When EE was swinging the excavator bit from a telehandler to loosen the head nut dust cover, EE realized its path was misaligned. EE attempted to hold the bit back mid-swing. EE was unable to hold it back and the bit slipped off the welded plate causing EE's right ring finger to contact the plate.

While opening the front gate the associate inserted a bolt with washer used to keep the gate from swinging shut. Before the bolt was inserted completely, the gate sprung backwards pinching the associates finger between the gate and the washer on the bolt causing a laceration to one finger that required stitches.

While delivering material I realized that my truck was having issues. I called towing company who came & loaded the truck. Then took the truck to the quarry to unload the material. After unloading the material, I proceeded to step out of the truck only to discover that the wrecker had raised the truck further from the ground causing me to fall & injure myself.

Employee was trying to remove a piece of metal that he had cut from the old jaw structure. He was moving it with a digging bar. He turned at the waist and pulled up on the bar and felt his lower back pull. The metal was 4'x 6' x 3/4".

While reworking a steel screen feed box EE cut a steel plate loose without securing it and it turned over on his arm. The plate cracked the bone in his right arm just above the wrist.
