Employee was stepping down to the catwalk from the conveyor after washing out the lime chute. As EE was stepping down, EE lost footing and slipped. As EE attempted to catch self, EE grabbed the metal structure of the baghouse with right hand. This caused EE to extend and twist the right shoulder. ER diagnosed with contusion to the right shoulder.

After clocking out at the end of his shift, employee decided to stop at the bulk diesel tank to pour the remaining 1-2 gallons of fuel treatment from a 25 gallon keg into the bulk diesel tank. While on top of the tank pouring the material in, the employee slipped and fell to the ground striking his head. This resulted in a laceration requiring 9 staples and a concussion.

A steel bar had been inserted under a conveyor belt to hold it up while cutting a portion of a metal plate. When attempting to pull the bar out the opposite end slipped off the conveyor frame resulting in the weight of the bar pinching employee's finger tip.
