While getting first aid treatment in the office, the employee fainted and broke his front tooth when he hit the desk.

EMPLOYEE ALLEGES THAT HE HURT HIS LEFT SHOULDER WHILE ROLLING A HYDRAULIC HOSE. EMPOLYEE WAS TREATED BY AN M.D. AND RETURNED TO WORK ON RESTRICTED DUTY AND WILL FOLLOW UP WITH HIS DOCTOR IN TWO WEEKS.
Employee states he slipped and bumped his right knee, two years ago and bumped again in March, but did not report the incident either time, until 6-15-09.

EE GETTING OFF LOADER, FOOT SLIPPED OFF THE STEPAND FEEL DOWN.
EE WAS TRYING TO LOOSEN A NUT ON A MIRROR HEAD FOR A TRUCK DOOR. THE NUT WAS VERY TIGHT AND WHEN HE APPLIED PRESSURE THE NUT CAME LOOSE (SUDDENLY) CAUSING HIS FINGER TO BE CUT BY THE MIRROR BRACKET. RESULTING IN HAVING TO HAVE 6 STITCHES.
