While shimming the jaw, EE reached up to grab the hoist, and some loose fine material fell and got behind EE's glasses and got in EE's eye.

While driving a haul truck, associate ran onto a berm. Driver was shaken by the incident & we decided to take EE to a panel doctor. The doctor wrote EE out of work to ensure the EE was well recovered.

Two associates were handling a handgun when it accidentally discharged injuring both associates.No serious injuries occurred. Both associates were transported to the hospital and subsequently released.

Associate was climbing onto a 980g loader. He was climbing using both hands but was wearing cotton gloves which are very slippery on smooth metal. He was on the second step when his hand slipped and he fell backward approximately 36 inches to the concrete floor landing on his bottom.

Employee was pulling waterhose to spray-clean equipment. This led to a twisting motion as he was jerking the hose forward. He twisted his left knee and felt a pop as he was pulling the hose.

While fabricating steel column. Plate that was tack welded on wrong location was being removed with a 2 pound hammer. The hammer slipped out of one man's hand and went about 10 feet and struck another man on the forehead.

THERE WAS AN UNGUARDED OPENING NEAR WHERE THE BASE OF A LADDER WAS SET UP. THE INJURED EMPLOYEE CLIMBED THE LADDER BEFORE HIS CO-WORKER CAME BACK TO THE WORK LOCATION FROM LUNCH. THEY HAD BEEN HOLDING THE LADDER FOR EACH OTHER. AS HE CLIMBED THE LADDER, THE BOTTOM SLIPPED & FELL THROUGH THE HOLE & HE FELL THROUGH THE HOLE TO A PLATFORM 12' BELOW.

ASSOCIATE WAS TAKING FEED BOX TO THE CONVEYOR W/CRANE. THE BOX WAS BEING PUT ON THE GROUND. WHEN CLOSE TO THE GROUND, HE PUT HIS LEFT HAND ON IT TO STEADY THE LANDING. HIS HAND WAS IN THE WRON G PLACE. WHEN BOX LANDED IT SHIFTED. HIS HAND WAS PINNED BETWEEN BOX AND BEAM.
