Employee was using a grinder to prepare surface when a fragment of the wire wheel struck EE in EE's eye. Employee was wearing EE's PPE for the task which included Hard Hat, Safety Glasses, Face Shield and Gloves. Employee used the Emergency Eye Wash station on site and after the rinse EE still felt that there was more in the eye. Sent for medical evaluation

3rd shift loadout operator struck right shin on gate while exiting the top of a hopper car from the ramp. Operator slipped on the ramp causing the fall. Right shin contusion diagnosis on 4/18/25.

Miner was dismounting from passenger side of a tractor trailer. Miner turned to climb down and missed grabbing the handle on the truck. This caused Miner to lose footing and fall to the ground. Miner struck knee on the ground in the fall, causing the tendon to separate from the kneecap.

Operator stepped off slurry car onto a nut that is part of the bracing of a wall. Operator rolled ankle and went to ER on 10/23/24 and was diagnosed with an ankle sprain.

Drilling steel with a reamer. Reamer caught and spun the drill around.

2nd Shift Packaging Operator went to adjust film in stretch wrapper and while pulling the film EE felt a sharp pain in EE's back.

Operator went to grab falling stack of pallets (~20) that were loaded on forks. When EE reached through the roof of the forklift the pallets lacerated EE's 3 middle fingers and EE received 13 stitches.

In the process of dislodging wedged forklift forks from underneath a bulk bagger, an operator struck the steering wheel of the forklift with chin, causing a chin laceration with 2 stitches.

1st shift packaging operator was packaging 50 lb bags. While twisting with a full bag employee strained right knee. The operator was removing a full bag from the spout and turning to stack the bag on a pallet on a turn table.

A packaging operator was standing on the platform at the back of an empty slurry railcar when EE lost EE's footing, hitting EE's right arm on the break wheel of the railcar. The operator sustained bruising to the upper part of the right arm. The operator was prescribed antibiotics and a Tetanus shot.

2nd shift packaging operator reported hurt elbow to lead supervisor. There was not a specific incident that caused elbow lateral epicondylitis.

2nd shift packaging operator reported right shoulder injury (pain) from hanging bulk bags that has been ongoing for several weeks. Restrictions of 5-8 hrs of driving and no more than 10 lbs lifting. Occasional bend, squat, and twist. No climb.

The employee was turning to leave the wet lab and strained right knee

After checking fluid levels, an employee twisted their knee when leaving the work area due to their foot being wedged between a pump and some piping.

Employee felt pain in wrist due to occupational illness associated with using forklift levers.

An employee was moving 50# bags from one pallet to another and felt pain in their knee.

Employee was positioning an empty bulk bag neck onto the fill spout when employee felt pain in shoulder.

Employee felt pain in EE's back when EE was transferring 50# bags into a bulk bag.

Employee was moving a pallet when EE felt a sharp pain in their shoulder.

Sparks from grinding activities above EE got into EE's eye. EE was wearing safety glasses.

An employee was bit by an insect on the elbow while putting EE's uniform shirt on, causing EE's arm to swell.

An employee was removing a bag jam from the roto packer palletizer. When EE lifted the bag and turned, EE sprained knee in the process.

Mechanic was walking down stairs and felt a slight twist in knee but completed the shift. After the weekend they came back to work and knee was feeling sore after working/climbing stairs for two hours.

A contractor pinched the tip of ring finger, resulting in a fracture. The contractor & another worker were ascending in a scissor lift when one of the worker's (the rider) fingers was pinched between the lift railing & a wrench that was positioned on one of the mill shell bolts for Px15B.

Employee was cutting a piece of metal with an electric grinder when the cutting disk bound up causing the grinder to kick back and cut the top of their hand.

An employee wearing gloves was manually rotating the FD #1 back mixer to inspect the paddles. While rotating the equipment the cord for the radio got caught on a paddle. When they went to untangle the cord one of the paddles pinched a thumb between the housing.

An employee went to clear a bag that was stuck in-between the discharge conveyor belt and idle roller on Roto Packer C, while the equipment was energized and still moving, in clear violation of procedures . When EE tugged on the bag it dislodged and pulled EE's hand and arm into the pinch point. The employee received bruising to EE's hand and arm.

An employee was hit on the back of EE's right shoulder by a guard beam for a rail loading dock. The injured employee was in a crouched position tending to a portable generator located adjacent to the beam, when another employee inadvertently dislodged the guard, causing the north side of it to fall -12 inches onto the employee below.

Employee was getting onto a forklift by pulling self up with two hands (one on the steering wheel and one on the handle). In the process, the employee felt pain in EE's shoulder.

Due to repetitive reaching (calf raises) to tie 60 inch bulk bags while the bag was in the raised position, an employee received a muscle strain related injury to EE's calf.

An employee stepped down from the boxcar loading platform, to get into position to close the boxcar door. When ee went to step, left foot landed on a tool, twisting ankle in the process.

An employee squatted down to remove a valve cap on a slurry car. When EE stood up, EE felt a "pop" in EE's right knee and it started to hurt.

An employee was pushing a 55 gallon used grease drum with a drum dolly. While pushing the drum, the employee felt a sharp pain in back.

An employee operating Rotopacker C, noticed that 1 of the 3 belts on the bag unwinding radimat was falling off the idle pully. The employee attempted to fix it alone by tugging on the belt with left hand, while at the same time 'jogging' the belt with the manual button using right hand to try and 'walk' the belt back onto the idle pully.

Inadvertent movement of load crushing contractors fingers in wire rope rigging resulting in the amputation of pinky finger.

After an employee picked up a bag of concrete and turned to walk, their knee "gave out" causing pain.

An employee was clearing a frozen slurry line using a steam hose. The frozen section released suddenly, creating back pressure, allowing the created hot water and slurry to spray out of the pipe opening and onto the employee's hands and arms.

Employee was going to hose down floor, EE went to open the valve for the hose water but mistakenly opened a hot water/steam line. The hose that is attached to this line was permanently open on the outlet end. The hose whipped around when it became pressurized, spraying the employee in the face, shoulder, back, leg and neck.

An employee was cleaning up a small oil spill underneath the Deag C blower. Employees' right index finger got caught on a corner of a metal shim, resulting in a laceration requiring 6 stitches. The employee was wearing nitrile chemical resistant gloves at the time.

Individual sat on a piece of vertically protruding pipe. Individual finished shift. Was sore the next morning and went to the doctors. Minor bruise.

Employee welding pipe in an awkward position, causing the employee to get a strained neck.

While an employee was using a parts washer, the hose for the brush disconnected, spraying the cleaning solution onto the employees torso and face. Some of the solution went behind his safety glasses and into his right eye. The employee received eye irritation with no damage.

Individual was driving a fork truck down a narrow pathway. Individual caught sleeve on protruding screw head. Individual pinched arm between fork truck and a wall.

Individual was lowering a shaft guard vertically and it came down on his foot. Individual was wearing steel toed protective footwear. Minor bruising on right foot.

While welding in the shop, a 4inch x 4inch by 1/4inch thick piece of angle iron was being welded to a 6inch x 6inch by 1/4inch thick base plate. While trying to square this 4foot piece of angle iron on the base plate, it tipped over. The individual received a cut to his ring finger on the right hand.

Employee was inspecting Sedigraph unit 803 due to error message. While inspecting the Sedigraph unit the employee put hand on mixing chamber which was extremely hot. This resulted in first and second degree burns on 3 fingers on right hand.

Employee lifted dome cover to air inlet. Employee did not lift the lid all the way open (the lid was balanced half way open). Employee went to put dangling chain for inlet back inside by valve. The dome lid came down on employee's thumb resulting in fracture of left thumb.

Contractor working with Mill operators attempting to clean heat exchanger connected a water hose to a diaphragm pump to see how pump worked. Contractor did not see pump function, so he went to disconnect hose and the hose came off striking him in the upper chest and chin.

While using a right angle grinder on a flange that had been cut off. individual lost control of the grinder. Individual received a cut to his right forearm.

Employee was removing a crane hook from a clevis when the clevis fell on his left pinky finger. The incident resulted in a broken pinky finger.

Employee was moving a metal framed cart, when his right pinkie finger was crushed between it and a stationary object.

Employee was removing bolts when he felt a pain in his right shoulder. The injury was first reported to the employer on 3/20/14 as a "record/report only". The employee decided to see a doctor on 3/28/14; where he was then placed on restricted duty and thus became an MSHA reportable.

Employee was stepping down from an elevated platform to retrieve a hose. His left foot sank into mud/fines, causing him to pitch forward and fall to the ground, spraining his left ankle in the process.

Employee was on the elevator when pulling down saftey cage. Employee felt a pain in his lower right back halfway into the pull. Employee was placed on alternate duty by physician.

While miner was placing a welder on a pallet, miner pinched right thumb between the welder and the pallet.

Employee was closing a door to a boiler while on a 4' step ladder. The employee felt a pop behind his right shoulder. The employee's diagnosis was a muscle strain, Right upper back, cervical spine.

While demoing a hopper, the individual pushed a cut piece of steel out of the pile. Glove got caught on the material and subject banged his arm.

Employee was transferring weight from his left foot to his right when he felt a sudden pain in the bottom of his left foot.

Individual was attempting to loosen a screw conveyor, while going through equipment checkout. procedure was to pull towards them with the pipe wrench. The screw would not loosen, subject attempted to move it in the opposite direction. The pipe wrench slipped off and individual fell forward injuring his arm with a minor laceration.

Employee fractured tip of left thumb, struck with hammer

Worker was moving scrapped high bay luminaries from rear of storage trailer to front of trailer. He was stacking luminaries on top of each other & didn't use proper lifting techniques & used a twisting motion of his body reaching to pick up a luminaries. This aggravated a year old herniated disk injury. Later in the night he went to Emergency Room for first aid to relieve pain.

Employee was carrying two 2.5 gallon buckets of oil when he felt pain in his lower back.

Worker was detaching small utility trailer from 2" ball hitch on SUV passenger vehicle. Terrain, vehicle and trailer position prevented trailer jack from clearing hitch ball by < 1/2" He decided to lower ball by standing on vehicle bumper placing foot directly behind ball. Trailer released from ball and shifted toward vehicle pinching workers foot between bumper and tongue.

Employee was removing frozen stone from his trailer, a chunk came loose and hit him in his right leg. He reported it to the office and said it was scraped and bruised.

Individual entered the rear of a one ton rack body truck containing a steel frame leaning against the East side of the truck rack. Individual was previously advised and trained not to do this. No damage to the truck or frame. Individual sustained a compound fracture to the lower leg.

Employee felt pain in R shoulder as he pulled himself up from a sitting position using a railcar handrail.

Employee fell into wall behind him onto right shoulder.

Employee stepped into area to place bulk bag straps onto forks so bag could be lifted. Forklift operator drove onto his right foot.

While opening top hatch on railcar employee crushed R index finger when caught between railcar dome and hatch.

While loosening a bolt, the wrench slipped and employee cut/bruised back of right hand

While cutting banding from a pallet of bags using a utility knife, the worker slipped and cut his left index finger.

While pulling basket filters from ball mill sump filter employee strained left wrist and back.

Employee slipped from the footstep while mounting a forklift and struck his abdomen on the chair rail causing a contusion.

While walking under ball mill, employee slipped on media and fell onto his back causing a strain to the lower lumbar area.

While loosening a bolt using a breaker bar employee felt pain in his lower left rib. The pain continued and he sought medical assessment and received a work restriction of "light duty-limited lifting".

Employee mistakenly lifted slurry loadout spout while still in use, pinching left index finger and middle finger between the spout and the railcar cover, resulting in 12 stitches.

Hit handrail post with elbow while picking up empty bags in rotopacker area causing a contusion.

Employee was on the 1st floor PX 7000D mill. He was on a boom lift replacing the structural bolts by using a torque wrench to tighten them up. When he was tightening the first bolt the wrench fell off the bolt while he was pushing down causing his hand to be in the middle of the concrete block and the wrench.

EE was lifting a vic groovier head from an employee's box to a pallet about 31 feet in distance. As soon as he placed the vic groovier down he felt pain in his groin.

Employee was climbing down platform at end of railcar and missed the last step, striking his right side on the railcar and causing a bruise. 3 days light duty.

Cut palm of left hand with utility knife while trying to cut rope.

Employee was cutting stainless plate with a carbon arc torch while standing on scaffolding. He noticed that the protective tarp was starting to smolder and catch on fire. He jumped down the 3 feet to the ground to put out the tarp. While doing so, he landed badly and hurt his knee.

Employee lifted a pallet onto a bulk-bagging machine and felt a muscle pull in his groin area.

Employee was working in the fab shop grinding. He slipped and the grinder hit his hand.

Employee was swinging a hammer to loosen material & struck a flange with his hand, resulting in a pinch/laceration requiring sutures.

Employee was dragging a 3" diameter vacuum hose when he felt pain in his abdomen.

Employee bent over to lift up boxcar cargo net. Employee was unaware that the cargo net was caught on the ramp and could not be lifted. Employee strained muscle in back as a result.

Employee disconnected a camlock fitting on a 4 foot long, 3" line connected to a spray dryer unit and was sprayed with hot slurry. Having residual pressure on this line during shutdown is atypical and unexpected.

Getting out of his truck to go to work when a gust of wind started & something flew in his eye.

Employee was adjusting alignment of bag feeder when the bag roll slipped and pinched his arm causing a contusion.

Employee arrived at work in vehicle and parked his car. He stepped out of his car and slipped on ice, falling and bruising his rib. Restricted duty only.

Irritation / rash / possible chemical burn to forearms. Unknown cause.

Employee and one other were using a bar to lift a ~150lb object. Felt pain in his back 3 days later & then went to a chiropractor on his own.

Employee stepped down from a trackmobile onto a sloped area and sprained his ankle.

Climbing down ladder - thought he was on first step. Instead he was on second step. He fell and the pipe hit him in the ribs.

Employee was operating a wheel jack to inch a railcar into position. He felt his foot slip, followed by pain in his ankle.

During process of loading a truck, the employee lifted up on the fill pipe and felt pain in his back.

While standing up, washing totes, employee felt his right knee stiffen. The knee showed swelling later in the day.

Employee was walking up a gently sloping dirt berm & stumbled, catching himself with his left arm. Employee felt pain in shoulder a week later.

Employee was cutting a hose from Darbed fitting water running down everything. Knife slipped & employee lacerated the palm of left hand. 4 stitches required.

Employee was walking through mill area when he stepped on some loose mill media and slipped. While trying to keep from falling, he felt a "pop" in his knee.

Employee was drilling 1/8" plate steel when he stepped on a piece of steel on the ground. The corner came up and lacerated the right calf. 5 stitches required. No lost time.

Employee was putting a filter back into service after filter sock change. The filter lid gasket leaked upon the introduction of slurry product. Hot slurry released onto the back of the employee's head/neck/behind ear area. 1st degree burn to approximately 6" x 6" area and 2nd degree burn to approximately 1" x 1" area.

Employee was walking on a paved roadway (for exercise) during unpaid lunch break. Employee tripped on pavement resulting in laceration to eyebrow.

Pain in both wists due to repetitive motion. Unknown cause, unknown date of occurrence. Employee feels that the cause is related to his bagging job in early 2006, but cannot provide specific information.

Employee working inside hopper car hanging netting. While on a ladder, the ladder slipped down the side wall of the hopper cars. Employee fell approximately 4 ft with the ladder, landing on top of the ladder.

While fitting one section of pipe to another, the employee pushed upwards on the pipe, straining his back.

Employee operating overhead hoist. The control pendant slipped from his grasp and struck his knee causing a contusion.

While uncoupling rail car, the employee pulled up on the release handle and strained lower back.

Bending a stick of 3/4" IMC pipe, employee had full body weight on the pipe when it slipped and hit his left arm and shoulder.

Employee bending over to close bottom gate on a hopper car. Employee felt back pain.

Employee bent over to pick up a piece of equipment. While bending down, the employee felt a pain in his lower back.

Employee was stepping down from a step (approx. 10" high) and strained his right calf muscle.

Employee was removing a floor grate in order to clean the location below the grate. While lifting, the grate slipped from his grasp and fell onto his foot causing a contusion.

Employee leaned over a conveyor to examine a broken drive chain. While leaning over, he placed his hands onto the conveyor rollers. A photoelectric sensor sensed a presence and activated the rollers, pinching the employee's left fingers & hand between the rollers (compression injury & laceration - 8 stitches).

Prior to shift start, employee was walking through plant to get to work area. Employee rounded a corner and slipped on spilled media. Employee fell to floor injuring a tendon in his right calf.

While lifting a bucket containing material, the employee turned to empty contents of bucket onto a screen. The employee reports that while doing this, his knee "snapped". This injury may be an aggravation of a knee injury occurring on 11/17/2004.

EE was operating a forklift & loading material onto an empty trailer. Upon leaving the dock & entering the trailer, the trailer "dropped" below the level of the dock plate. The automatic dock plate, sensing no trailer was present, retracted & returned to home position. Unknowingly, EE backed the forklift out of the trailer & into the retracted dock plate.

EE WAS DISMANTLING COWLES MIXER BLADE FROM A SHAFT. EE WAS USING A WRENCH AND SOCKET WITH RATCHET THAT WERE NOT THE CORRECT SIZE FOR THE JOB. THE BOLT CAME LOOSE, EE'S HAND SLIPPED AND HIT THE BLADE.

A PIECE OF WOOD/DUST FROM PALLET FLEW INTO EE'S EYE. EE HAD EYE PROTECTION ON.

EE WAS HAND-SPOUTING EVERY OTHER SPOUT ON ROTO PACKER WITH PLASTIC BAGS. EE FOUND HIS ARM/SHOULDER TO BE SORE WHEN SHIFT WAS OVER.

While hanging bulk-bags, employee experienced sharp pain in side.

Slipped on track taking samples from under railcar and injured knee. This accident was also reported on submission #116554 but the employee's name and information was not included.

While pulling on hand lift, twisted and strained knee.

While employee was standing on a piece of dewatering equipment trying to free a bound rotating assembly with a bar, the bar slipped and employee lost his balance, fell and broke a bone in his leg.

While working with SBB employee slipped and hit his elbow on ground.

Employee was talking with forklift driver, driver backed up and ran over his foot.

Employee slipped onto energized roller, hand got pulled in between two rollers. Full pallet proceeded down line and pinched leg into roller bracket.

Employee reports wrists and arms are numb and ache, especially at night. Doctor feels the repetitive motion may be a cause of the problem.

WHILE CLEANING PIN MILL DUST IN AIR GOT INTO EMPLOYEE'S EYE. AT THE TIME EMPLOYEE REPORTED THAT HIS EYE WAS FINE. FOURTEEN DAYS LATER HE SAID HIS EYE WAS HURTING AND GOT MEDICAL TREATMENT. REC EIVED EYE DROPS AND NOW SAYS HIS EYE IS FINE.

WASHING HANDS WITH SOAP WHICH CONTAINS GRIT. GOT SOME OF THE GRIT IN HIS EYE.

EE WAS CUTTING A BAND ON A CARTON OF PAPER. WHEN BAND BROKE, KNIFE SLIPPED AND STRUCK OTHER ARM.

WHILE BENDING, TWISTING, AND PULLING VALVE OUT OF TIGHT QUARTERS, EE FELT PAIN IN BACK.

EE EXPERIENCED TENDONITIS IN HIS LEFT WRIST WHEN HANGING SEMI-BULK BAGS.

EMPLOYEE WAS GETTING OUT OF HER CAR AND SLIPPED ON ICE HIDDEN BY A LIGHT COVER OF SNOW.

EE FRACTURED WRIST WHILE LIFTING PALLETS.

EE WAS CHANGING FILTER SOCKS, DID NOT TURN OFF DRAIN VALVE. WHEN HOT WATER WAS TURNED ON HOT WATER WENT DOWN EE'S BOOT.

EE JUMPED TO GROUND FROM BACK LANDING OF RAILCAR. WHEN HE LANDED HE FELT STABBING PAIN IN RIGHT SIDE OF BACK.

EE LIFTED 50# BAG OF PRODUCT OUT OF ROTOPACKER B. FELT PAIN IN BACK.

TWHEN FORKLIFT OPERATOR WAS PLACING SBB ON PRESS, LOOP GOT HUNG UP ON OUTSIDE BOLT. THE OPERATOR ASKED AN EE WASLKING BY TO HELP FREE LOOP. FORKLIFT OPERATOR'S FOOT SLIPPED OFF BRAKE PINCHING EE S HAND BETWEEN PLATE AND FORKLIFT FORK.

WHILE HANGING SEMI-BULK BAGS, BOLT LOOSENED AND RAMP COLLAPSED. EE FELL FROM RAMP.

EE HAD BEEN WORKING WITH 3 INCH HOSE FILLED WITH MATERIAL. HE SET THE HOSE DOWN & TURNED TO TALK WITH ANOTHER EE. AS HE TURNED, HE FELT PAIN IN MIDDLE & RIGHT SIDE OF BACK & NECK.

WHILE ADJUSTING HAND VALVE ON SLURRY FLOW PIPE, SLURRY BYPASSED A SEAL ON AN ADJACENT SLURRY FILTER. EMPLOYEE RECEIVED SECOND DEGREE BURN ON FACE AND NECK.

A HAMMER FELL OFF THE WALL AND LANDED ON EMPLOYEE'S FOOT IN THE BOXCAR HE WAS WORKING ON.

WHILE CLEANING DRYER SYSTEM, OPERATOR OPENED DISCHARGE CHUTE AND INHALED DUST FROM PRODUCT CONTAMINATED WITH FURNACE SOOT. SOOT CAUSED BREATHING PROBLEMS LATER AT NIGHT.

PULLING ON CHAIN FALL TO RAISE CENTRIFUGE HE STRAINED HIS SHOULDER/NECK AREA.

EE'S N ECK IS A LITTLE SORE. WENT TO DR. ON 2-22. DR. SAID FOR EE TO COME BACK IN ON 2-25. ON 2-25 DR. SAID TO REST FOR 2 WEEKS.

CLEANING UNDER PIPES WITH 1" HOSE, HARD HAT FELLOFF. EE STOOD UP, HIT HIS HEAD ON A VALVE ACTUATOR AND CUT IT OPEN, REQUIRING STITCHES.

WHILE ALLIGNING & UNLOADING APPARATUS UNDER RAILCAR IN ORDER TO DRAIN THE RAILCAR, A WHEEL BE- CAME STUCK BETWEEN THE RAIL AND THE UNLOADING APPARATUS, WHILE PULLING THE INJURED EMPLOYEE HURT HIS LEFT SHOULDER. DR. SAID HE STRAINED HISLEFT SHOULDER AND TOLD HIM TO TAKE IBUPROHEN AS NEEDED.

MOVING AN EMPTY SUCTION HOSE SLIGHTLY SO IT COULD BE ATTACHED TO THE PUMP ON ABOUT A 20 LB HOSE. WHILE MOVING THE HOSE, HE FELT A SNAP IN HIS RIGHT ARM.
