EE WAS GETTING INTO CAB OF EXCAVATOR, UTILIZING THREE POINT CONTACT WHEN HIS LEFT FOOT SLIPPED CAUSING HIM TO FALL TO THE GROUND.

EMPLOYEE WAS DIGGING ROCK OUT OF SHOT. HOOKED BUCKET TEETH ON BIG ROCK AND TRIED TO LIFT ROCK. THE REAR OF LOADER RAISED UP OFF THE GROUND; AT THAT POINT, THE BUCKET SLIPPED OFF OF THE ROCK AND THE REAR OF THE LOADER DROPPED TO THE GROUND.
