While using a hoist to lower 24" pipe wrench from screen tower the pipe got caught on step causing it to swing up and strike the employee above EE's left eye.

Employee reported while driving a haul truck that it hit a pothole and bottomed out causing pain in employee's back. Employee reported the incident to employee's supervisor on 7-20-2016 but did not request to seek medical treatment until 8/4/2016. Employee was diagnosed with a back strain. Employee was able to continue working until 1/27/2017.

EE was assisting to assemble a HP 500 crusher, tightening the screw down, using a shop crane hoist and drive. The drive went under the screw and employee's right middle finger was contacted between the wrench and the drive, causing a laceration to the finger.

The employee was changing skirt rubber on the surge tunnel conveyor.He was carrying a piece of angle iron into the tunnel, lost his footing and fell backwards. It was propelled forward as he fell backwards. As the angle iron fell, the end closest to the employee struck him on the right foot , penetrated his boot and lacerated the inside of his foot.

EE's left hand finger was crushed between pry bar and a metal skirt, while using a pry bar to clear a rock out of the plant feeder at conveyor #2, receiving ten stitches.

EE WAS CLEANING AND INSPECTING THE SHOP GREASE PUMP WHICH HAD BEEN LEAKING GREASE AROUND THE PISTON. HE TOOK THE GUARDS OFF TO FIND THE LEAK, WHEN THE PISTON GRABBED THE RAG IN HIS HAND. WHEN ATTEMPTING TO FREE THE RAG THE PISTON CAUGHT HIS FINGER, LACERATING A 2" LONG PIECE OF SKIN.
