Employee was traveling inby in the No. 4 Belt Entry on a Mac 2 vehicle when he hit a hole causing the Mac 2 to bottom out. Employee felt sharp pain in his lower back when this occurred.
Roof Fall was discovered in the #1 entry (Return) of #9 belt A West 50' outby SS #6297. Fall measured app. 20' wide by 32' long by 11' high.
Possible occupational illness due to exposure to respiratory dust.
On 8/29/2011 a roof fall occurred at approx. 11:18 am. at approx. 100 ft. outby spad #6254 in #2 entry on the A-West Panel. the fall measured 20 ft. by 25 ft. by 7 ft. high.
Discovered roof fall at approx. 4:05p.m. on the A West Panel, 200' inby survey station 6254 in the #2 neutral entry. The Roof Fall was at or above anchorage zone of the roof bolts installed in that area and the fall did not impair ventilation nor impede passage or travel of personnel. The fall meas. 30'L. X 10'W. X 8'H. Currently there are no plans of cleaning up the fall.
Individual stated he was riding on top right rear fender of the mantrip. When the mantrip went around a curve, he slid and his right foot was caught between mantrip and rib.
The individual was spinning roof bolt and a piece of foreign matter struck him in the right eye. The individual was wearing safety glasses. The individual had to have the foreign matter removed by a physician. This is a medical treatment with no lost or restricted workdays.
The ee was riding with the owl shift crew from the 001 section to the surface. Along the #9 travel way, he failed to adequately lower himself for clearance and was struck in the right forehead by a cross beam installed across the travel way. The contact caused a laceration which required stitches to close.
The individual was standing in the center of the bolter as the ATRS was coming down. His foot was caught in the scissors part of the ATRS causing fracture to the toe area of the left foot.
A roof fall occurred on the #13 belt line in the #4 entry approx. 2000' outby the face. The fall occurred in the intersection at SS6526 and measured 40'x30'x7'. The area was initially bolted with 48" x 7/8" resin grouted rods and 6' cable bolts C2 between rows.
The individual was checking the cable reel on the MRS. The lid for the cable reel fell on left hand.
This fall was found at 12:15pm on 10/7/09. The fall was 40'x30'x7' 90' outby SS 6591 in B northeast in the #4 enty. The area had been initially supported by 48" cable bolts and supplemented with 8' cable bolts. The fall will be cleaned up.
The individual was taking the miner cable down when a rope hanger flipped and struck him in the right eye.
The employee was loading belt structure into the scoop bucket and strained his lower back. He is working restricted duty because of the injury. The injury was not reported until 5-7-09.
A fall was discovered in the 3 Left off 1 North marker area of the mine approximately 50' inby survey station No. 6262 (No. 6 entry). The fall apprears to be approximately 30' long by 8'+ high by 20' wide. There were no injuries or equipment involved. Ventilation was not affected. The fall will not be cleaned up.
The employee was getting in the deck of the shuttle car when a piece of draw fell from the mine roof and struck him on his right shoulder.
The employee was using a set of torches to remove wear strips from a continuous mining machine. He was positioned under the machine and a piece of slag fell in his ear. On January 5, 2009 a physician determined that medical treatment was necessary to treat infection of the ear.
A fall was discovered in the no. 1 entry of 3 west marker outby survey station no. 5388. The fall was about 6,300 feet outby the MMU 003-0 section and measured approximately 30' long x 84" thick x 18' wide. There were no injuries or equipment involved. Ventilation was not interrupted. The fall will not be cleaned up.
The employee loading wooden crib blocks into a supply trailer when he slipped and twisted his right knee.
The employee was repairing a broken shuttle car conveyor chain. He was using a shop hammer to dislodge the chain and struck his left thumb with the hammer.
The injured was installing roof bolts when a small piece of rock rolled from the rib and struck his left rib. This resulted in a bruise to his knee.
A fall was discovered in a crosscut adjacent to the No. 1 entry of No. 3 west marker about 90 feet inby survey station No. 5387. The fall was about 2,000 feet outby the MMU 003-0 and measured approximately 15' long x 8' wide x 6' high. The fall will not be cleaned up. There were no injuries or equipment involved.
Cable for roof bolter machine was hung against the mine roof with a rope hammer hooked into a roof bolt plate. EE was taking the cable loose when the hook , on the end of the rope hammer, struck EE on the nose, laceration and 1 stitch.
A roof fall was discovered in the No 1 entry of the No 3 west marker section about 4000' outby the developing MMU 003-0. The fall measured approximately 30' long x 15' wide x 6' high and was located in the intersection 1 break outby survey station No 6169. There were no injuries or equipment involved.
The employee was removing a step-down transformer offt the top of a distribution box. the transformer slid off the corner of the the distribution box & struck the injured's left leg. This resulted in a fracture of the small bone in his lower left leg. NOTE: The DB Box & transformer were stored and were not in service
The injured was preparing to cut a piece of metal using a band saw. He lost his balance and his gloved band contacted the moving band gladp. The injured suffered a laceration to his right index finger which requirely sutures.
The individual was preparing to install the 1st row of roof bolts in 2 left face of unit 3. He was visually examining the mine roof as he set the floor jack on the roof bolting machine. The floor jack contacted his right little toe, causing a fracture and laceration. He was positioned on his knees at the time of the injury.
The individual was shoveling spillage from under a conveyor belt and strained his lower back.
Fall was discovered in the No 2 entry of 3 west marker at survey station No 5395. The fall measured approx 18' X 20' X 5' and will be cleaned up. Ventilation and passage of men was not affected. There were no injuries.
The ee was dropping cables which were hung from the mine roof, during a power move upon removing one hook of a double-hooked rope cable hanger, the handler spun around and the loose hook struck him in the lower lip causing a laceration. the laceration required sutures.
The belt drive take-up was fouled on a shieve. The injured was attempting to free the rope by striking a hammer held against the rope with another hammer. A metal fragment hit his right eye above his cheek and below his lower eyelash.
The employee was replacing a coupling on the feeder. He was tightening a bolt with a socket & ratchet when the ratchet slipped off the bolt head causing his right index finger to strike the bed plate resulting in a fracture to the end of his finger.
The injured was operating a continuous mining machine in the #2 entry of the rooms mined left off (about 133' inby SS #5867) northwest mains. The rib rolled and a piece of the rib consisting of coal and rock (approx 4" thick X 9" wide X 8' long) struck him, resulting in fractured ribs and a fractured pelvis.
A roof fall was discovered between the No 4 and NBo 5 Entries of No 1 North marker at survey station No 5476. The fall was 2900 feet 0utby the MMU 004-0 and measured approximately 5' High x 18' Wide by 85' Long. There were no injuries or equipment damage involved. Ventilation and passage of men were not affected.
Roof fall was discovered in the No.1 north marker set of entries just outby survey station no 5687 in the no.1 entry. The fall measured approx 18'w x 10'h x 30' + long. The fall is 1200 outby the mmu004-0. There was no equipment involved. There were no injuries. Fall did not affect ventilation or the passage of men.
Pillars were being mined on the MMU 001-0 section. As the MRS supports were being pulled up from the heading, a piece of rock fell (12'X15'X3') outby the MRS. The fall was in the #7 heading about 105' to the right of survey station #4987. There were no injuried involved.
The injured person & another Ee were making a splice in a pump cable. Upon completing the splice he attempted to stand up and felt a pop and experienced pain in his right knee.
The individual has been workng the past couple of weeks constructing mine ventilation seals. As a result of the repeated exposure to the concrete being used in the Seal construction process, he was diagnosed with cellulitis.
A fall was discovered in the no. 5 intake entry of the 1 North marker set of entries. The fall measures approx. 35'long x 20'wide x 2'- 4'thick. There was no equipment involved, there were no injuries & ventilation was not affected. The fall will not be cleaned up.
The injured was assisting in removing of the final drive cover on the continuous mining machine. The cover came loose suddenly and caught his ring and index finger between the cover and the rub rail of the machine.
The ee and another ee were in the process of changing a drive chain on a belt drive. While moving the chain in place, the weight of the chain broke the weld holding the oil bath in place causing the oil bath to fall and strike the injured on the left little toe.
A pillar fall occurred after taking the final lift from the No.8 block on the 3-right off No.1 Northwest panel (3 breaks inby survey station No. 5274). The fall disable the #4 mrs location in the #9 entry & caused the #2 mrs to go to solid preventing it from being moored.The mrf were recovered.
The injured was working in the shop using a chop saw and cut his right index finger. The laceration required sutures.
The injured and another EE were setting 100-ton roof support jack. The injured struck his head against a cable bolt head, which was protruding from the mine roof, causing a strain type injury to his neck.
The 3 left marker section mined into the old 1 left area of the oad R&B mining corporations mine No.1 The cut thru was made in accordance with an approved plan. Once the cut-thru was made it was discovered that there was a fall in the old R&B mining corporation mine. The extent of the fall has not been detrtmined.
The injured was in the process of installing conduit on the ripper motor leaks of a continuous mining machine. As he turned to walk, he twisted his right knee.
The injured employee along with another person were returning oxygen & acetylene tanks and cutting torches to the storage area. The injured unloaded the oxygen tank and when he bent over to unload the acetylene tank, he strained his back.
EE was operating a Cont. Miner when he saw a coal rib working beside the machine. He turned to hurry and leave area and tripped on the cable of mining machine which caused his arm to strike shuttle car he was loading.This resulted in a laceration (Avulsion Type) to his arm.
The employee was carrying a belt clamp toward the belt. Because of the mining height, he had to walk in a bent-over position and felt pain in his lower back.
THE INJURED WAS IN PROCESS OF MAKING A BELT SPLICE. WHILE CUTTING THE BELT WITH A UTILITY KNIFE, THE BLADE OF THE KNIFE BROKE CAUSING HIM TO CUT HIS FINGER.
The injured was working with another employee, pulling the power cable to the section feeder, during a power move and strained his right knee.
THE INJURED WAS INSTALLING BELT STRUCTURE DURING A BELT MOVE. HE AND ANOTHER EMPLOYEE WERE LIFTING A PIECE OF BELT STRUCTURE WHEN THE INJURED FELT PAIN IN HIS LOWER BACK.
THE INJURED WAS SPREADING ROCKDUST BY HAND. HE WAS CARRYING 1/2 LB. OF ROCKDUST DOWN A SLICK GRADE WHEN HE SLIPPED AND FELL. THIS RESULTED IN A FRACTURED RIGHT ANKLE.
THE INJURED WAS RIDING A RUBBER TIRED 4-WHEELER ALONG THE NO. 5 BELT AND STRUCK A BOTTOM ROLLER HANGER INJURING HIS LEFT UPPER CHEEK NEAR HIS EYE. THE LACERTAION REQUIRED SUTURES.
The employee was using a pry bar to position a water valve. The pry bar slipped and struck him on his right cheek. This resulted in a laceration which required sutures. AS A RESULT OF THIS INJURY, THE INJURED EMPLOYEE STRTED MISSING WORK ON 8/31/05.
The injured was tramming a fork lift on the haul road between the marker portal and the mine office. He ran over a rough place in the road causing him to bounce and strike his head on the canopy of the forklift. The mri report received 8-22-05 confirmed injury.
THE EE WAS CARRYING A CHAIN & FELT A SHARP PAIN IN HIS LOWER ABDOMEN & GROIN
The injured was rolling a cutter drum, for a continous mining machine with a floor jack. The top of the jack kiced out of place and struck the individual on the left elbow resulting in a bruise.
The injured employee was moving the miner cable when a piece of draw rock fell striking him on the right foot causing bruising and a fracture to the big toe.
THE INJURED WAS MOVING A PIECE OF CURTAIN FROM THE SHUTTLE CAR ROADWAY AND WHEN HE TURNED TO WALK BACK TO THE SHUTTLE CAR, HE CAUGHT HIS NOSE ON A CABLE HANGER RESULTING IN A LACERATION WHICH REQUIRED SUTURES
The injured employee was outby the mmu 001-0 loading point, aligning the no.6 belt. The mine height in this area is approx. 8' and the injured stated that a small piece of draw rock fell striking him on the head. The injured did not start missing work until 6/06/05.
The employee was walking toward the roof bolting machine and stepped on a loose rock resulting in a sprained left ankle and knee.
The injured was securing a rock duster in the bucket of the scoop. He was securing the duster with 1 of 2 chain binders when the binder twisted, slipped out of his hand and the handle of the binder struck him on the right side. He continued to work his regular job. However, an MRI on 6-17-05 indicated a fractured rib. He is now missing work.
The injured was installing a roof bolt when a piece of draw rock fell and struck him on the right foot. This resulted in a fracture to his toe.
A FALL WAS DISCOVERED IN THE #3 ENTRY (1 BREAK INBY SURVEY STATION 4517) IN THE #1 SOUTHWEST MARKER SET OF ENTRIES, THE FALL MEASURED 140' LONG X 18' WIDE X 10' HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. THE FALL WILL NOT BE CLEANED UP
A ROOF FALL WAS DISCOVERED IN #2 ENTRY (1 BREAK INBY SURVEY STATION #4481) ABOUT 1200' OUTBY THE MMU003-0. THE FALL WAS IN AN INTERSECTION AND MEASURED 30' LONG X 20' WIDE X 10' HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED, VENTILATION WAS NOT AFFECTED. THE FALL WILL NOT BE CLEANED UP. THE AREA WAS BOLTED WITH 5' RESIN GROUTED BOLTS, SUPPLEMENTED WITH 8' BOLTS
THIS REPORT IS HEREBY SUBMITTED UNDER PROTEST AND ONLY FOR THE PURPOSE OF TERMINATING CITATION #7343331. THE ALLEGED INJURY OCCURED DURING A BELT MOVE. ANOTHER PERSON WAS THROWING TOP BELT STRUTS UP BETWEEN TOP + BOTTOM BELTS. EE THREW A PIECE TO HARD AND IT STRUCK INJURED ON THE RT UPPER LEG CAUSING A BRUISE AND ALLEGED BACK INJURY
Injured was helping remove a ripper motor from a cutter boom of the cont mining machine. As the motor moved, a part on the motor struck the ee's watch band and pushed the band into the top of his wrist causing a laceration.
A piece of rock had the cable to the roof bolting machine fouled under the deck of the machine. While trying to break the rock, the injured was holding a hammer against the rock while another person was using a hammer to strike a blow. A piece of metal flew off the hammer and imbedded into the injured's right leg.
EE WAS DOWN-STACKING A PALLET OF 6" CINDER BLOCKS IN PREPARATION FOR DELIVERY UNDERGROUND WHEN HE FELT A PAIN IN HIS RIGHT HIP AND DOWN HIS RIGHT LEG.
The injured employee was hanging the cable for the continuous mining machine and experienced a pain in his groin. He did not go to a physician until November when it was determined he had a hernia. He started missing work on 11-18-04 when he had surgery to repair the hernia.
METAL DOOR OVER TOP CABLE REEL WAS OPEN WHEN EE ATTEMPTED TO CLOSE DOOR, IT DROPPED ON HIS FINGER, CUTTING HIS FINGER WHICH REQUIRED STITCHES
EE WAS DRIVING A BIT INSERT INTO A BIT LINE ON THE RIPPER HEAD WITH A SLEDGE HAMMER. THE EE STRUCK GLANCING BLOW AND THE HAMMER STRUCK HIM ON TIP OF HIS LEFT FOOT.
DURING THE WEEKLY EXAMINATION A FALL WAS DISCOVERED IN THE NO. 5 ENTRY OF THE 4-WEST BLEEDER, APPROX. 5000 FT AWAY FROM THE ACTIVE SECTION 2 BRAKS OUTBY SURVEY STATION 608. THE FALL MEASURED 8 0' LONG X 18' WIDE X 6' HIGH. THERE WERE NO INJURIED OR EQUIPMENT INVOLVED AND VENTILATION WAS NOT EFFECTED. THE FALL WILL NOT BE CLEANED UP. NOTE: THIS AREA WAS MINED INT HE 1970'S.
EE WAS HAULING TWO BUNDLES OF BELT RAILS INTO THE MINE. THE TOP BUNDLE 50 RAILS SLID TO THE SIDE. THE EE ATTEMPTED TO PUSH THE BUNDLE BACK ON TOP AND STRAINED THE MUSCLES IN HIS RIB CAGE, RESU LTING IN A DETACHED MUSCLE FROM HIS RIBS.
THE CONTINUOUS MINER HAD BEEN BACKED OUT OF THE CUT BEING TAKEN BECAUSE A MACHINE GUARD HAD FALLEN OFF THE MACHINE WHILE IN THE CUT. THE EE & MINER OPERATOR WERE OBSERVING THE LOCATION OF THE GUARD WHEN A LARGE ROCK RIB FELL FROM BEHIND THELINE CURTAIN STRIKING THE EE ON THE RIGHT SHOULDER AND HIP.
RIB ROLLED OFF AND STRUCK ROOF BOLTER ON THE RIGHT FOOT AND LEG FORCING HIS LEFT FOOT UNDER BOLTING MACHINE. EE HAD WORKED HIS REGULAR DUTIES FROM 9/12 - 9/17. HE BEGAN LIGHT DUTY ON 9/18/03.
THE ROOF BOLTING CREW WAS BOLTING THE FIRST CUT IN THE RIGHT CROSSCUT OFF THE BELT ENTRY. THE INJURED EMPLOYEE WAS OPERATING THE RIGHT SIDE OF THE MACHINE. IN POSITIONING THE DRILL HEAD TO INS TALL A RIB BOLT THE CANOPY CONTRACTED AND DISLODGED COAL AND ROCK STRUCK EMPLOYEE'S LEFT CALF AND BACK OF KNEE.
EE WAS GETTING OUT OF THE DECK OF SCOOP BUT HIS FOOT DOWN SLIPPED ON THE WET BOTTOM TWISTING HISRIGHT FOOT.
EMPLOYEE WAS OBSERVING THE OPERATION OF THE CONTINUOUS MINER IN THE NO. 5 ENTRY WHEN A ROCK FELL FROM BETWEEN ROOF BOLTS STRIKING HIS HEAD & LEFT SHOULDER INJURING NECK, BACK & RIGHT LEG. HE D IDN'T START MISSING WORK UNTIL 8/22/03.
EE WAS INSTALLING ROOF BOLTS. 2 ROLLS OF BOLTS HAD BEEN PUT UP, AND WHILE WAITING FOR DRILL TO BE MOVED, HE WAS GETTING MORE BOLTS WHEN A ROCK CAME OFF RIB HITTING EE ON LOWER LEFT LEG AND ANK LE.
EMPLOYEE WAS MOVING A SMALL PIECE OF ROCK BY HAND FROM IN FRONT OF A MOBILE ROOF SUPPORT AND INADVERTENTLY ALLOWED HIS LEFT HAND TO GET CAUGHT BETWEEN THE ROCK AND THE FRAME OF THE MRS. *EMPLO YEE DIDN'T START MISSING WORK UNTIL 7/28/03.
EMPLOYEE RAN OVER A ROCK IN S & S SCOOP (482), IT THREW HIM INTO THE CANOPY.
THE DRILL HEAD HAD STOPPED UP DUE TO A SMALL ROCK BEING IN THE HEAD. EMPLOYEE WAS REMOVING THE ROCK FROM THE HEAD AND HAD LEFT THE DRILL STEEL IN THE HOLE. THE DRILL STEEL DROPPED OUT OF THE H OLE HITTING HIS LEFT THUMB.
EMPLOYEE PARKED HIS SHUTTLE CAR AND WALKED TO THE MINER TO HELP SET BITS. UPON ARRIVING AT THE MINER, HE SLIPPED ON SLICK BOTTOM AND SPRAINED HIS LEFT KNEE. EMPLOYEE DID NOT START MISSING WORK UNTIL 5/5/03.
EE WAS IN THE PROCESS OF TAKING THE MINER CABLE DOWN. THE CABLE WAS HUNG APPROX 7 FT HIGH. HE AND OTHER EE WERE DROPPING THE CABLE WHEN INJURED FELT A SHARP STINGING BURNING TYPE PAIN IN THE R IGHT GROIN AREA. EE DID NOT START MISSING WORK UNTIL 4/9/03.
EE WAS COUPLING DRILL STEEL TOGETHER AND CAUGHT THUMB BETWEEN JOINTS OF STEEL, CLIPPING THE END OF HIS LEFT THUMB.
EE WAS BOLTING TOP RIB CAME LOOSE ABOUT 8 FT LONG, 4 FT HIGH, SLIPPED OFF KICKING A PIECE OF COAL AND ROCK LOOSE 57 IN LONG, 12 INCH THICK, PUSHING DRILL OPERATORS RIGHT LEG UNDER DRILL HEAD.
A ROOF FALL WAS DISCOVERED IN THE NO.A NORTH BLEEDER ENTRIES ADJACENT TO SURVEY STATION NO. 715.THE FALL IS IN THE CROSSCUT BETWEEN THE NO.5 ANDNO.6 ENTRIES & MEASURES APPROX. 40'L X 20'W X 5' HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED.
A ROOF FALL WAS DISCOVERED IN THE NO A NORTH BLEEDER ENTRIES ADJACENT TO SURVEY STATION NO 715. THE FALL WAS IN CROSSCUT BETWEEN THE NO. 5 AND NO. 6 ENTRIES AND MEASURES APPROX 40 FT LONG BY 2 0 FT WIDE BY 5 FT HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. VENTILATION NOT AFFECTED.
EMPLOYEE PULLED UP ON PERMISSABLE RIDE GOING TO TAKE POWER OUT WHEN ROCK FELL BETWEEN BOLTS WITHOUT WARNING, HITTING RIGHT AND LEFT THIGH OF EMPLOYEE.
EE WAS IN THE PROCESS OF TERMINATING A NEW HIGH VOLTAGE CABLE. THE END WAS TIED UP AND HE WAS CUTTING THE "ROPE" STRANDS FROM THE SKINNED OUT CABLE. AS HE CUT THE STRANDS, HE STRUCK THE BACK O F HIS LEFT HAND WITH THE UTILITY KNIFE APPROX 3 INCHES LONG. THIS REQUIRED SUTURES.
EMPLOYEE WAS BACKING DRILL UP SPOT BOLT IN LAMINATED SAND ROCK AND WAS IN THE PROCESS OF POSITIONING THE DRILL FOR SPOTTING THE BOLT WHEN THE ROCK FELL AND STRUCK EE ON THE HEAD AND SLID DOWN AND CAUGHT HIS RIGHT INDEX FINGER AGAINST THE TRAY OF THE OFF SIDE DRILL HEAD.

EE WAS OPERTING MOBILE ROOF SUPPORT WHEN DRAW ROCK FELL FROM BETWEEN BOLTS AND STRUCK EE ON THE CALF OF OF HIS LEFT HIND LEG. EE DID NOT GO TOTHE DR. UNTIL 10/26/02 AND DID NOT START MISSING OCT.28.

EMPLOYEE WAS ROLLING A PIECE OF VENTILATION CURTAIN BESIDE THE BUCKET OF THE SCOOP. HE SAID HE HEARD THE RIB BEHIND HIM BREAK AND ATTEMPTED TO JUMP CLEAR, AS HE DID THE RIB ROLLED AND CAUGHT THE CURTAIN, HE STILL HAD A HEAD OF THE CURTAIN WHEN HE JUMPED, THE RIB CAUGHT THE CURTAIN A NAIL WAS IN THE CURTAIN, IT WAS DRAWN INTO AND TORE SKIN ON LEFT INDEX FINGER.
EMPLOYEE WAS AT #3 DRIVE DUMPING POINT CLEARING UP SPILL AT THE CHUTE CAUSED BY A PIECE OF DRAW ROCK THAT LODGED IN THE DUMP. HE USED A PIECE OF ANGLE IRON FOR A PRY BAR TO DISLODGE THE ROCK. AS THE MATERIAL CLEARED, THE BELT RESTARTED AND ANOTHER PIECE OF ROCK HIT THE END OF THE ANGLE IRON CAUSING IT TO CATCH HIM UNDER THE CHIN.
EE WAS REMOVING OLD BRATTICE AT #85 CC ON INTAKEAND A PIECE OF DRAW ROCK FELL STRIKING EE ON TOPOF HIS LT. HAND.
EMPLOYEE ALLEGED THAT HE WAS STANDING ON A PILE OF ROCK WHILE HANGING A CURTAIN. HE FELL TO MINE FLOOR.
EMPLOYEE WAS MOVING MINER CABLE FROM ROADWAY WHEN HE EXPERIENCED PAIN. EMPLOYEE DID NOT STARTMISSING WORK UNTIL 11/13/01.

EE WAS HELPING MOVE MINER WHEN IT SEE-SAWED ON UNEVEN BOTTOM. HE CAUGHT HIS RIGHT ARM BETWEEN ROOF AND MINER. HE STARTED MISSING WORK ON FEB. 2 FOR SURGERY.
