed, no injury occurred and the fall will not be cleaned up.

A roof fall occurred measuring 6-8' high in the #2 heading #1 pillar block 40' inby SS 7544 last right hand cut in #1 block rock had fallen in cut only when attempting to move MRS out 2 rows of bolts fell out above anchorage. Equipment involved was an MRS, no ventilation was disturbed, no injury occurred and the fall will not be cleaned up.

Employee was cutting metal with oxygen and acetylene torch when a piece of hot slag struck him on his right elbow, causing a small burn. This injury was not reported to his employer until 8-24-06 & ee worked thru 8-25-06 at which time he sought medical attention. He was admitted to a local hospital & diagnosed with an infected bursa sack that required surgery to drain.

The EE alleges that he was in the process of cleaning the #9 belt drive, when he experienced pain in his lower back. The EE continued to work until he was taken off work by the physician on 11/28/06.

Employee was using a utility knife to cut belt, the knife slipped causing a laceration to his right thigh that required stitches.

Employee was loading rock dust onto a scoop when he experienced a hernia of the groin that required surgery.

An ignition occurred at approx. 9:15 am in the #1 block in the #2 heading on the #1 section (002). This was the first left hand cut in block. No equipment damage or personal injury involved. No violations were issued by MSHA or DMME.

Employee lost his footing when he stepped on rocks in the roadway, causing him to over extend right shoulder while trying to stop his fall.

EE was pulling miner cable thru conduit with telephone cable. The telephone cable broke, causing employee to fall backwards landing on miner cable resulting in a fracture to his tailbone.

Employee was walking around the side of the roof bolter, as he turned the corner of drill, he twisted his right knee, causing pain. He continued to work until 8/14/06 at which time surgery was performed on his knee.
