Employee was splicing a belt using a battery operated drill, while tightening one of the nuts down it bottomed out. The torque from the drill wrenched from their hands, the drill spun around and the battery struck the employee in the jaw chipping a molar. There was no bruising or laceration. They went to the dentist where a crown was installed on the tooth.

Employee spliced C1 conveyor belt and was tracking the belt when the laces came around and hit the strap that was holding the rear skirt rubber. Employee took a pry bar and pried up on the strip without locking out the conveyor first. Pry bar became wedged causing EE's left index finger to be smashed between the belt and the bar resulting in two lacerations on EE's finger.

EE was unhooking the bolts off the VSI and when EE was getting down instead of using a ladder like EE was asked to use twice EE just jumped down a 2 foot drop.

EE was raising the sand belt up on the Descander and the hydraulic cylinder on the plant wasn't strong enough so they had to help it go up and when it was lifted up they lifted it up too far and the linkage came out and it dropped and hit EE in the head.

While removing a hydraulic hose from the 980G Cat Loader employee's hand slipped and hit the metal next to the hose causing it to swell up and be sore.

Employee was helping a change the cutting edges on the 962G Cat loader, he tried to grab the head of the bolt, he didn't know it was sharp and when he took the bottom of the bolt out it cut through his glove and got his hand. The local emergency room give him a few stitches and sent back to work full duty no restrictions immediately following the visit. He has no lost time.

Employee went to the scale office to fill his water jug. He was driving back to the wash plant and next thing he knew the loader bucket curled down and dug into the road, he then guesses he went forward putting his chest into the steering wheel which he believes caused him to hit all controls forward and when he did that he curled the bucket more into the ground.

EE was loading liners for the loader bucket on the Belshe Trailer assisted by a co-worker. Co-worker was pushing and EE was pulling and when they finally gave and slid on to the trailer the liner sliced EE's finger open, he was wearing gloves. No stitches were needed the wound was cleaned dressed and he was sent back to normal duty with no lost time.

EE was at the bottom of the belt of the desander and his foot slipped and he fell on the belt and hit the angle iron that crosses the belt. **EE's injury took place on 04/22 but he did not report it to seek medical treatment until 04/25, he did not have any missed time from work.**

EE was cutting a conveyor belt with his box knife when his hand slipped and cut his leg. EE received 7 stitches and work release with restrictions (sedentary for next 4 days; no bending at knee for next 10 days) until stitches are removed 10 days later.

EE was changing loader teeth and according to co-worker who was working with him; EE took a hammer and tapped the loader tooth and a sliver of metal off the loader tooth penetrated his eye. EE was not wearing his safety glasses. We are uncertain of impairment at this time.
