EE states that while pulling water hose to back to hose reel on water truck that the EE's foot hooked a frozen clump of material, causing a fall to the ground on EE's left side. EE was seen at urgent care and was issued restrictions due to limited mobility and pain in left shoulder and left thigh.

Suspected aggravated workplace injury following comment of back soreness at the end of shift employee went home, returned following morning and stated he was fine. Later in the day complained of pain again and was sent to doctor. Diagnosed with sciatica, employee commented it was that same type of pain as he had several years ago when he got a shot in his back.

Employee was installing stump pan bolts using an impact ratchet when a piece of foreign material blew into his right eye. Employee received an abrasion on the right eye. Employee did not miss any time or require restricted duty.

Employee fell from dozer arms landing on the ground resulting in a dislocation of 4th finger. Employee was exiting the right side of the dozer using both hands on the hand rail while climbing down, his foot slipped and the employee jumped backward and landed on his left hand.

POWER CREWS WERE CALLED TO REPLACE A GSCS BLOWN TRANSFORMER. CREW MEMBER & MINE EE PROCEEDED TO OPEN A 12KV AIR SWITCH LOCATED APPROX. 7'FROM THE GROUND. A 9'LADDER WAS POSITIONED UNDER SWITCH HELD IN PLACE BY THE MINE EE. CREW MEMBER OPENED THE SWITCH & WAS ATTEMPTING TO INSERT A HOLDING PIN WHEN THE HANDLE SWUNG UPWARD STRIKING HIM UNDER THE CHIN.

EES WERE TAKING A V-BELT OFF THE DRYER DUST COLLECTOR FAN. THEY WERE MOVING THE B-BELT FROM ONE DRIVE TO ANOTHER WHEN EE FAILED TO REMOVE HIS HAND AND THE V BELT CAUGHT TWO FINGERS IN THE GROO VE.
