While using a track jack, the jack bar slipped out of the employee's hand and struck them in the chin causing a laceration.

The employee was standing on the roof bolter. While moving the hung miner cable by hand, they lost balance falling backwards hitting the drill mast, resulting in a 1-inch laceration on the right side of their neck.

On 08/30/2025 at 14:15 pm a roof fall was discovered in the intersection of crosscut 17 #2 entry in 12 right tailgate. It was determined at that time the roof fall was above the anchor point for the primary roof support. The Msha Hotline was notified of the roof fall at 14:22 pm 08/30/2025

Working in a 2-man crew, driller opened rod clamp while chuck was not engaged with 300 ft of drill rod in the horizontal borehole. Water flow from the borehole pushed the free rods backwards into the helper behind the drill rig, striking their right side and causing bruising.

On 6/27/2025 EE was hit by a softball size piece of coal coming off shearer drum. CT scan was negative, and EE returned to work. On 7/5/2025 EE was diagnosed with pleurisy. Workers Comp investigated alleged diagnoses and could not determine if it was related or not. They accepted the claim making the incident reportable on 7/22/2025.

Employees were loading a butterfly valve onto an underground truck. The valve shifted and accidentally rolled onto employee's hand that was on the bed of the truck. Employee pulled EE's hand out from underneath the valve causing injury to two fingers.

Surface Miner was walking to the forklift. EE stepped over something buried in the snow. When EE did, EE stepped on a belt bottom roller buried in the snow. EE fell back and to the side landing on a roller. This caused broken ribs on the left side.

Surface Miner was trying to close a damaged roll up door. The door was tweaked so EE was trying to raise one side in order for door to be level. The door dropped onto a crib EE had placed under the door. EE's thumb was caught and was fractured. No surgery was required.

The top in front of the end tailgate shield and in the immediate tailgate entry of the 11RT longwall section fell and restricted the airflow below the required minimum quantities. MSHA's hotline was called and a modification was approved to mine with low air flow.

At approximately 4:00 pm January 29, our longwall Tailgate had closed off enough that we needed to go on our Longwall Tailgate Blockage Plan. Field Office Manager was contacted. A field inspector came to the mine and issued a K-Order. We properly trained our LW miners and set up to mine under the TG Blockage and K-Order. The TG Blockage was due to an unforseen displacement.

EE could see a cable could be hit by the loader bucket. EE stepped alongside the bucket to quickly move cable. The bucket slid to EE's side and struck EE's ankle. EE received a fractured ankle.

Employee fell from the bed of a supply truck while pulling a roll of lay flat waterline off a pallet. Employee braced the fall with right hand and this resulted in a hurt shoulder. Employee was taken to hospital and X-Ray was negative. Employee did not miss any work. It was later found on an MRI that there was damage. Employee had surgery on 12/13/2022 making it a Lost Time injury.

An unplanned tailgate blockage at the tailgate drive area of the longwall face that impedes passage from the face and into the tailgate entry. K-Order 9157666 was issued and Skyline's tailgate blockage plan was put in place. Skyline's CFM delivered to the face, and the FPM on the face stayed in compliance. No additional action was required to continue mining.

Employee was throwing a crib behind a steel set leg and pinched right hand pinkie finger between the crib and the steel set leg. Employee was wearing cut resistant gloves. This incident resulted in a laceration to tip of finger and loss of nail.

While traveling into 7RT Bleeder, roof fall was found. The top at crosscut 18 had weakened and fell. The measurements are 20 feet wide, 20 feet long, and 10 feet high at the middle of the fall. Area is not wet. Area was bolted with 7 foot rebar resin grouted bolts. Ventilation was not disturbed.

EE was hand unloading face conveyor flyghts that were brought in to section in back of dodge truck. As EE was putting flyghts in jaws of the can setter, their index finger was pinched. This caused a fracture to the tip of the finger.

Water Supervisor found this roof fall at 6:30 am while investigating a water issue. This intersection was mined 5+ years ago. This intersection has little to no water in the roof strata. Roof condition in adjoining entries look good. This roof fall is not in a haulageway, travelway, or escapeway. It does not impede weekly examination, and does not affect ventilation.

EE was hanging bolter cable in the last open crosscut. Outby approximately 250 feet the 4 inch discharge water pipe came unhooked from the roof and fell. It dominoed inby striking EE on the shoulder. EE was taken to the hospital and a small hairline fracture was found. EE also has a deep tissue bruise. EE was given a full release to RTW on next scheduled shift.

Directly in front of shields 1, 2, and 3, the immediate roof fell. The roof fall area was approximately 20' X 20' X 10' high. This fall was from the shield tips to the already loose top that had sat down on the Longwall crusher and stage loader. An MSHA plan was approved for rehab on 5/31/2022

On 5/21/2022 EE slipped and twisted knee. EE was taken to the hospital and X-Ray was negative. EE returned to work and has continued to come to work. An MRI was 6/9/2022 and the results show a strained ACL. EE had ACL surgery 10 years ago and this incident has made it that EE needs Physical Therapy.

Injured was polishing a 22" long, 2" in diameter pin on the shop lathe. EE was using a 4 foot long piece of emery cloth. When the emery cloth was worn, EE choked up on one end of the emery cloth to have better grid on the emery cloth. As EE was working the emery cloth back and forth, EE got EE's thumb too close to the bar and the rotating bar pulled EE in and broke EE's left arm.

In a 48 hour timeframe we had a 350hp pump failure at XC87 an 200hp sump failure at XC5 in SM1. These failures allowed water to flow to the SM1 seals. Once the water roofed out in XC6 in SM1, normal ventilation was affected. A 103(k) order was written. As of 4/13/2022 water level has lowered to allow normal ventilation in SM1.

Employees were removing a Kennedy Stopping with a mandoor. Injured employee was holding the mandoor with right hand as the clips on the cross bars were being removed. When the clips on the panel next to the door were removed, the door fell to the side and cut the palm of the EE hand. EE was wearing cut resistant gloves and still received a cut that required stitches.

The shear cut through a fracture zone where sandstone is falling out 3-4', shield 15 to 27. The face sloughed out enough that a tall pile of material was between the conveyor and the face. There was minimal material in the conveyor. A rock fell out of the roof in the face and hit the pile of material. It fell at an angle that allowed the rock to come over the pan striking employee.

Bolter operator was pushing the end of a mat up to wire it in place so a bolt could be installed. Operator was pushing up with their head while wiring. The mat slipped off operator's hardhat and fell striking the operator. This caused a 1 1/2 inch laceration that required 3 stitches.

Surface miner was on foot under BC20 reclaim belt looking to see if the de-icer sprays were working. A small piece of coal fell from between the spill plate and the chain link fence and struck EE on the right ear causing a laceration. Miner was wearing safety glasses and hard hat.

A severe floor heave occurred 100' outby the LW face in the intake. In addition to the floor heave, the pillar rib burst. There were three miners at the end of the can line. One miner received fractures to their left leg. Another miner received stitches to the left side of their face. The third miner was check out and was fine.

While unloading a pump out of the back of a pickup truck, the pump slid to the side. The tip of the miner's middle right hand finger was pinched. This resulted in 8 stitches.

EE was using a handheld cordless bandsaw to cut rib bolt into 1 foot lengths. EE was holding the bolt with the left hand and was using the saw with the right hand. When the blade contacted the bolt, it rolled up the bolt and cut EE's hand. EE was wearing cut resistant gloves. Using the gloves greatly reduced the laceration.

Employee was in the process of installing the kickstand on the sloughing plate of the shear. Another employee had started to pull a staple to repair a leaking fitting. The employees were reminded to get the plate blocked. Work on the fitting stopped. Sloughing plate come down pinning the employee. The pinned employee received 2 staples in the back of EE's head.

EE was installing 7' roof bolt. Bolt was pushed into hole most of the way. The bolt bent so EE adjusted drill mast to straighten bolt. When EE tried to push it in again, the wrench broke and hit EE in the cheek. This caused fractures to EE's cheek.

Employee got out of the passenger side of a parked mantrip. The mantrip had parked on a piece of wood with the rear tire, leaving the wood leveraged up 4". The employee did not see the wood in walkway and got foot caught under it. This caused the employee to fall with foot still stuck under the wood piece. This resulted in a broken ankle.

EE was replacing a scoop push plate ram. Two miners were assisting employee. As the connection pin was being hit through the block and into the ram eye, the injured EE stuck a finger in the block hole to see if it was lined up. At that point the eyelet dropped a couple inches and the tip of the finger was pinched. This resulted in 11 stitches and the tip of finger fractured.

A fire at the tail of BC8 surface belt was found by surface employees. This fire was in the tunnel exiting the crusher building. Fire was out by 5:00 am. A distance of 135 feet of top belt and 135 feet of bottom belt was burned. All communication to and from the mine was lost. Was able to contact MSHA at 4:35 am by cell phone by leaving the mine property.

Two employees were pushing a 150 hp pump to the back of the bed of a dodge truck to unload it. The pump was top heavy and it tipped over and pinched the pinkie finger of the employee.

EE was trying to split a small wedge with an axe. EE held it flat against a set can and struck the wedge. It did not break so they lifted it up with left hand and with the axe in right hand. When they struck the wedge against the can a thumb was under the blade of the axe. EE received 3 stitches.

Roof fall was found during belt preshift. It had only fell 3' high. Examiner reported it to shift coordinator. When coordinator arrived with timbers and workers, it had fell up to 8' high, making it reportable. Coordinator had it reported. Fall did not affect mining, ventilation or escapeway(s).

Employee was standing by foreman. Employee fell over forward and died. Miners tried CPR and Defibrillator but could not revive them. We believe the autopsy will reveal a heart attack. This fatal is not mine related.

The rib was supported with mesh & 4 foot bolts.

EE returned to work. Elbow has healed but still has pain in shoulder. EE had an MRI done on 8/21/2020 and has began missing work 8/26/2020 due to pain in shoulder.

This area during advancement had been bolted with 7 foot rebar, resin grouted bolts.

Section mechanic positioned himself on the right side of the roof bolter to begin repairs on a bantam rockduster. The right side bolter operator finished installing two rib mesh bolts and swung drill mast in and back in order to move roof bolter. Left side bolter operator found mechanic caught between bolter frame and right side drill mast that was swung back in place.
