The driller was changing a drill steel when he noticed the steel was cross threaded on the power head. He pushed on the steel with his rt hand to correct the problem & the steel suddenly disconnected from power head & dropped onto the table, crushing the tips of his 2nd & 3rd fingers on his lt hand which he had rested on the table. The 1st joint of his ring finger was amputated.

Associate was pulling bars out of feeder area of jaw crusher. Rock from above fell, hit bar, throwing bar against side of jaw, mashing the associate's left thumb between bar and side of jaw.
