Discovered roof fall at approx. 4:05p.m. on the A West Panel, 200' inby survey station 6254 in the #2 neutral entry. The Roof Fall was at or above anchorage zone of the roof bolts installed in that area and the fall did not impair ventilation nor impede passage or travel of personnel. The fall meas. 30'L. X 10'W. X 8'H. Currently there are no plans of cleaning up the fall.

The ee was riding with the owl shift crew from the 001 section to the surface. Along the #9 travel way, he failed to adequately lower himself for clearance and was struck in the right forehead by a cross beam installed across the travel way. The contact caused a laceration which required stitches to close.

The employee was using a set of torches to remove wear strips from a continuous mining machine. He was positioned under the machine and a piece of slag fell in his ear. On January 5, 2009 a physician determined that medical treatment was necessary to treat infection of the ear.

The injured was preparing to cut a piece of metal using a band saw. He lost his balance and his gloved band contacted the moving band gladp. The injured suffered a laceration to his right index finger which requirely sutures.

The individual was preparing to install the 1st row of roof bolts in 2 left face of unit 3. He was visually examining the mine roof as he set the floor jack on the roof bolting machine. The floor jack contacted his right little toe, causing a fracture and laceration. He was positioned on his knees at the time of the injury.

The injured was operating a continuous mining machine in the #2 entry of the rooms mined left off (about 133' inby SS #5867) northwest mains. The rib rolled and a piece of the rib consisting of coal and rock (approx 4" thick X 9" wide X 8' long) struck him, resulting in fractured ribs and a fractured pelvis.

The fall was 2900 feet 0utby the MMU 004-0 and measured approximately 5' High x 18' Wide by 85' Long. There were no injuries or equipment damage involved. Ventilation and passage of men were not affected.

The fall is 1200 outby the mmu004-0. There was no equipment involved. There were no injuries. Fall did not affect ventilation or the passage of men.

Pillars were being mined on the MMU 001-0 section. As the MRS supports were being pulled up from the heading, a piece of rock fell (12'X15'X3') outby the MRS. The fall was in the #7 heading about 105' to the right of survey station #4987. There were no injuried involved.

The injured was assisting in removing of the final drive cover on the continuous mining machine. The cover came loose suddenly and caught his ring and index finger between the cover and the rub rail of the machine.
