An employee was using the V-belts to rotate a pump by hand while troubleshooting. The employee's fingertip got pinched between the V-belts and the sheave, causing a small fracture.

Employee was repairing a poly pipe when some unconsolidated material from the hole employee was working in sluffed off. The material pushed employee's knee into the pipe causing pain. The employee was later examined by a doctor and was not given restrictions until 1/3/2025.

Employee was removing a pump impeller from shaft with a pipe wrench. While straining to breaks threads loose employee felt a discomfort in lower abdomen. Monday morning employee reported it to mine management and went to the doctor and they think it is torn lower abdominal muscles.

In removing the table of a VSI crusher the rigging was a little off center. When the table cleared the shaft it swung into employee sitting on the side of the crusher pinching their left foot/ankle between table and anvil ring.

In removing a drive belt guard a nut that was welded to the back plate had broken loose taking a piece of the back plate with it. When employee held a backup wrench on the nut and the impact was energized the wrench slipped out of employee's hand doing a 360 deg. turn slapping employee in the hand

IE and another EE were inspecting The VSI crusher. EE was on top pulling on the handle and as the door lifted IE was helping by pushing on the bottom of the lid. When the door was partially open IE thought EE had control and quit helping and the weight was to much for EE to handle. The door slipped out of EE's hand and fell onto IE's right hand.

Employee was using a water hose to wash off a slab when water sprayed back into their face. The employee noticed a slight irritation in the right eye shortly after. The condition worsened after they arrived home. The employee reported the incident the following morning. Employee was then taken to an urgent care provider were and was prescribed antibiotic eye drops.

A mechanic was installing a track pin with a hammer when a piece of the pin broke off and embedded into the employee's arm. The employee removed the object and was taken to urgent care where they received three stitches.

Employee was using a 5' long bar to lock a cam on a rail car door when EE lost footing and fell forward catching self with EE's hands on the ground. The rock bar fell to the tracks and leveraged the end up striking EE on the chin. EE received a 2" laceration requiring 5 stitches to close.

Employee was walking quickly up two flights of stairs on Station 4 when employee felt tension in the lower left back. After making it back down the stairs to a pick-up, the tension turned into extreme pain.

After seeing a lawyer on the 21st EE's pre existing, lower back, off duty injury became an on the job un-diagnosed alleged neck injury. We took it upon ourselves to modify EE's duty until the claim could be substantiated. Took EE to the doctor on the 23rd of Nov. where a workers comp. claim was started, and on Dec. 12th EE's injury claim was denied.

Employee was preparing rail car for loading after a light mist. When climbing into the gondola using the ladder on the end of the car his right foot slipped off a rung causing him to fall, landing on his feet on the ground below. In the fall his right hand struck a ladder rung on the way down breaking one bone in the back of his hand leading to his ring finger.

Injured was improperly riding a rail car in transit when the train came to a stop. He fell from the top of the car to the knuckle below, fracturing the small bone from his wrist to his little finger on his right hand.

IP was attempting to reach behind a guard and hit a troughing idler with a hammer. IP does not remember exact details but sustained an injury when his LT hand passed between a troughing idler "can" and the running conveyor belt. IP's LT hand was bruised and he was placed on modified duty.

While installing a pin on an conveyor slider bed, IP pryed up on the bed to line up the hole for the pin. As he was installing the pin, the bar holding the bed up slipped off, the pin shifted up, and caught the IP's first finger on his right hand between the bed and the pin. Injury required 3 stitches.

IP put too much tension on a belt wiper. IP decided to relieve some of it and in this process the wrench slipped out of his hand causing it to go 180 degrees around the tensioner and strike him on the right calf.

Aggregate filled the jaw to the level of the grizzly bars and the jaw stopped. The employee placed a rock on the pile on top of the grizzlies; the rock shifted and started to roll back into the jaw. He attempted to stop the rock and crushed his fingers between the rock and the steel plate between the jaw and the grizzlies.

Employee believes that the injury occurred while cleaning the plant at shutdown. When finished, IP stood up and felt a slight pain in the groin area. IP believed it was a muscle cramp that did not need medical treatment and the strain would subside with time. The pain subsided for a short period but returned. IP was taken to the doctor on 11/2 and placed on light duty.

IP was removing a damaged bed lift cylinder from a haul truck. As IP was cutting the polish rod with a torch, the rod gave way. The barrel end of the cylinder struck the IP as he was attempting to get away. The barrel came to rest across the legs of the IP. The incident resulted in a fractured right tibia and left knee.

Employee was changing screens on the wet plant at the secondary. While lifting the screen, he twisted with the object he was lifting. This caused a strain to the back and neck. Employee was later terminated due to failing a post-accident drug test.

EE was working with a crew on the primary feeder removing a timing gear. The crew was using a porta-power to pull the gear. While He was pumping on the porta-power pump, one of the bolts on the gear puller broke. As a result, the porta-power ram fell from the feeder striking him on the left arm.

EE was working on the 42? belt with a crew of two. The crew was removing broken idlers from the conveyor. As He was removing the second idler, the idler hung up on the conveyor frame. Another employee picked up on the opposite side of the idler freeing it. When the idler was freed, the inertia of him pulling on it caused a rapid movement forward.

A conveyor belt was being spliced at the primary plant. EE was placing a sheet of plywood between the belt and the troughing idlers. While pushing the plywood under the belt, He lost grip on the plywood and scraped his right arm on the sheet of plywood. This resulted in a splinter 1/4 inches wide by three inches long being buried in his forearm.

The employee was helping clean up a pile of scrap iron on 6/8/2007. He does not recall anything happening to have directly caused an injury, but he started feeling pain in his left arm some time later. The employee believes that the pain started while cleaning up the scrap metal. The employee reported the injury on 6/11/2007 to the plant manager.
