The accident occurred when employee was attempting to get a hammer disassemble in a set of jaws. They put a spacer piece in to help the jaws grip better on the hammer, and the spacer piece split into pieces. One of the steel chips struck and lodged into employee's arms. The entry site bled but was wrapped up and the employee finished the job they were working on.

The cable on the drill were caught in the tray. EE reached to free the cables when EE felt 3 pops in EE's right bicep.

The injured employee was helping another employee load a wire cloth screen (48" x 54") on the back of the welding truck when left index finger became caught between the screen and the bed of the truck resulting in a laceration.
