EMPLOYEE WAS CUTTING PIECES OF PLASTIC ON THE TABLE SAW WHEN ONE OF THE CUT PIECES VIBRATED BACK INTO THE BLADE. THE PLASTIC WENT IN THE AIR STRIKING HIM IN THE CHIN.
