On 2/27/26, an employee performing a routine inspection of Rotex Screens felt a strain in EE's left shoulder while reaching overhead to close a top door latch. Employee sought additional treatment on 03/26/26 and was placed on restricted duty.

8/5/24 employee stated drinking water (5 gal dispenser) tastes funny. Water jug is sent off to a lab for testing. 8/7/24 employee calls in sick to work, lab results for water jug states positive for E.coli. Employee does not want to go to the doctor, finally goes on Friday 8/9/24. Tests revealed neg. for E.Coli so it is unclear if the illness is from the mine site.

Employee was using a torque multiplier to remove a bolt from a front end loader axle when the multiplier gear slipped causing EE to jerk forward, injuring EE's L pectoral. Employee was seen at the ER on 8/06/24 and given a return to work with restrictions, pending an MRI. EE stated it did not bother EE until then. An MRI was scheduled for 8/20/24 where EE was told to stay off work

While shoveling sand and small gravel from the dryer, employee turned to dump the shovel load when employee felt a strain and pop in lower back.

Employee was working under the dozer when EE reached into the belly pan and was bit by a rattlesnake on the right wrist. EE was taken to the ER and given anti-venom and released the next morning.

Employee was changing out the floc bag using an overhead hoist. As the employee moved the new bag to the hopper, they tried to step on the pallet the new bag was on, but caught their foot on the corner, causing them to trip/fall and injure their left ankle. Status changed on 9.26.23

Employee felt irritation in EE's right eye during EE's shift until it got worse. Employee tried to flush EE's eye with no success and went to the hospital where they removed a metal shaving and gave EE prescribed medication. EE has no knowledge of working with metal that shift, but the shift prior, used a grinder. EE may have rubbed EE's eye.

While installing insulation over a access panel, the employee realized it was upside down while on a ladder and was going to climb down, to turn it around. A gust of wind caught the insulation cover and caused EE to become unstable on the ladder. The employee jumped to the ground approx. 44" and injured EE's left Achilles.

While driving a Komatsu WA500 Loader, an employee drove over a rough road and bounced in EE's seat, injuring EE's shoulder. When EE bounced, EE caught self on the way down with EE's R arm on the arm rest, which ended up hurting EE's R shoulder. The employee said speed was not a factor.

Employee was traversing from one platform to another when EE twisted their right knee. The employee was stepping over a chute and did not have a good area to step which led to the injury. The employee was checked by medical and released to full duty, but after a few weeks, employees knee grew more painful. EE was scheduled for surgery on 4/23/21, but worked fully up to that point.

Employee was assisting in closing a wheel handled, knife gate style valve. As the valve became harder to close by hand, the employees used a pipe wrench, gripping the shaft of the wheel handle. As they were handing the wrench back and forth, it became loose from their grasp, striking employee on the 2 front teeth, chipping them both.

Contract employee was stepping back away from a customer sand truck to look at the top hatch and tripped over a concrete footer. This caused EE to stumble back and come down on the right ankle then EE's body twisted and the foot did not, causing an ankle fracture.
