EMPLOYEE WAS ATTEMPTING TO REMOVE STONE BUILDUP FROM UNDER SHAFT KILN CARRIAGE COUNTER WEIGHT. HE CLIMBED OVER A GUARD INTO RESTRICTED AREA WHERE THE WEIGHT WAS LOCATED ONLY INCHES OVER HIS HE AD. THE WEIGHT MADE ITS SCHEDULED DESCENT AND EE WAS TRAPPED UNDER IT.

Temp was terminated due to positive drug results.

On 10/15 EE notified of leaving ER & due to sciatica. After having pain on 10/15 while outside of work EE believes it is related to near miss on 10/11. On 05/04/2017, Beitzel Corporation employee, was using a 6' grinder with a cutoff wheel while working to cut metal as part of a skirt skid fabrication project at the Carmeuse Lime & Stone processing plant in Strasburg, Virginia. While cutting, the grinder appears to have caught in the metal, kicking back and striking ee in the left leg just above the knee.

Top of power pole was observed as smoldering with intermittant flames. Contacted local fire department and power company. Fire was approximately 12-16" in diameter at the top of the pole and did not travel down the pole. Approximately 3 hours elapased before it could be verified as 100% out. MSHA hotline called at 11:32 following inability to confirm it was out beyond 30 min.

Employee stepped into a pile of lime and water. Minor burns to both ankles noted, first aid provided. Employee refused further medical treatment. Two days later employee wanted to be seen by doctor and was told diagnosed with 2nd/3rd degree burns.

Injured was helping to install a new 30" conveyor belt at the HiCal crushing plant. Injured had put clamps on the belt in order to pull the belts together when one side hung up on part of the frame of the conveyor. Injured lifted up on the chain that was hooked to the clamp and pinched his right hand between the clamp and the angle on the conveyor.

Employee and co-worker were attempting to wash out #2 ram at the preheater due to a fusing issue. While washing with high-pressure water, a mixture of lime and water splashed back into employee's face and eyes. He was wearing safety glasses and a faceshield at the time of injury.

A fire in the plant was noticed. Gas line was observed leaking and on fire. Columbia Gas was called and informed us to let it burn until someone came out. 911 was then called. Columbia Gas then showed up at 6:15pm and turned the main valve off. We found the gas line leaving the pre-regulator clamped to a piece of angle and it was deteriorated at this point which caused the leak.

Employee was cleaning the pre heater. When he finished the task he felt some discomfort in his lower back. He requested medical attention the following morning. He was diagnosed with a lumbar strain.

Employee #1 was holding the slug wrench in place while employee #2 was hitting wrench with a hammer. The hammer hit employee #1's left middle finger. Employee was diagnosed with a fracture to the tip of finger. He received a tetanus shot, glue for the laceration, a splint and an antibiotic prescription.

Employee was hitting the clogged chute and pinched his ring finger between the moving sledge hammer handle and the vibrator. He felt the impact but thought it insignificant. A few minutes later he noticed some blood on his finger. No problems with the injury until 12/28/10.

The employee was involved in snow clean up at the rotary kiln tunnel. We had 24 inches on the ground and it was still snowing. The loader cleared the area which compacted the remaining snow on the ground and made the area slick. The employee slipped on the snow and struck his left knee on the ground. Employee sustained a knee sprain.

Employee was using a pipe wrench to tighten a fitting next to the 02 analyizer. The wrench slipped off the pipe causing the employee's right arm to contact the flu gas discharge pipe which was hot. The employee sufferd 2nd degree burns to his arm and hand.

EE was placing a container of hydrated lime on an overhead shelf, the container fell striking the employee, the lid came off and lime splashed in EE face and eyes (EE was wearing safety glasses) burning cornea of right eye. This resulted in two days lost time.

EE was removing wet material blocking an airlock using a 2.5' pry bar (wearing heavy gloves). While gouging out the material his left ring finger struck against a connection flange. This resulted in sutures which made it a reportable, no lost time accident.

Reparing water line with two shop EE's who were using a bar (~15lbs.)to raise line, the bar slipped and struck the EE on the right shoulder. The foreman was aware but EE did not think he needed medical attention. Monday when EE reported to work with pain in right should was sent to medical treatment. This resulted in MSHA reportable.

Closing lid on pneumatic trailer using a 8' pole from catwalk above loadout chute. Leaned over the handrail to reach lid, it was stuck requiring extra force. He felt a slight pull in lower back but continued to work. Reported injury on 4/07/06 but did not seek medical attention until 4/11/06.

EE was standing on engine compartment of drill engine-EE removed panel-EE stepped on raised side of engine-compartment panel fill causing the EE to lose balance. EE fell approximately 6.5 feet. EE received fracture to left hip and lacarations to left elbow.

Ee rolled ankle when bolt broke from mushroom damper. Ee was bracing himself, standing on conveyor walls of the chute. Ee left foot was forward, right foot behind. Left foot was used to apply pressure to break the bolt. When bolt broke, ee weight shifted to right foot causing his ankle to roll over on the chute wall.

EE tripped getting away from a back flow of dusty material from a clean out port and fell to the catwalk upon his left knee. EE was using an air hose to remove blockage in the feed chute to the Gayco.

EE received an electrical shock when he pressed a reset resulting in 2nd degree burns. Section B is not completed since the matter was already immediately reported, in good faith, to MSHA and based on the medical information we have received to date, does not appear to be a serious injury as defined by MSHA regulations.

Employee was exiting right side of a dozer to remove a rock between dozer arm and sheild. Employee was using both hands on the hand rail while climbing down onto the dozer arm. His left foot slipped and he jumped backwards and turned landing on his left hand.

EMPLOYEES WERE ATTEMPTING TO MOVE AN ID FAN MOTOR. ONE WAS HOLDING A 2' LONG 4X4 WOODEN POST WHILE KNEELING ON THE FLOOR. ANOTHER WAS USING A SLEDGE HAMMER TO ALIGN THE MOTOR WITH THE ID FAN. A 10'X4" HEAT SHIELD WEIGHING OVER 100 POUNDS FELL FROM THE IMPACT ONTO THE FIRST EMPLOYEE'S LEG.
