EE was attempting to mount 966F to inspect Product Link for machine. Used a ladder that was attached to the machine. The sides of the ladder were made of flexible rubber and were worn. Once the technician stepped on the ladder, it broke causing the ee to fall to the ground hard on left foot. Diagnosed with left ankle/foot bone bruise

EE was on ladder against I beam that was attached to structure to be demo. EE was attaching a choker chain from crane when the beam/ladder began to fall. EE grabbed the chain and left little finger was caught between the chain and the beam. EE was approx 4' high, structure and ladder was at ground level. Finger tip has linear fracture, 2 sutures to hold nail in place.

A contracted coal truck mechanic broke a bone in right foot when the truck that the mechanic parked at the new refuse bin rolled back in to the mechanic and knocked the mechanic down. The brakes were checked by both a company inspector and an MSHA inspector with no deficiencies found. The driver apparently did not have the vehicle in "Park".

The R4 Refuse bin collapsed on contractor truck while it was being loaded from above, which prevented the driver from exiting the vehicle for approximately twenty minutes.

The employee was wiping off a glass beaker after having washed it out. Employee cut right hand ring finger on the top of the beaker due to an unnoticed crack in the glass. The cut required stitches.

As employee was exiting a 980G loader he slipped on the bottom step and fell to the ground (approximately 2 feet) and landed on his right shoulder. Employee continued to work and due to complications he started missing work on 11-18- 2009.

Employee got the index finger on the left hand cut when he stuck it in a hole in a coal chute as a co-worker was prying with a 3/4 inch pipe trying to unstop the chute. The pipe caught his finger between the edge of the hole and the pipe. It took 5 suters to close the cut.
