Employee has filed a hearing loss claim dated February 5, 2013, with the Virginia Workers' Compensation Commission. As per the audiologist's report, employee suffers from "moderate sloping to severe sensorineuria loss." Employee's last date of service with company was 10/21/2012. **THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM.**

The individual was tightening the steering arm bolts with a wrench when he felt pain in his left arm and shoulder. This is a medical only reportable accident as the indiviual performed his normal duties and did not have any lost or restricted work days.

The individual had been grinding in the shop area. When he finished he rubbed his eyes with his hands and got a sliver of metal in his right eye. He was wearing prescription safety glasses while he was grinding. The individual received prescription medication for his eye.

The employee alleges that he was hanging a miner cable and had it on his shoulder. The cable pulled him to the ground, resulting in a possible bruise to his hip. The alleged injury is still under investigation and we reserve the right to withdraw the report upon completion of the investigation.

Employee got truck out of gear on hill to slow truck down. He steered truck into drainage ditch; impact from going into ditch caused water tank to become loose and come into cab on driver.

Excessive rain caused the highwall above the belt portal entry to erode. The additional weight of the wet material on the canopy caused the portal canopy to partially collapse onto the belt conveyor. Ventilation was not affected ant there were no injuries or equipment involved. A new canopy has been installed.

A roof fall was discovered on the No 1 south panel, which was being pillared. The fall was discovered in the No 5 entry, just inby survey station 3219. The fall was an extension of a pillar fall which extended outby the break-line about 60'. The fall was 20' wide X approx 5' high. There were no injuries or equipment involved.

The employee was working on the section conveyor belt tailpiece when his wrench slipped. When the wrench slipped it caused him to strike his elbow against the tailpiece. He worked his regular job until 2-26-08. He did not miss work or receive medical treatment until 2-26-08.

The EE was installing a roof bolt, when a piece of rock fell from the mine roof and caught his finger against the drill head. This resulted in a laceration which required sutures.

The EE was welding a piece of equipment and was blowing slag from the weld with an air nozzle, when a foreign object entered his right eye. The small partical imbedded and was removed by a physician.

The EE was installing roof bolts. He installed the rib bolt and as he repositioned the drill head to install the second bolt, a piece of draw rock fell and struck him on the shoulder.
