Employee opened the latch and cover of the rotary vane discharge chute for inspection. While using EE's right hand to clean the chute, EE's hand contacted the vane of the rotary feeder. This resulted in an amputation at the level of the distal interphalangeal joint of the 3rd and 4th digits.

Employee was replacing bottom metal screen deck which caused pressure to right wrist. Employee was on stomach between the top and bottom decks. Employee had an injury occur to the same wrist in past. When putting the deck into place employee said ee felt a sharp pain and lost feeling in hand.

On 1/15/16 EE was helping install a return roller. EE crawled under the conveyor and turned to push up on the roller. When EE turned EE felt a sharp pain in EE's lower back. EE reported it to EE's supervisor. EE had no further complaints of pain until 1/29/16 which followed shoveling spillage on 1/28/16. EE was taken to the doctor and diagnosed with a lumbar sprain on 1/29/16.

Employee was shoveling material on 11/3/15 at about 7:15AM, he felt a momentary sharp pain in his lower back as he turned with a full shovel. He reported it to supervisor that afternoon. He worked the remainder of the week. He reported his back got worse over the weekend. He did not report for work and was diagnosed with a lumbosacral strain on 11/10/15 by his doctor.

Employee was moving a piece of steel 1/2" x 6" x 84" in the steel storage area. As he lifted one end and began to move to his left, he tripped over another piece of steel and fell. He had a cut on his left knee that required 3 stitches. The steel was not organized very well and weeds had grown up in the area. He was on restricted duty until 9/13/13.

On Nov. 28 as EE pulled the ball mill chute back he felt a pain in his right shoulder. He said it was minor and he did not need to see a doctor. It was treated as a first aid. He kept management aware that it continued to bother him and he asked to/and saw a Dr. on Dec. 14th. He is now on light duty and has been referred to/and seen by a specialist and an MRI is scheduled.

While using an electric hoist and nylon strap to remove a broken grizzly bar from the jaw crusher, the broken bar spun, momentarily pinching employee's left hand against the side of the feeder. He was taken for medical attention and was on light duty for three days.

While reaching inside the warman pump with his hand to inspect the impellor for wear, EE's arm contacted the edge of the vane and was cut by the sharp worn edge. The cut was on his right forearm about 1 inch above the wrist.

Following a 5.8 earthquake at 1:51PM the dry plant had a lot of dust fall from the rafters & visibility was low. EE was hurrying down a flight of stairs in the excitement & low visibility from the dust, he missed a step & when his foot hit the floor he felt a slight pain in his back. At home his back worsened & he was taken to the doctor & diagnosed with a back sprain.

Miner bent over slightly to pick up a steel sawhorse. While lifting the sawhorse and turning slightly he felt a sharp pain about the middle of his back to the left of his spine.

Miner was climbing a fixed ladder at the jaw crusher. As he was climbing up the ladder, he felt a sharp pain in the upper portion of his right bicep. Medical assessment on 09/22/10 stated right biceps hematoma/defect. Miner was prescribed elbow support, topical NSAID, and limited duty.

EE was using a short handled sledge hammer to beat a dent out of a skid steer bucket. In doing so his left pinkey finger struck a bolt head on the cutting edge of bucket. The EE received 12 stitches and a possible fracture/ break on the same finger.

Employee was trying to splice a conveyor belt back together. A rope is used to pull the two pieces back together. In doing this, the rope broke, snapping back and striking his right elbow, causing an abrasion, and ultimately a painful contusion.

THE EMPLOYEE WAS ATTEMPTING TO TAKE A PUMP CASING OFF FRO REPAIRS. THE EMPLOYEE WAS USING A 3/4" DRIVE RATCHET TO BREAK CASING NUTS LOOSE & WAS PULLING UP ON THE RATCHET WHEN HE FELT PAIN IN HIS LEFT GROIN AREA.
