While installing the bolts that connect the top shell to the bottom shell of the crusher, EE was holding the head of the bolt from the bottom & using 2 fingers to support the center of the bolt. Another associate began to tighten the nut, the bolt spun, grabbing EE's fingers and spinning them across the steel plate deck. The steel deck sliced the middle finger.

While swinging a sledgehammer with his less dominant hand to knock a pin into the bucket of the excavator, the associate felt a sharp pain in his hand.

EE exited the loader facing forward. His tripped over the door jam and fell forward, catching himself with his arms. He did not go to the doctor until 07/12/2012.

Loading a Pre split shot noticed a burning feeling in groin area. (On 9/19 went to general surgeon to discuss possible surgery options, will update 7000-1 when Mr. Jenkins has his recommended surgery.)

2 associates were relining the chutes in the tunnel at Richmond, they needed to get a 200 lb plate into the tunnel feeder at the back. They carried it halfway and sat it down to take a break, when they started to pick it up EE didn't have good enough grip and it slipped out of his hand, falling about 2 inches, pinching his finger between the plate and the concrete.

EE was helping put in mainframe bolt on crusher. Another ee dropped the bolt in place. When the bolt dropped a burr on the bolt caught EE's thumb and lacerated it. He received 11 stitches.

Employee had completed work on machine and was sitting on back of service truck. He reached into the bed of the truck to retrieve a shirt and caught his lower forearm on a splinter of metal on the tailgate of the truck, causing a laceration.

On 4/3/06 it rained that night. On 4/4/06 EE was climbing the ladder of his haul truck. EE slipped on the wet step and fell to the ground. On 4/5/06 EE asked to go to the Doctor.

Ee was holding a wrench on top of a bolt. When the nut was tightened. The Allen Wrench slipped and pinned his finger between the wrench and the side plate.

Miner was using a belt punch to put a hole in a piece of skirtboard rubber. Upon striking the 5-6 blow he struck punch w/ a glancing blow. Punch angled to side of blow punching his left index finger between punch & rubber that he was working on. Miner was wearing gloves, however the force of punching action resulted in tear of skin on his left index finger resulting in 10 stitches.
