Employee was outside under feeder cutting bolt heads off. Employee stated wasn't feeling well and started coughing, stopped cutting and sat on tailgate of truck till started feeling better. Next day came in still coughing sent to ER.
Employee drives an on road dump truck, EE went to get out missed the bottom step and fell to ground. EE bruised EE's hip and back area causing EE to lose time from work
Employee was using a hammer to tighten cheek plates in jaw, when eye that was holding cheek plate broke off, striking employee in lower right arm, causing a bruise.
Employee was taking cheek plates out of jaw using a hammer and wedges, wedge slipped and either wedge or cheek plate mashed his finger
EMPLOYEE WAS USING A METAL BAR AT JAW CRUSHER TO GET ROCK LOOSE WHEN ROCK SLIPPED, HIT BAR PUSHING HAND INTO HAND RAIL AND CAUSING A CUT AND PINCH TO EMPLOYEE'S RIGHT THUMB. HIS THUMB REQUIRED ONE STICH.
EMPLOYEE WAS USING HOOK TO PULL ROCK UP TO PUT WEDGE UNDER TO DISLODGE ROCK IN JAW CRUSHER, WHEN HE FLET PAIN TO HIS LEFT HAND. EMPLOYEE SPRAINED HIS LEFT HAND.
