EE was installing the hammer of the PC490. The employee was using a bar in a swinging motion to push the pin out of the boss on the stick. The pin was almost out as the employee was swinging the bar toward the pin. The pin fell out and the PC490 stick pushed downward on the bar causing the end of the bar to rapidly move upward striking the employee's head.

EE was dumping metal 55 gallon trash can into dumpster. Dumpster height is 32 inches. EE described lifting the trash can up and feeling a pop in right bicep.

During screen cloth change the employee was using a pry bar to maneuver the screen cloth. The pry bar became stuck in the screen cloth opening. The employee used a hammer to hit the pry bar, where the bar goes into the handle. They struck upward and away from themselves. The pry bar flipped up striking the employee on the left eyebrow causing a laceration.

Employee was a passenger in a service truck driven by another employee that struck a concrete footer that supports the primary discharge conveyor on 3/10/2015. At the time employee felt a pain in his neck. He requested medical treatment on 5/18/2015.

Two workers were setting up the crane to make lifts on a crusher. The operators cab was raised to let snow & ice slide off the top glass. The cab stuck in the up position. EE kneeled down & reached under the cab & loosened a hydraulic hose to lower the cab. The cab gently lowered onto his back and held him for a few minutes until co-worker was able to lift the cab to let him out.

The employee was using a Dye Grinder to cut a piece of metal from a pressure washer. Possibly, the grinder bolt caught the parabolic metal putting the wheel in a bind. The metal became a projectile and hit the employee in the arm and face causing open wounds needing medical care.

Employee's left index finger was lacerated while replacing Cat 990 loader bucket segments using his left hand to hold the bolt from spinning instead of using a bar. The bolt cut through the Kevlar glove, receiving seven stitches.
