Employee was stepping over eight inch pipe and twisted his left ankle. Employee was examined by doctor and diagnosed was a sprained ankle. employee was returned to work with no climbing restriction and will follow-up with doctor on 8-8-12

EE WAS GETTING OFF OF HIS BELLY DUMP TO CHECK HIS TIRES WHEN HE SLIPPED AND FELL. TRYING TO BRACE HIMSELF WITH HIS LEFT HAND AND HURTING IT. EE WENT TO DR AND FOUND OUT HE BROKE HIS HAND. CAST WAS PUT ON HIS HAND. DR SAID FOR HIM TO STAY HOME UNTIL 11-10-00 AND RETURN TO WORK ON MONDAY 11-13-00.
